It is close to local shops and bus routes to towns on the Wirral and Liverpool. The home offers care and support to 22 residents who are over the age of 65 years. This registration includes providing a service to 6 people who have dementia. Accommodation is provided on two floors with passenger lift and stair access. The

On the ground floor there are two sitting areas and a separate dining room, some bedrooms, an adapted bathroom and toilets. Outside the home there are garden areas and some parking spaces. Basement areas in the home are used as an office, a staff room, a laundry and for storage. The weekly fees for the service range from GBP366.17 to GBP395.22. Additional charges are made for hairdressing and chiropody. A service user guide and a statement of purpose, which describe the services, offered is made available to people who are interested in using the service, their relatives and professionals before a person comes to live at Sylvan House. A selection of care plans were examined. While additional risk assessments had been drawn up, they held only basic information and were not always up to date. Care plans had not been formulated from this information. Service users medical and social care needs had been identified, but again supporting care plans had not been drawn up. The only care plans in place were day/night care plans which held basic information in relation service users individual routines. Information was not always signed or dated so it was not possible to establish how current it was. Staff have not been provided with formal care planning training, although they have been given information to read and the manager has held informal discussions. The provider intends to provide staff with new literature on care planning in April 2010. No changes have been made to the way the home is managed and an up to date development plan has not been drawn up, the provider has agreed to submit this document. Service users medicines are audited by the supplying pharmacist and a visual check of medicines is carried out each day. A record of this check is not kept. The recommendations from the Environmental Health Department have not been implemented, although the kitchen was clean and tidy. All staff have a Criminal Record Bureau check, although this has not been renewed in line with good practice which is every 3 years. A policy which asks staff to declare any offences between these checks has not been introduced. The night staffing levels have been reduced on some nights due to a staff shortage. To ensure the service users care needs are continually met, appropriate staffing levels must always be provided. What the care home does well: What they could do better:
More detailed information need to be collated during the assessment process so that service users know they will receive the care and support they need. Improvements need to be made to the care plans and risk assessments so that staff have all the information they need on how to provide service users with the care and support they need. Staff should be provided with more training in this area to support them in
Care Homes for Older People Page 4 of 9 their role. Medicines should be audited regularly in the home so that staff can monitor how they are used. Management systems need to be improved to ensure the efficient and effective running of the business. Criminal Record Bureau checks should be updated every 3 years. A policy should be developed which asks staff to declare any criminal offences between these checks. This will ensure staff are suitable to work with older people. Appropriate night staffing levels must be provided to ensure service users care needs are continually met.
