Date of last inspection Brief description of the care home Summerville Nursing Home has recently been taken over by BUPA. It is situated on the border between the Stockton Heath and Grappenhall areas of south Warrington. There is ample care parking space and a good public transport network around the home. It is within easy reach of Warrington town centre, the M6 and M56 motorways.
Care Homes for Older People Page 2 of 8 Brief description of the care home The home is a large period house set in its own grounds by the Bridgewater canal. The original building has six large bedrooms, a bathroom, lounge, dining room, kitchen, laundry, staff facilities and office accommodation. A large purpose built extension houses thirty-seven bedrooms, two smaller lounges and bathrooms. All residents have access to the nurse call system. The charges range from #480 to per #620 week. Two inspectors visited the home on 12th November 2009 in response to three complaints made to us. We looked at care plans for three people who live at the home who had complex needs. these were written in a person centered way, were detailed and contained information to guide staff as to how to meet the needs of the people. Fluid balance charts were looked at in the rooms of four people and these had been completed correctly on the day we were visiting. We spoke with a number of visitors and the staff on duty on the day of the visit. All were of the opinion that the numbers of staff on duty was insufficient to meet the needs of the people living at the home. Two people said that they felt that their complaints had not been resolved. A meeting had been held with relatives and issues they discussed were , staff shortages, people not getting drinks, fluid balance charts not being completed and call bells not being answered. Some relatives said that some of these issues had improved since the meeting. There were two nurses and seven care staff on duty on the day of the visit until PM. All staff appeared busy , however call bells were not being answered promptly. We observed a call bell ringing when we arrived at the home for an excess of fifteen minutes. The administrator asked staff to answer the call bell. During the feedback meeting with the manager and deputy manager a call bell was ringing for an excess of twelve minutes. Relatives spoken with said that call bells were not answered quickly. Staff spoken with said they could not respond quickly as they were too busy. One said we are trying to provide good care but it is difficult as people have high dependency another said we are always struggling,, the home is so busy Most of the people living at the home were highly dependent and two members of staff were needed to attend to the needs of the people. On speaking with the manager she said that eleven people at the home are nursed in bed at all times. Lunch was due to be served at 12 :30 , however, it was not served until 1:35 to people in the dining room as staff were moving people from the lounge to the dining tables using one hoist which took time. The home does have other hoists that can be used. Staff spoken with said that there is only room to use one. The people who were staying in their bedrooms for lunch were served later. Family members have been asked not to visit during mealtimes and some relatives spoken with said they were unhappy with this arrangement. Some relatives were in the home assisting people with their meal in their bedrooms. The staff appeared to be very busy but the home appeared disorganised. Although relatives felt that the home was short staffed they praisd the people working at the home and said the staff are very good but rushed off their feet the staff are lovely but there doesnt seem to enough of them the staff are fine but they are seriously overworked On the evening shift there were two nurses and five to six care staff on duty. At night from PM to AM there is one nurse and four care staff on duty which is not sufficient to meet the needs of the people living at the home. A full assessment of the number and dependency levels of the people living at the home should be made to enable adequate skill mix and numbers of staff to be on duty at all times to meet the needs of the people living there. The manager agreed to increase the staff at night as a matter of urgency. The home was clean and free from odours at the time of the visit. The complaints log was looked at and some complaints had been fully recorded with all appropriate letters in place so that an audit trail could be followed. However, the
Care Homes for Older People Page 4 of 8 complaints folder was confusing as not all complaints listed had full information present and it was unclear as to which complaints had been finalised and which were still being investigated. There have been twelve complaints made to the home since July 2009. The management from BUPA are addressing some of these issues. An open meeting is due to be held with all relatives by the Deputy Regional Director on the 17th November. A quality assurance audit is to be carried out at the home on 27th November. The managers office is at the back of the home and this may make it difficult for people to have access to see her. Night time must be assessed as a priority. So that there is adequate staff on duty to fully meet the needs of the people living there at all times.
