Conditions of registration: 1. The service may admit four persons 60 years of age and over. Date of last inspection 14 December 2004 Brief Description of the Service: The Beeches is a purpose built registered care home providing personal care for up 44 older people whom have dementia. The home is situated in the Wath Upon Dearne area of Rotherham and is owned by Winnie Care (Highgrove Ltd). They also own other homes in the surrounding area. The home is organised into two units of twenty-two single rooms, one on the lower and one on the upper floor. There is a lift to gain access to the upper floor for service users unable to use the stairs. All rooms have en-suite facilities. There are two sets of single bedrooms that have adjoining doors in order to provide two double rooms if they are requested. There is a small, enclosed garden with a patio area for service users to use and car parking spaces are provided at the side of the building. This is an overview of what the inspector found during the inspection. A statutory unannounced Inspection was completed on the 13th September 2005,starting at 13.20 and finished at 18.00 hours. During the Inspection we looked at a chosen selection of the National Minimum Standards, sampling of records, document reading, partial tour of the two units and direct and indirect observation of staff and residents. Discussion with management, interviews with staff members and discussion with residents and visitors in the home at the time of the Inspection. A number of comment cards were left at the home for relatives and visitors to complete, some of which were received back and their comments included in the report. What the service does well: What has improved since the last inspection?
The home has a variation to enable them to admit residents 60 years old, the manager has produced an action plan that outlines key areas to consider when admitting these residents, and those in the home at the time of Inspection appear to have settled and their needs met. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 10 Residents are treated with respect and privacy maintained. EVIDENCE: During the Inspection staff were observed conversing with residents and promoting privacy and dignity when required. Staff that was interviewed demonstrated good knowledge of residents care needs and aware of residents capabilities and stated that if residents are able to make choices, they are consulted and their wishes carried out. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12 13 14 Social activities are arranged by the home and residents are able to participate if they wish, and staff promote choice and independence to all residents. EVIDENCE: The home has a new activities person in post; she was interviewed and asked about her role within the home. She tries to ensures that residents and likes, dislikes and hobbies are recorded, and evidence was available that any social activities that residents take part in are documented. Links with local community are encouraged and visits to local shops and pubs for lunches take place. She his very enthusiastic about her role and the company should support her training needs to ensure she can develop her role. Visitors seen on the day confirmed they can visit at any reasonable time, and can see their relatives in private if necessary, both floors do have quiet rooms for visitors. Fulfilment can be difficult to provide into the lives of residents who are vulnerable and require ongoing supervision, but it was evident when interviewing staff that they are committed to maximise choice and independence to all residents.

The intended outcomes for Standards 16 - 18 are: 16. 17. 18. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16 18 Residents and relatives are provided with information to enable them to raise concerns or complaints about the home and their care. Adult protection policies, procedures and training for staff are in place; staff had knowledge and understanding, which promotes protection of service users from abuse. EVIDENCE: The home has an appropriate complaints procedure with all the relevant details. Records show that any complaint was dealt with quickly and effectively with a recorded outcome. The home has adult protection policy and procedures in place and staff confirm they are aware of these procedures and that they have recently received updated training in abuse and this was also discussed in supervision sessions. Recent incident at the home resulted in a referral to Adult Protection; this was fully investigated by the adult protection team and was resolved, but waiting for the report. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19 26 The home is suitable for its stated purpose, and residents live in a safe and well-maintained environment, which meets individual needs in a comfortable and homely way. EVIDENCE: The home was purpose built and meets all the environmental standards, tour of the home found it to be clean and tidy and well maintained. A notice board that displays planned activities and other items of interest for relatives and visitors is situated in the entrance lobby of the home. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27 28 29 Recruitment policies are followed ensuring the safety and protection of residents, and staff has the skills and knowledge to fulfil their roles within the home. EVIDENCE: Staffing was discussed with the manager; the home is fully staffed and has no vacant posts. Duty rota examined showed that the appropriate staff were on duty for the assessed needs of residents, although one visitor did comment that they feel the home is short staffed at times, but no evidence of this was found. The activities person is extra to the care numbers and the manager is supernummary and the deputy manager as some management hours each week, either the manager or her deputy is on call at all times. The home are committed to meet the 2005 target and hope to have 50 of staff with NVQ qualification, at the present eight staff has already completed NVQ training and other staff are on courses. Recruitment files of new staff were checked and found that the home operates a thorough procedure based on equal opportunities, references obtained and ensuring satisfactory police check for the protection of residents. All staff receive contract of terms and conditions and given copies of the code of conduct. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 33 35 36 38 Residents live in a home that is run and managed by a manager that is skilled and experienced to run the home and ensures so far as is reasonably practicable the health, safety and welfare of residents and staff are protected. EVIDENCE: Records show that the home has various monitoring systems in place and these audits were available to examine. Although most of the residents are not capable of giving feedback on the service and care provided, the home send out questionnaires to relatives and visitors and offer support meeting at the home, these are minutes taken and these were available to examine. Procedures are in place to ensure the financial interests of residents are safeguarded, and a number of records were checked and found satisfactory. Staff receives formal supervision required to ensure they have the opportunities to develop new skills and knowledge.
THE BEECHES RESIDENTIAL CARE HOME 20050831 Beeches(Wath) X00015 UI Stage 4 S55216 V186997.doc Version 1.40 Page 16 Accident reports are analysed by the manager to ensure risk assessments are developed where required, but when records were checked it was noted that the home did not send Regulation 37 notifications to the Commission for Social Care Inspection to inform the Inspector that residents had attended Accident and Emergency. Maintenance and service records examined were up to date and current to the services provided. The manager has the required Health and Safety policies and procedures and displays the relevant notices. Fire safety procedures are in place and service records were examined and were current, ensuring the safety of service users.
