The inspector found there to be outstanding requirements from the previous inspection report but made no statutory requirements on the home.

What the care home does well

Activities at the home are well planned and carried out on a regular basis. Residents are delighted with the weekly trips out on the minibus and enjoy visiting the local community in this way. Residents are provided with a warm, safe and comfortable environment that welcomes visitors and makes them feel at home. The home is clean and staff work hard to make sure the building is odour free. Residents and visitors were complimentary about the manager and the staff. One individual said `the staff are friendly and always willing to stop and have a chat with you`. Two other people living at the home said ` the general manager is caring, she visits us every day and listens to any problems we may have and takes action if needed`.

What has improved since the last inspection?

The home continues to meet the standards set out in Care Homes for Older People and exceeds the standard in a number of care and practice areas (see the Inspection report for 11th May 2005 as well as this report).

What the care home could do better:

Staff must give out medication to the residents at the correct time and not sign for any medication until it has been administered, to make sure that the residents are protected from risk and harm.

To admit another service user under the `younger disabled` category, therefore, increasing the number in this category to three (3). 11th May 2005 Date of last inspection Brief Description of the Service: Sunningdale Court is a care home with nursing, which provides accommodation for 49 residents within the categories of Dementia, Old age, Physical Disabilities and Terminal Illness. The home is purpose built for its current use and accommodation is located on two levels, with passenger lift access to the upper floor. The home is owned by Ashborne Homes, a national organisation, which provides support to the homes management team from a regional office. The home is situated to the east of the City of Hull, easily accessible by the citys link road and close to public transport services. Local amenities are within a short walk of the home. This unannounced inspection was carried out with the General manager, staff and residents of Saltshouse Haven. The inspection took 3.5 hours and included a tour of the premises, examination of staff and resident files and records relating to the service. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 3. All residents undergo a full needs assessment and are given sufficient information about the home and its facilities prior to admission, to enable them to be confident that their needs can be met by the service. EVIDENCE: Standard three continues to be met by the home. Three personal files for new residents to the home were looked at and the information within them included an assessment of need from the funding authorities and a pre-admission assessment completed by the home. Those residents at the home who receive nursing care have undergone an assessment by a NHS registered nurse from the Health Authority, to determine the level of nursing input required by each individual. Each resident has an up to date care plan in place, which has been developed from the information within the assessments of need. Discussion with the three residents found that they have a good understanding of their care needs and their comments indicated that staff have helped them to settle into the home and they are satisfied with the service being offered to them. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 9. The procedure for the administration of medication has deteriorated since the last inspection, placing residents at risk and harm. EVIDENCE: All the residents spoken to prefer to have the staff administer their medication and were happy with the way it is given. Checks of the medication system showed that the home has taken on board the recent changes to Legislation, around the way that Nursing Homes must dispose of unwanted medication. They have made arrangements to safely dispose of medication through a licensed waste disposal company and have received the necessary paperwork and equipment from this company. A number of errors on the medication charts were seen during the checks of the system. The nurse on duty was spoken to and it emerged that she was having some difficulties in time management around the administration times of the medication. Errors noted included medication being signed for as given at 08.00 when in fact some was observed by the Inspector to be given as late as 11.00am. The midday medication had been signed for as given even though it was still in the medication trolley and had yet to go out to the Sunningdale Court Nursing And Residential Home Hull DS0000000956.V263839.R01.S.doc Version 5.0 Page 10 residents. These are not acceptable practices and must stop immediately as they pose a serious risk to the health and safety of the residents. Discussion with the General manager indicated that the matter would be dealt with immediately. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12. Residents are provided with a wide range of social activities and entertainment that enriches their daily lives and meets their social needs and expectations. EVIDENCE: Verbal feedback from four residents was extremely positive about the home and the social activities that are provided for the resident’s entertainment. One individual said that they had enjoyed the concert given by local school children on the previous day to the inspection. Everyone was looking forward to the Christmas party scheduled to take place on the 20th December and some residents had started the season’s partying early by going to the Dorchester Hotel for Christmas Dinner on 14th December. Residents said they were going to a Pantomime in the local community and a number of relatives were booked in for Christmas Dinner at the home, so they could spend the day with their loved ones. There are two activity co-ordinators working at the home and residents commented that these staff members did a marvellous job in keeping them busy and entertained. On the day of this inspection a karaoke session was in full swing, with a large number of residents and relatives joining in with the singing. Discussion with the General manager indicated that the activity co-ordinators are now working one weekend a month to give a more even spread to the entertainment on offer to the residents. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 16. The home has a satisfactory complaints system with evidence that residents’ views are listened to and acted upon. Staff and residents are confident about reporting any concerns and the general manager acts quickly on any issues raised. EVIDENCE: The home continues to meet standard 16 and residents spoken to were confident about using the complaints system if needed or necessary. The complaints records show that there have been no complaints made since the last inspection and residents spoken to said ‘Ann (the general manager) comes round to see us every day and talks to us about any niggles we may have. She tries to solve them immediately and will get back to us if she needs to take time to resolve them’. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19 and 24. The standard of the environment within this home is good, providing residents with an attractive and homely place to live. EVIDENCE: There is an ongoing programme of refurbishment and renewal within the home. The General manager said that two more profiling beds have been purchased since the last inspection and eight new divan beds. The upstairs corridor carpets have been replaced and four bedrooms have been decorated and had new carpets fitted. All areas seen by the inspector were clean, well decorated and welcoming. Three new residents were spoken to and they commented that ‘staff have helped us settle into the home and are extremely kind and supportive’. The residents have brought personal possessions with them into the home and their bedrooms are individually decorated, looking welcoming and homely. All bedrooms are supplied with door locks and lockable storage space to ensure resident’s valuables are kept safe. Staff have a master key, which can be used to gain access in an emergency. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27. Staffing levels within the home are sufficient to meet the needs of the residents and offer them support and assistance when required. EVIDENCE: There have been no changes to standard 27 since the last inspection and the home continues to offer sufficient staffing to meet the needs of the residents. Information in the staffing rotas shows that there is two nurses and seven care staff on duty during the day shift and one nurse and four care staff at night. Extra staff are called in to cover for taking residents to appointments and additional ancillary staff are on duty throughout the day. Residents spoken to are very happy with the amount of staff on duty and said ‘there is always someone around to help you if you need assistance’ Sunningdale Court Nursing And Residential Home Hull DS0000000956.V263839.R01.S.doc Version 5.0 Page 15 Management and Administration The intended outcomes for Standards 31 – 38 are: 31. 32. 33. 34. 35. 36. 37. 38. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 35 and 38. Robust accounting and financial systems are in place to protect and safeguard the interests of the residents. EVIDENCE: Checks of the finance systems within the home found that written records are kept and up dated by the administrator on a daily basis. Ten residents have arranged for their pensions to be paid into the home’s account where their fees are deducted and their personal allowance transferred to their own separate Personal Allowance account. Other residents have their families looking after their financial affairs and their relatives top up the resident’s individual personal allowance account on a regular basis. Resident’s who have asked the home to look after their personal allowances are able to access their money on request. All monies are kept safe and secure within the home and only the administrator or General manager have access to the funds. The homes accounts are audited twice a year by external senior management staff employed by the company. Accident books are filled in appropriately and regulation 37 reports completed and sent on to the Commission where appropriate. Staff have received training in safe working practices and the General manager has completed generic risk assessments for a safe environment within the home. Risk assessments were seen regarding fire, moving and handling, cot sides and daily activities of living.
