This inspection was carried out on 4th September 2009.

It is an annual review prepared by CQC after examining previous reports and information from the provider. At the time of this report, CQC judged the service to be Excellent.

The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

If yes, what have they been: Date of last key inspection: Date of last annual service review (if applicable): Brief description of the service The Brook is located on the outskirts of Leyland town centre, and provides 24-hour support for up to 6 people with learning disabilities. The Brook offers accommodation on two floors, and all rooms are single. One room has a toilet en-suite facility, and a second has a shower en-suite facility. The communal lounge, dining area and kitchen are on the ground floor. The home has a private patio area to the rear. The fees depend on the assessed support needs of the individual service users and can be discussed with the home manager. Service users pay for their own leisure activities, chiropody, hairdressing and personal items, including clothing and toiletries. Annual Service Review Page 2 of 6 Annual Service Review Page 3 of 6 Service update since the last key inspection or annual service review: What did we do for this annual service review? The annual service review for The Brooke included the following: An Annual Quality Assurance Assessment (AQAA) was sent to the home by The Care Quality Commission (CQC) and completed by the manager and returned within the timescale. The AQAA is a dataset that must be completed annually by all registered providers. It is one of the main ways we gather information about the provision of care and how providers believe they are meeting the assessed needs of people using their services. The manager gave details of what the home has achieved during the last 12 months and what she hopes will be achieved during the next 12 months. We sent out survey questionnaires to residents, their families and members of the staff team. We looked at information about how the manager has dealt with any issues or complaints. We checked previous key inspection reports and the annual service review that was completed last year. We looked at any notifications about the home sent to CQC since the last annual service review. We also checked if there had been any concerns raised by other external agencies. What has this told us about the service? From the details outlined on the AQAA and information received from other sources we judged that this home continues to exceed the National Minimum Standards (NMS) of the Care Standards Act. We received returned survey forms from people living in The Brook, all of which evidenced that they received very good care and support from the manager and staff. One resident said, I like living in The Brook. I am happy here. Comments from members of the care staff team included, We enable all the service users to live as independently as possible, Information about clients needs is always made available and We have monthly reviews and the manager is always happy to give constructive information. The information in the AQAA evidenced how the manager listened to the views of residents and staff through meetings and one to one chats. Over the past 12 months information about the transition period before a resident moves in to the home has improved and is now kept on file. Person centred care plans and health action plans have been implemented and the use of as and when medication has decreased. Residents are already involved in the running of the home and this will continue. Staffing hours are being managed more efficiently to allow staff to implement activities specifically requested by the residents. Daily routines have been changed to give more flexibility of choice to the times residents get up and go to bed. This is of course in accordance with the information recorded in the care plan. Staff handover periods now include medication and financial checks to ensure all are up to date and correct. Recycling has improved and one of the residents has planted out part of the garden border and provided hanging baskets during the Spring. Staff are now working closely with external healthcare professionals to ensure all the assessed needs and expectations are met. This will also ensure the quality of life and Annual Service Review Page 4 of 6 level of independence remain high. Over the next 12 months the manager would like to improve staff training with regards to the assessment of daily needs and life skills. She also hopes to give more time to the preparation and development of the residents yearly plans to ensure each one is completely unique to the individual. The home will look at individual training needs and will include training in Deprivation of Liberty. Staff involvement in the risk assessing of health, safety and security matters will continue and interpersonal relationships with external agencies will further improve the support already provided to those living in The Brook. What are we going to do as a result of this annual service review? We are not going to change our inspection plan and will complete a key inspection by the 4th April 2010.
