The service should employ a suitably qualified and experienced manager who is registered with the Commission for Social Care Inspection The home may accommodate up to 23 persons in the category OP (older persons). 17th January 2006 Date of last inspection Brief Description of the Service: The Knowle is registered as a care home providing accommodation and personal care for up to 24 older persons of either sex aged 65 years and over. There are 21 service users presently accommodated. The home is situated in a residential area on the outskirts of Preston. The Knowle is a large Victorian property that is in keeping with the surrounding area and it has retained many of its original features. Service users accommodation is provided on ground and first floor and access is provided by a passenger lift. There are fourteen single and five double bedrooms, five of which have on-suite facilities. Communal areas comprise of two lounges, a dining room and a conservatory, these are situated on ground floor. There is a pleasant secluded garden at the rear of the home with ramped access. The Knowle is reasonably close to shops and local amenities, and has excellent transport links for the city centre. The fees at the home are £365 per week. This is an overview of what the inspector found during the inspection. This site visit took place unannounced. During the visit residents, staff and visitors were spoken with. Records and information held at the home was looked at. The premises were toured. Information was provided in a Pre Inspection Questionnaire completed by the manager, no resident comment cards were returned. What the service does well: What has improved since the last inspection?
The deputy at the home has worked hard to improve care plans, these are now all kept in order. She has also compiled some very interesting social histories with residents and their families, these have been useful in helping to provide activities and support in line with the needs and expectations of residents. The care plans also contain an emergency admission sheet, which details all relevant information should a resident be admitted to hospital. Some fire doors have been replaced following an inspection by the local fire service. Following the last inspection an outing was organised to a local pub. Some residents really enjoyed this. The manager is to look at arranging further visits. Staff members are working hard to find activities that residents enjoy. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 3 Quality in this outcome group is good. This judgement has been made using available evidence including a visit to this service. The Knowle provides written information and encourages all prospective residents to visit, pre-admission assessments are completed and these processes ensure that a decision to move to the home is based on a good understanding of both the service offered at the home and on an understanding of the support needs of residents. EVIDENCE: Some residents spoken with had come to the home directly from hospital, and had not had a chance to visit but had received written information about the home. Other residents had visited the home prior to moving there, and had found that the home lived up to their expectations. The files looked at during the inspection contained relevant pre-admission assessments including hospital discharge assessments and social services assessments. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7,8,9,10 Quality in this outcome group is good. This judgement has been made using available evidence including a visit to this service. The health and personal care needs of residents are identified and regularly reviewed, this ensures that appropriate support is offered. EVIDENCE: Each resident has a care plan which sets out their support needs. There are assessments of all aspects of care for example, mobility needs and dietary requirements. In some cases these assessments were signed by the resident. Each resident has a care plan that covers their needs during the day, and also another plan that covers their needs during the night. The care plans were informative and regularly reviewed. Daily notes were maintained. The medical needs of each resident were understood and documented. There was evidence on file of respect of the wishes of individual residents, for example if they wish to refuse hospital treatment. Residents spoken with felt that staff would access any help they needed. The district nurses are involved with a number of residents and any instructions left are recorded and followed by the staff. Those residents who wish to retain control over their own medication are able to do so based on an individual risk assessment. Senior staff have recently completed a medication course which involved completing a written exam. Patient information leaflets are kept on file to ensure that staff are aware of contra indications of medicines. Each resident has a Hospital Admission sheet prepared, which details relevant information including medications taken, and any allergies. Staff act in ways which respects the privacy and dignity of residents. Staff were observed providing support in inclusive ways, for example speaking with the resident and asking their opinion about the support being offered. Residents choices are respected, for example residents choose when to get up and go to bed, residents said that they were able to decide how to spend their time. Residents said that their opinions were regularly sought and listened to, an example of this was the purchase of new garden furniture after a request by a resident. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12,13,14,15 Quality in this outcome group is good. This judgement has been made using available evidence including a visit to this service. The social, cultural and religious needs of residents are identified and these are reflected in the activities and social life of residents. EVIDENCE: At the time of admission information is gathered about religious and cultural requirements. This information is supplemented by the compilation of a social history, this ensures that staff have an understanding of the life experience and lifestyle of a resident previous to them moving the The Knowle. Residents have regular visits from Roman Catholic and Church of England clergy, and arrangements to attend the church of their choice can be made. A visitor commented that he was always made to feel welcome and was very satisfied with the standard of care he observed at the home. Staff have recognised the difficulties faced by those residents who use wheelchairs, and have ensured the home is fully accessible. The deputy is investigating local transport available that will enable wheelchair users to take part in group outings. The activities held at the home, for example aromatherapy and musical entertainment, are included in the weekly fee. Individual interests are also encouraged for example one resident has her own

Regular outings from the home should be available, there has been one trip out which was enjoyed by residents, however in the main residents are reliant on relatives and friends if they wish to go out and about. Residents where pleased with the quality of food at the home. Residents have a choice at meal times. The chef is aware of dietary needs, and an assessment is completed at the time of admission. During the visit residents were seen to enjoy meals of their choice, for example one gentleman was enjoying a full cooked breakfast, another preferred a lighter option. The complaints and adult protection procedures at the home safeguard the welfare of residents. EVIDENCE: There have been no complaints made to the home or to CSCI since the last inspection. The home has a complaints procedure which is understood by residents and staff members. A record of complaints is kept on each residents file. Residents spoken with felt that if they had any concerns these would be dealt with by the care staff. Staff spoken with were aware of adult protection procedures. All staff have received formal training in these procedures through their National Vocational Qualification studies. JUDGEMENT – we looked at outcomes for the following standard(s): 19, 26 Quality in this outcome group is good. This judgement has been made using available evidence including a visit to this service. The home is warm and clean, and there is an ongoing programme of redecoration. EVIDENCE: The home has a range of communal areas, including a large conservatory leading on to a pleasant back garden. All areas of the home are accessible for residents. The home is furnished to a good standard. There have been a number of rooms decorated and repairs have been carried out to the downstairs shower room. There are plans to redecorate the lounges. A number of fire doors had been replaced following a visit from the fire service. Residents liked their bedrooms and were able to bring their own furniture if they wished. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27, 28, 29, 30 Quality in this outcome group is good. This judgement has been made using available evidence including a visit to this service. The home is staffed in accordance with the needs of the residents. The recruitment procedure ensures that residents are protected. The Knowle has exceeded the recommended ratio of staff who have completed an NVQ ensuring that the care provided is based on an understanding of good care practices. EVIDENCE: Staff are available in line with the needs and number of residents. The staff have all completed National Vocational Qualifications at level 2 or above. The residents felt that the staff were helpful and respectful and enthusiastic about their work. The home operates a sound recruitment process and all staff have appropriate checks completed before commencing employment at the home. Staff have on going training opportunities and undertake mandatory training in Moving and Handling, First Aid and Basic Food Hygiene. Each member of staff has an individual training record. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. This judgement has been made using available evidence including a visit to this service. Resident’s views are sought and taken account of. The manager encourages an open and positive atmosphere. The health and safety of residents and staff are promoted through the policies and practices of the home. EVIDENCE: The manager of the home has yet to complete her NVQ level 4, she is committed to doing so but is finding it difficult to find time to complete this course. She is well supported in her role by her staff team. The owner is a regular visitor to the home and has frequent contact with residents. There are periodic resident surveys to gather information about satisfaction levels at the home. The home has Investors in People quality assurance award. The home does not hold responsibility for the finances of any resident, all residents control their own finances or have a representative.
