What the care home does well

Residents and their relatives spoke very positively in respect of the care and accommodation provided for them at The Bungalow residential home. The home and gardens are maintained to a good standard and residents are able to access all areas of the home that are designated for their use. Comments made by residents to the inspector included `the staff are very caring and treat my mum with respect`, `the staff help me do things I can`t do for myself any more`,` I am free to chose what I do with my time and the staff are kind and pleasant`.

What has improved since the last inspection?

The requirements made in the last report have been addressed and complied with. Residents and their relatives who were spoken to said that the quality of the activities, outings and events has improved over recent months

What the care home could do better:

Some environmental issues were identified in the course of the inspection and these are detailed in the environmental swection of this report. Also ammendements are required to the hom`e application form used for potential staff to increase the existing strategies to protect service users from potential abuse.

Date of last inspection Brief Description of the Service: The Bungalow is a privately owned care home providing personal care and accommodation for up to 18 older people. The home is situated in the Bolton area of Farnworth and is within relatively easy reach of shops, pubs, bus routes and other amenities. A park is situated across the road from the home. The home is a single storey building, surrounded by attractive gardens. All bedrooms are single. This unannounced inspection took place on the 23rd of August 2006. The inspection took place over six hours. The inspection included discussion with residents, their relatives, a tour of the premises, inspection of care and other records maintained at the home and discussion with management and staff. The home was being appropriately managed and residents were content in their environment and appeared to be well cared for. Residents said they were supported and cared for appropriately and encouraged to make personal choices and retain as much personal independence as possible. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 1,3,4,5. Standard 6 does not apply to this service. Quality in this outcome area is adequate. This judgement has been made using available evidence including a visit to the service. No prospective residents are admitted to the home without their care and support needs being assessed appropriately. EVIDENCE: Prior to residents being admitted to the home the manager carries out an assessment of the prospective resident’s needs in consultation with the resident, their relatives and relevant health (for example doctors) and social care professionals (for example social workers). The purpose of such an assessment is to assist the prospective resident (and their relatives) in their considerations of how appropriate a placement at the home would be and enable the managere conducting the assessment if the home will be able to meet the prospective resident’s needs appropriately. A statement of purpose and service users guide is provided and contains information that assists prospective residents and their supporters to make informed choices about the suitability of the home. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7,8,9,10. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to the service. The health and personal care residents at the home receive appears to be suitable and appropriate and is meeting the expectations of residents and their relatives. EVIDENCE: The care records of three residents were inspected. These contained care plans that were initially based on the pre-admission assessment that is referred to earlier in this report. Care plans addressed the health, personal and social care needs of residents appropriately and were evaluated at least monthly. Risk assessments, that seek to protect resident’s health and safety were also in the main recorded in respect of residents skin integrity (assessing the risk of pressure sores), mobility (including the risk of potential falls and fractures, and nutrition (including weight monitoring) and other relevant areas and these were evaluated periodically. It is however strongly reccommended that these should be formally evaluated at least monthly. All residents are registered with a local GP and it was evident that all were enabled to access opticians, chiropodists, district nurses and other specialist services that individual resident’s require. Residents and relatives spoken to The Bungalow Residential Home DS0000062979.V301753.R01.S.doc Version 5.2 Page 11 (and in pre inspection questionnaire responses) were of the view that they were kept informed of all significant changes in their relation’s health. The procedures for the receipt, recording, storage, handling, administration and disposal of resident’s medicines were appropriate and safe. Staff responsible for the management of residents medicines have all received appropriate training. Medicine records had been completed properly. Discussion with residents, relatives and staff revealed that residents were treated with respect and that their right to privacy was upheld. Comments made included; ‘the staff are very nice and treat me well’, ‘I am able to choose when to get up and when to go to bed’, ‘the staff make sure you get the food you wan’t to eat’, ‘the doctors and district nurses examine all residents in their own room when they come to see our residents’. During the inspection staff were observed to treat residents with respect, protected their dignity and assisted them properly. Residents felt satisfied staff support them appropriately in trying to maximise their health and well being, and treat them decently and with respect. These are all very important areas in resident’s lives and are important measures of their satisfaction (or otherwise) of how effectively a care home is looking after them. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12,13,14,15. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to the service. The provision of leisure and social activities is of a very high standard and residents spoke very positively in respect of food provision at the home. Both are very important to the quality of life experienced by residents. EVIDENCE: Discussion with resident’s revealed they were enabled to make choices in their every day life. In particular they stated that they were able to chhose what time they got up and went to bed, whether they wish to participate in activities, to go to their rooms when they choose, where to take their meals and were aslo able to chose what they ate and drank, and are able to personalise their rooms. Many are able to go out with realtives/friends and staff and do so regularly. Residents felt that staff encouraged them to retain their independence and individuality. They were also very satisfied with the social and leisure activities provided stating that the range of activities and frequent outings available are varied, appropriate and that they are regularly The Bungalow Residential Home DS0000062979.V301753.R01.S.doc Version 5.2 Page 13 consulted at residents meetings about such activities and that they efectively decide what is good and what needs to be changed. The home maintains a record of all activities and outings provided. Residents spoke very positively in respect of the food provided at the home, flexibility of meal times, choice of meals and dining areas provided – in which new dining furniture has been provided since the last inspection. Comments about the food provided included ‘ the food is good here and we get a cooked breakfast every day’, ‘I can always request something different if what is on the menu is not suitable’. 