This inspection was carried out on 9th August 2005.

CSCI has not published a star rating for this report, though using similar criteria we estimate that the report is Good. The way we rate inspection reports is consistent for all houses, though please be aware that this may be different from an official CSCI judgement.

The inspector found there to be outstanding requirements from the previous inspection report but made no statutory requirements on the home.

What the care home does well

On entering the home the atmosphere was friendly and relaxed. The home was airy, clean and comfortable. There was a lot of chit-chat and laughter between service users and staff in the main foyer and communal lounges. Service users spoken to commented that they were satisfied with the overall service and accommodation. Comments included "Good company is important", "No one forces you to take part in activities", " I have had a nice bath, and feel ready to run twice around the block". Service users have the opportunity to take part in a range of activities inside the home and outside in the community. Records examined for the protection of service users during the inspection where found to be accurate and up to date.

What has improved since the last inspection?

Since the last inspection staff now have up to date information on medicines prescribed to service users. Staff now have the opportunity to discuss their care role and development with their line manager to extend their skills and knowledge enabling them to provide a good standard of service to everyone living in the home.

What the care home could do better:

There are still outstanding issues about relocation of staff room and extra storage provision that the company need to consider. Extra staffing to meet increasing dependency levels of service users needs to be considered by the company (Outstanding since the last inspection). The home should continue to ensure all care staff are equipped with the skillsand abilities to carry out a competant service to the people who live in the home.

Up to 4 persons with a physical disability over the age of 50 may be accommodated, commensurate with the home`s Statement of Purpose. 2. The home may accommodate a named individual: As set out in a letter to the registered person dated 3 February, 20 May and 6 July 2005 which establishes the basis on which the individual`s needs will be met by the home. Where necessary the homes statement of purpose shall reflect any changes in service provision required for this arrangement. This condition may not apply to anyone else other than the named individual who falls outside the registered category. Date of last inspection 3 February 2005 Brief Description of the Service: Stoneleigh is a registered care home with nursing providing personal care and accommodation for up to 36 older persons in the category (OP). It is owned by Southern Cross Care Homes Limited and operates within the private sector.The home is located in the centre of Annfield Plain, close to local amenities. The home is a purpose built; two-storey building situated in its own private well-maintained grounds. Personal accommodation and communal space is located on both floors. Service areas are located on the ground floor. This unannounced inspection was carried out on 9/8/05 over a period of 3.5 hours. During the inspection I spoke to service users, staff and the manager. I toured the building and a number of records where examined. Two of the three requirements made at the last inspecton where met. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 3 and 5 The process for admission to the home is clear, involving service users and their relatives in the process and providing them with the opportunity to make an informed decision as to whether they wish to move into the home. EVIDENCE: The manager said all prospective service users have a full assessment carried out by persons trained to do so prior to admission. Care plans examined contained a full pre-admission assessment . The manager said service users and their relatives were involved in this process. The manager said all prospective service users are invited with their family to visit and spend time in the home prior to admission. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7 and 9 The care planning process provides staff with clear information to meet the needs of the people who live in the home. Staff are provided with up to date information on medication to enable them to ensure the protection of service users. EVIDENCE: Since the last inspection care plans examined contained evidence to show that monthly evaluation of individual care plans had improved providing staff with clear information to enable them to meet the needs of the service users. The manager said a new care plan format is to be implemented which will cover in more detail the individual health needs of service users, improving the quality of information for staff, in meeting the needs of the service users’. Since the last inspection the home has purchased an up to date British National Formula, which provides staff with current information on medication administered to service users. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12 and 13 A flexible routine operates within the home and links with the community are good supporting and enriching service users social and leisure opportunities. EVIDENCE: The manager said the daily routine within the home is flexible and service users are given choice and their independence promoted by staff. On touring the home a group of service users where observed taking part in Picture Bingo. The activities co-ordinator explained how the game worked and service users who lacked capacity were seen to be supported by staff to take part. One service users spoken to said “ You have the choice to join in activities, no one forces you”. Another service user told me about a recent trip to Anwick Gardens and that due to good access for disabled service users she was able to go into the tree house. The home has a varied range of internal and external activities. A daily activity board is displayed in the foyer for service users information. The manager said service users can have visitors at any reasonable time, and their visitors can join them for a meal if they so wish. Evidence was available to show that the home is actively involved with the local community. Another service user said she was not religious but attended coffee mornings and other events at the local church. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19 The standard of décor and general enivironment within this home is good providing service users with an attractive and homely place to live. EVIDENCE: On touring the home it was observed to be clean, comfortable, accessible and fit for its stated purpose. Service users spoken to said they were satisfied with the standard of communal and personal accommodation. One service user spoken to said “ The company in the lounge is nice, but I also like the my privacy of my own room”. Since the last inspection the concrete floor in the rear lobby area between the laundry and staff room has been repaired to prevent staff from trips or falls. Bathing facilities are adequate , but the company should consider the proposal put forward by the manager for the provision of a shower, which will offer service users more choice when bathing. Service users needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27 and 28 Service users needs are currently being met, but staffing levels and skills must develop to meet the increasing dependency needs of the people who live in the home. EVIDENCE: At the last inspection due to the increasing dependency levels of service users the manager was recommended to review staffing levels to ensure that service users needs whre being met. The manager said that the company is currently considering this proposal. Since the last inspection a number of care staff have commenced NVQ2 in Care, but the level of staff having achieved this award remains low. The manager should ensure that 50 of care staff achieve this award. The manager said staff are positive towards increasing their skills and knowledge to ensure a good care service delivery to the people who live in the home. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 32, 34, 36 and 37 The manager has a clear sense of vision for the home which she effectively communicates to staff and the people who live and visit the home. EVIDENCE: The manager described her style of management as open door, treat people with repect and involving service users their relatives and staff in discussions about the home. The company needs to provide financial evidence to CSCI to show that this home is run in the best interests of the service users. (Outstanding since the last inspection). Since the last inspection a system of formal supervision has been introduced for all staff working in the home.
