CQC found this care home to be providing an Good service.

The inspector found there to be outstanding requirements from the previous inspection report. These are things the inspector asked to be changed, but found they had not done. The inspector also made 3 statutory requirements (actions the home must comply with) as a result of this inspection.

What the care home does well

Service users are supported in being able to self medicate. A good P.R.N. protocol for medication was put into place to support one person who has anxieties about traveling. This protocol had been devised after consultation with the persons G.P.

What the care home could do better:

Risk assessments and care plans detailing the support that should be given to people taking social leave or holidays should be in place. This will ensure that their needs continue to be met. Guidance should be produced for staff to follow when service users are planning holidays away from the home. Medicines must only be administered from the containers supplied by the pharmacy. The medicine policy should be updated in line with current guidance so that staff understand how to administer medicine safely.

The Grove Suite incorporates a 15 bedded unit for older people. The Lodge provides 14 nursing beds for people with dementia. The Cleveland provides 14 beds and The Courtyard 12 beds for younger adults with a mental disorder of a neurological cause. All rooms have ensuite facilities which include toilet and wash-hand basin and some rooms include a shower. The home is located centrally in Middlesbrough; it is on a busy main road, close to public transport, shops, public houses and churches. The weekly fees for The Grove and The Courtyard range from three hundred and ninety five pounds to five hundred and twenty five pounds. Care Homes for Older People Page 3 of 9 What we found: This home was last inspected on 15th December 2008. It was rated as good. In August 2009 we received some information that indicated that the home may have breached regulations. It was decided that a random inspection visit would be carried out. The random inspection visit was carried out on 25 August 2009 by two inspectors, one regulatory inspector and one pharmacist inspector. This inspection focused on the arrangements that are put into place to support service users who spend time away from the home for example on social leave or holiday. We did not look at evidence to confirm that any previous requirements had been met. These will be looked at during the next key inspection visit. We looked at the care plans and medicine administration charts for three people who have recently spent time away from the home. We also talked to staff who are responsible for these arrangements. We found that none of the individual service user files that we looked at had evidence of a social leave risk assessment or care plan being put into place. We would have expected that these documents would be required to identify the care and support that was needed to facilitate each persons holiday or social leave arrangements. Care plans covering social leave arrangements were produced for these people before our inspection visit ended. We asked to see any guidance that the home had for the preparation of holidays and short breaks away from the home. The only guidance that was available to us was the policy for issuing medication for holidays and short breaks. The medication administration records that we looked at showed that on occasions the policy had not been followed correctly by staff. For example, for one person a separate dispensing arranged to cover a holiday period had not been collected on time resulting in medication being dispensed in an unsatisfactory manner. We also found that the procedure of recording medication given for short breaks and holidays and had not been followed for two people. We found that staff had not used the correct code on the front of the medicine administration record indicating social leave had been taken. And staff had not recorded medication, that had been issued for social leave, on the back of the medicine administration record. Signatures to confirm that the medication had been issued by the home or returned to the home were also missing. One person had a risk assessment covering medication for social leave but this had not been signed by the person who had completed the assessment. We looked at the process being followed for service users who self medicate and found that the practices being used were not covered by any policy documentation. For example, staff transferring medication from the pack supplied by the pharmacist into unlabelled weekly packs. We asked about the arrangements that are put into place when service users go on holidays for longer periods. We were shown the documents that would be completed and were shown some examples of consent forms that had been signed for a forth coming holiday. We were told that risk assessments would be completed nearer the date of the holiday.