4 weekly menus are provided and are varied, balanced and provide choice. Staff served and assisted residents appropriately and sensitively with their lunch on the day of inspection. This was a hot, substantial and well presented meal. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 16,18. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to the service. Residents (who were able to) and relatives spoken to said they felt comfortable enough to make a complaint if they felt the need and knew how to do so. Policies and procedures of the home seek to protect residents from potential abuse. EVIDENCE: The complaints procedure was prominently displayed in the entrance to the home and is contained in the service users guide issued by the home. Residents and relatives spoken to said that any concerns or worries brought to the manager’s attention are responded to quickly and rarely become formal complaints. A complaints log/incident log is maintained. Issues appear to be addressed appropriately and swiftly with an outcome recorded. The Bungalow care home operate ‘abuse and whistle blowing’ policies that seek to protect elderly persons. In addition, a copy of Bolton’s Inter agency protection procedure is held on site. Staff spoken to confirmed that they had received training and were aware of the whistle-blowing policy. Staff training records indicated that all staff receive regular training in this important area of protection of vulnerable adults. Service users live in a safe, well-maintained environment. Service users have access to safe and comfortable indoor and outdoor communal facilities. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19,20,21,22,24,25,26. Quality in this outcome area is adequate. This judgement has been made using available evidence including a visit to the service. There have been improvements to the environment of the home. EVIDENCE: The Bungalow care home provides an appropriate setting for residents to receive personal care and accommodation. The home is situated in a residential area in the Farnworth area of Bolton. Structurally the home appears to be appropriately maintained and residents benefit from a large rear garden/patio which they can easily access. At the time of this unannounced inspection all areas of the home were clean, warm, appropriately furnished and residents were of the view that they were provided with a homely and comfortable environment. The inspector was informed that a programme of redecoration is shortly to be implemented in respect of resident’s bedrooms of which 9 were inspected on this occasion, these were appropriately/comfortably furnished and very The Bungalow Residential Home DS0000062979.V301753.R01.S.doc Version 5.2 Page 16 personalised. Residents said they were enabled and encouraged to personalise their rooms with their own choice of pictures, items of furniture, ornaments etc. They also said they could access their rooms at any time. Adequate and suitably adapted WC and bathing/shower facilities were provided and these were in the main easily accessible to all residents. However the WC opposite room 13 was in need of repair at the time of inspection. The inspector was informed that this was to be shortly repaired. Hot water temperatures, are controlled by thermostatic mixer valves, to prevent residents sustaining burns. A new gas boiler has been installed since the last inspection which it is hoped will improve the heating in bedroom 13 which has in the past been problematic. The 2 lounges and the dining room were very clean and have benefited from new easy chairs and dining furniture since the last inspection. Appropriate aids and adaptations were in place throughout the home that assist residents to maintain their safety and meet their physical needs. Individual aids and adaptations are provided following referral to the appropriate health care professional. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27,28,29,30 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to the service. Staff appeared to be meeting the needs of residents appropriately and staff are subject to an appropriate recruitment procedure that is consolidated by a very well planned and implemented programme of staff. EVIDENCE: Inspection of staffing rotas provided by the home indicated that staffing provision at the home complied with the current minimum Department Of Health requirements that apply to care homes for older people. Discussion with the manager at the home indicated that they were of the view that staffing levels were appropriate to meet the dependency levels of resident’s. Over 90 of the staff employed at the home have achieved an NVQ 2 (or above) qualification in care and this is to be commended. Staff who have not already obtained such qualifications have enrolled on appropriate courses to do so. Training records and discussion with staff revealed that staff training is taken very seriously by the manager of the home and a very robust and appropriate training programme is in operation that seeks to promote the health and welfare of residents. The Bungalow Residential Home DS0000062979.V301753.R01.S.doc Version 5.2 Page 18 Random inspection of 3 recently employed staff personnel files revealed that these contained an application form (including health declaration), 2 written references, proof of identity (including a photograph), and evidence of induction training and CRB checks (including POVA first checks). However the application form currently used needs to be modified to obtain a detailed history of employment from prospective applicants and a clear request that one of the referee should be their current/last employer. These ammendements can only seek to enhance the actions already taken by the home to protect residents from potential abuse. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31,33,35,36,38. Quality in this outcome area is adequate. This judgement has been made using available evidence including a visit to the service. The Bungalow care home view is being appropriately managed in a way that enables residents, their relatives and staff to feel to feel that they are being supported properly. EVIDENCE: Resident and relatives spoken to were of the view that the manager has adopted an open and positive approach to the management of the home. Both groups stated the manager was very accessible and approachable. And that any concerns were always taken seriously and responded to promptly. Records of formal staff supervision were also maintained. The registered manager is very experienced in caring and has completed the Registered Managers Award. Inspection of the arrangements to manage service users personal allowances, where these are managed by the home, found that these were secure, regularly checked and appropriately documented (including receipts). The following safety/servicing certificates were inspected at the time of inspection and found to be up to date; 5 yearly electrical safety certificate, servicing of lifting equipment, servicing of fire safety equipment, the fire alarm system and gas safety in the home. A current gas safety certificate was not available for inspection. Accidents that occur in the home were appropriately recorded and appeared to have been appropriately managed.
