This inspection was carried out on 13th December 2005.

CSCI has not published a star rating for this report, though using similar criteria we estimate that the report is Good. The way we rate inspection reports is consistent for all houses, though please be aware that this may be different from an official CSCI judgement.

The inspector found there to be outstanding requirements from the previous inspection report but made no statutory requirements on the home.

What the care home does well

The home continues to provide good homely care. Residents spoken with were complimentary of the care they received at the home. One gentleman who had initially come to the home on a respite basis had now decided to stay on a permanent basis. He was extremely praiseworthy of the treatment he received stating, "It couldn`t be better".

What has improved since the last inspection?

The maintenance and decoration programme within the home continues. Some further radiators have now been provided with covers. Training for staff continues to be addressed and whilst the number of qualified staff remains low, the homes, management are attempting to address this via a partnership with a training organisation.

The home is registered to provide care and accommodation for up to 30 older people. Accommodation is provided on two floors, with a passenger lift allowing access to the first floor. Access to two rooms on the first floor still requires the negotiation of a short flight of stairs. Local shops, health services and large retail shops are within close proximity to the home, with local bus services available adjacent to the home. The Cottage DS0000000841.V274279.R01.S.doc Version 5.1 Page 5 SUMMARY This is an overview of what the inspector found during the inspection. This unannounced inspection was carried out over 4 hours. The manager and deputy manager were spoken with, a tour of the premises was undertaken and residents were spoken with. Whilst the home is registered for 30 residents, they presently accommodate 26, the shared rooms being used as single accommodation. What the service does well: What has improved since the last inspection? What they could do better: Work remains to be needed to the premises. The exterior paintwork requires attention and those radiators, which do not have covers or low surface temperature provision still need to be addressed. Only a quarter of the homes care staff are qualified to a level equivalent to NVQ level 2, The requirement is for at least 50 of staff to be qualified. This matter must be addressed urgently. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 8 & 10. Health care needs of residents are met and their privacy is respected. EVIDENCE: One resident spoken with at length explained how his health care needs had been promoted and met with the assistance of the homes management. He still had some unresolved issues following treatment and the homes management were helping him to ensure these were not forgotten. The resident explained that his privacy was respected by all staff at this difficult time. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12 & 13. Contact with visitors and the local community is encouraged. Activities and entertainment continues to be provided. EVIDENCE: Activities and entertainment within the home continues to be provided on a regular basis, with specific themes having been planned for the forthcoming Christmas festivities. One resident said he didn’t wish to join in the entertainment at the minute but enjoyed watching sport on the television in his room. Visitors continue to be able to call at any reasonable time of the day, one male resident said that his wife visits six days a week. Service users live in a safe, well-maintained environment. Service users have access to safe and comfortable indoor and outdoor communal facilities. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19 & 25. The home provides a comfortable and clean environment for residents. Some areas of maintenance must be addressed. EVIDENCE: The home continues to be generally well presented and was clean and hygienic. The small lounge has now been completely redecorated and the larger lounge has redecoration underway. All the downstairs areas of the home are to have new carpets fitted in the spring of 2006. More radiator covers have been provided since the last inspection, however there are still some that are unguarded. The manager explained that these covers were having to be made to measure and that they would be fitted as soon as possible. These must be provided as soon as possible. The Cottage DS0000000841.V274279.R01.S.doc Version 5.1 Page 13 The external paintwork to the home has not received any attention since the last inspection, the manager stated that this would be addressed as soon as the weather allowed. A sash cord was observed to be broken on one of the downstairs windows. It is important that this is addressed and all other windows are checked as soon as possible in order to ensure safety. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27 & 28. The mix of staff member’s skills enables resident’s needs to be addressed. Formal qualifications do not meet the required standard. EVIDENCE: The home employs a group of sixteen care staff, who are a mixture of young and more experienced carers. There are two male carers employed in this group. The differing backgrounds and experience of care staff ensures that the day to day needs of residents can be met. The number of care staff who hold an NVQ qualification at a level 2 or equivalent remains low. Only three staff have completed the qualification, approximately 20 of the care staff group. Four staff members are undertaking the course and five more have recently registered. The home is using the services of the HYA training organisation. The home has three staff that are NVQ qualified assessors. The assistant manager is undertaking an NVQ 3 qualification. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 35. Resident’s financial interests are safeguarded by the homes practice. EVIDENCE: The homes management does not act as an appointee for any residents; solicitors or family members are used where appropriate. A policy is available in regard to this area. All fees are paid directly to the homes proprietor. Any resident’s monies held are held individually with a record of all ins and outs held which is audited on a weekly basis. Any valuables held on resident’s behalf are recorded on individual care plans.
