Conditions of registration: Date of last inspection 18th November 2004 Brief Description of the Service: Summerfield is an adapted property with two extensions to the original building. The establishment provides accommodation for thirty- three service users whose special needs may include physical and/or dementia related illnesses in both single and twin bedrooms on two floors. There are two lounges a dining room and conservatory. The home is situated on a bus route on the outskirts of Silsden village. This is an overview of what the inspector found during the inspection. The inspection was undertaken in one day by one inspector, the registered manager was on duty throughout and feedback was given during the inspection. The inspection process included reading records talking to the manager, residents and staff. The home has information that is available to prospective residents and their families prior to admission. Pre admission assessment are undertaken and information is available to staff to commence the care plan on admission. The care planning system is being expanded to include the skills of other staff The storage and administration of medications has improved since the last inspection. Activities are offered but these are not extensive and there is an expectation that relatives continue to be involved in this with individuals. The environment continues to be good and improvements continue to benefit residents and staff. Training is on going for all staff and the ethos of the home benefits residents. A quality assurance system should be developed that reflects the views of the residents and their relatives. There were no health and safety issues at the time of the inspection and regular checks of the fire system and equipment are undertaken. What the service does well:
The registered person provides basic good quality care providing a good diet in a good environment where the residents appear to be happy and are kept safe by the policies and procedures in place and there are appropriately trained staff. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 1,3, and 5 The home ensures that the residents needs are appropriate to the home by providing accurate information to the individual and by gathering information on assessment prior to admission EVIDENCE: There have been no changes to the Service User Guide or the Statement of Function and Purpose a copy of the Statement of Purpose is available within the home. Many of the residents are referred to the home by the social services and have assessment undertaken by social workers and are available to staff before admission. The manager also assesses the individual prior to admission and both the prospective resident and their family undertake introductory visits where appropriate. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7, 8 and 9 Improvements in the care planning are on going with the involvement of other staff. Updating of information continues on a regular basis and residents are involved in developing their plan. The safe storage and administration of medication has been improved. EVIDENCE: All the residents have individual care plans that are updated regularly and reviewed annually or sooner if required. The manager has normally taken responsibility for updating these but has recently decided to involve other staff so that they have a more active involvement in record keeping. The storage of medication has recently improved and is now administered from a trolley rather than a cupboard this allows the medication to be administered directly to the residents speeding up the system and improving safety. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12, 13, and 15 The home does offer activities but there is an expectation for relatives to be involved where individuals are going out. The management does encourage relatives maintain contact. The food continues to be of a good standard with a consistent choice. EVIDENCE: Many of the residents say they enjoy the activities offered and the homely surroundings. There are no residents who have any specific cultural needs but the religious needs are addressed and religious leaders of differing denominations visit the home periodically. Relatives and visitors are welcome at any time and relatives of residents were around the home at the time of the inspection. The menu continues to offer choice at all meal times with a three-course meal at lunch time. The residents had no concerns with the food the cook has been there many years and residents said the standard was good. JUDGEMENT – we looked at outcomes for the following standard(s): 16 Complaints are seen as important and are acted upon. EVIDENCE: There is evidence that relatives and friends have had their complaints and concerns listened to, and appropriate action has been taken. Other agencies have been involved and there advice has been taken. There is a complaints procedure that is easy to understand and gives the complainant information on how and who to complain to including the name and address of the Commission. Service users have access to safe and comfortable indoor and outdoor communal facilities. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19, 20, 23, 25 and 26 The registered person has shown a commitment to improve the environment over a consistent period of time. The environment exceeds the standard required. EVIDENCE: The registered person continues to invest in the environment a second passenger lift has been installed recently en suits have been added to a number of bedrooms and one bedroom that was very small is to be enlarged. Carpets have been replaced and areas of the home were being redecorated at the time of the inspection. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27, 29 and 30 The home is protecting residents by ensuring that staff are trained to deliver the care and that appropriate staff are in place through a good recruitment procedure. EVIDENCE: The staffing team remain consistent with a very low turn over. The previous deputy did leave in August but an appointment was made and a replacement was in post by October. Training is on going and many staff have gained NVQ at level 2 and are undertaking level3. Distance learning courses in Moving and Handling has been undertaken, senior staff have undertaken training in medications. Other training includes Basic Food Hygiene and Health and Hygiene. Training for the future includes Dementia Awareness and Challenging Behaviour. On inspecting the records of the staff all the appropriate checks are undertaken including Criminal Records and references. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31, 32, 33, and 38 The quality of are at the home is influenced by the registered person and manager. The home provides a high quality of services in the best interests of most of the residents. EVIDENCE: The manager of the home is registered as a fit person with the Commission. She is appropriately qualified to required level and has worked the home for many years. Though residents views of the home are not collected formally there is evidence in other areas that the home is run in the best interests of the residents including the environment, the menus improvements in care issues. However, there should be a formal process of quality assurance based on the views of the residents. The fire systems and equipment are checked on a regular basis there was no evidence of risk to residents, staff or visitors at the time of the inspection.
