What the care home does well

The home provides a welcoming atmosphere and prospective residents are encouraged to visit the home, if at all possible, before moving in. The staff are smart and professional in their approach. Visitors are made welcome and are offered refreshments. The gardens are well maintained and are well used by the residents with seating areas provided. The home offers an exceptional activities programme for the residents. The interior of the home is exceptionally clean. Care planning is of a very good standard.

What has improved since the last inspection?

It is a teaching nursing home. Students now attend the home for longer placements of ten weeks towards the end of their training, instead of a three weeks taster programme. Carers are now more involved in the care planning process. The manager has put together a package, and mock care plans around the activities of daily living in order to educate the carers interested in planning the care for the residents receiving personal care. The one single room has been converted to a large single room.

The place for MD(E) is specifically for the service user identified in the application dated 26.8.4 16th September 2005 Date of last inspection Brief Description of the Service: The Beeches Care Home is a purpose built facility situated in the Wibsey area of Bradford It provides care for 64 Elderly people with Physical Disabilities over the age of 65 yrs. It is serviced by a regular bus route, and there are ample parking places available. There are pubs, shops and churches nearby. Care is provided on two floors, in 63 single rooms. There is also a spacious dining room on the ground floor of the home with panoramic views over Bradford where residents can sit and dine in congenial settings. There are three ample sized lounges where residents may choose to spend their day. The Commission for Social Care Inspection has to carry out at least two inspections of care homes every year. The inspection year runs from April to March and this was the second inspection visit for 2005/2006. Copies of previous inspection reports are available at the home or on the Internet at www.csci.org.uk. The last inspection of the home was on 16 September 2005. This was an Announced inspection carried out by inspector. The main purpose of this inspection was to make sure that the home provides a good standard of care for the service users and to assess progress on meeting any requirements or recommendations made at the last visit. The methods used at this inspection included looking at care records; observing working practices and talking to staff, service users, relatives, and to the manager. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 1 and 3 EVIDENCE: Very good information is provided to prospective residents showing how the home is run and how the staff are trained. The manager takes responsibility for checking the prospective residents needs can be met before admission is agreed. A trial visit is then offered so that the prospective resident can check if the home is suitable for them. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7, 10 and 11. The health care needs of residents are met and care plans provide clear and detailed instructions for staff to follow. Residents are treated with respect and their privacy is upheld. Staff are aware of the residents’ needs and there is good communication amongst the staff group. Residents are treated with dignity and their privacy maintained at all times. Staff treat residents with dignity, maintain their privacy at all times and show an awareness of the residents’ needs. EVIDENCE: Care plans chosen for review and case tracking identified needs were identified, and adequate care plans written following a pre-admission assessment. West Yorkshire Police did a drugs check at the home in 2005 Relatives are asked about the residents preferences of end of life situations. GP’s are involved in any multidisciplinary decisions regarding this. The relatives of deceased residents are contacted by the Coroners Officer if a death is unexpected. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12 and 15 Residents’ choices are respected and contact with family and friends is encouraged. Social activities are well planned. A good, varied and nutritious diet taking into account individual choices is provided at the home. EVIDENCE: The home employs two activity organisers who consult with residents on a regular basis about the activities they like. Recently trips have been made to Bridlington, the White Rose Centre and Meadowhall in Sheffield. I was told that no resident had expressed a wish to go way on holiday, although many go out with relatives for pub lunches, to relatives homes for Sunday lunch and to Church. One lady resident I spoke to said although she did not take part in arranged activities, she was always kept busy, knitting for staff who were expecting babies. She was currently working on making a rucksack for a teddy bear. Activities were taking place in the large lounge during the inspection, and one group of gentlemen said how much they enjoyed playing dominoes. The Beeches Nursing Home DS0000029133.V272960.R01.S.doc Version 5.0 Page 12 Another lady resident said she has plenty of visitors, and went out shopping with one of the activity organisers the day before the inspection, which made her feel special. The same lady said that she didn’t enjoy the food very much, that it wasn’t to her liking. I spoke to the chef and suggested he spoke with her to get some idea of the foods she liked. He told me he had already done that the day before my visit, and had cooked something individually for her, but that she hadn’t liked what he prepared The Beeches Nursing Home DS0000029133.V272960.R01.S.doc Version 5.0 Page 13 Complaints and Protection The intended outcomes for Standards 16 - 18 are: 16. 17. 18. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): All standards were assessed. Complaints are dealt with appropriately. The home has a detailed complaints and adult protection procedure which is robust and protects service users. The level of staff understanding gives assurance that complaints will be taken seriously and service users will be protected from abuse. EVIDENCE: There has been one in house complaint which was dealt with, appropriately handled responded to in writing and resolved to the satisfaction of the complainant. Documentation pertaining to the complaint was reviewed. Residents have postal votes or are taken to the local polling station. Any unopened polling documentation is returned to the returning officer. Resident’s are enabled to se solicitors in the privacy of their rooms, and advocacy is sought for those residents requiring it. NVQ training contains a section on abuse awareness, however to supplement this the manager has bought videos on the subject, and care staff are provided with workbooks. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): All standards were assessed. The home offers a safe, well-maintained environment for the residents and provides appropriate bathing and toilet facilities. The system of laundering soiled linen negates the possibility of cross contamination occurring. The standards of cleaning are consistently high throughout the home. EVIDENCE: The owner keeps a high profile in the home to ensure that health and safety standards are implemented. The manager being the Health and Safety representative for the home has completed training in this. Communal areas are kept to a high standard and are spacious and comfortable. Bedrooms are redecorated and refurbished as they become vacant. Since the last inspection double rooms have been converted into single rooms. All rooms with the exception of 2 have en-suite facilities. Each bedroom contained personal belongings and was arranged according to the preferences of the occupant. Some residents have a private telephone line. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27 and 30 Staffing numbers and skills ensure that residents’ needs can be met. Staff are trained and competent to do their jobs. EVIDENCE: Staff were present on duty on the day of the visit in sufficient numbers and of appropriate skill mix to meet the needs of the residents. Training takes a very high priority in the home. The home has enlisted an external trainer to provide Palliative Care training for all staff; this takes place either at one of the other homes in the group, or at the Marie Curie centre in Bradford. I was told that the training is provided in conjunction with Bradford University, and that there is a package put together by the Macmillan Nursing Services with workbooks, which is supported by the University and tailored specifically for care homes. NVQ training is on going with 60 of the care staff in the home having either achieved level 2 or level 3. Other training taking place in addition to mandatory training is management training for the qualified staff. Training in challenging behaviour, Falls risk management, administration of medication of medication, Infection control, First Aid, Health and Safety training, continence care and food hygiene. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31, 33 and 34 The home is very well managed and the manager is well able to discharge her responsibilities. She offers good leadership to the staff and ensures the residents are protected and cared for in a correct manner. Staff are well motivated, residents are consulted and their interests are safeguarded at all times, the interests of the residents are seen as very important to the manager EVIDENCE: There is soon to be a change of manager. The company have identified a prospective manager to take over the role, and an application form has been forwarded in respect of this. This should be returned with the appropriate fee, which at this time is £518. These, when returned anonymous or not are audited and kept for reference, this enables any changes in the service to be recognised and acted upon.
