This pharmacist inspection lasted five hours and was carried out to follow up medication concerns after a number of notifications. During this visit we examined medication policy and proceedure, the current medicine administration records (MAR) and the medication ordering, storage and handling arrangements. There are few administration gaps on the MAR charts meaning that people living in the home can expect to receive their prescribed medicines correctly. However some medicines had pharmacy labels where the name and dosage had been obscured by felt tip pen and the holes holding the MAR charts in the folder had become torn making it difficult to read the directions. The quantities of medicines received into the home and brought forward are clearly entered. A number of medicines were listed on the MAR charts with a dose as directed or when required. The prescriber or pharmacist should be asked to provide information on the correct dosage to make sure that medicine is given correctly. Medicines are stored in one trolley and in a locked cupboard. There is insufficient space on the trolley to store all the medicines for each person and therefore medicines are administered from boxes which were not dispensed by the pharmacy for the person. The temperature of the medicine storage room are regularly monitored but the records show that the temperature was regularly recorded as above twenty five degrees centigrade. The records kept of fridge temperature were poor with only four records kept between 30/7/09 and 10/9/09. The date of opening was not added to all liquids, eye preparations and medicines with a limited use once open. Storing medicines at temperatures and conditions recommended by the manufacturer means that staff will know medicines are safe to use when needed.
