Located on a residential estate on the outskirts of Crewe the home is close to shops, church and other local facilities. It is also on the bus route to Crewe town centre. All the bedrooms in the bungalow are single and contain handwashing facilities. Sufficient bathrooms and toilets are provided for service users. Communal space consists of a separate dining room and a lounge which leads out to a secure garden to the rear of the home. The home has a people carrier which is used to provide transport for service users. Provision has been made for service users with mobility problems. Staff are on duty twenty four hours a day to deliver care to service users. This is an overview of what the inspector found during the inspection. This inspection was carried out over four hours on the 2nd August 2005 as part of the yearly inspection process. One hour was spent preparing for the inspection that included reading the previous inspection report and reviewing the service history for the home. All the residents were spoken with during the inspection. Two relatives and the three staff on duty were also spoken with. A partial tour of the building was carried out. Two residents’ records were seen as were a number of the home records. Residents have limited communication capabilities. What the service does well:
Staff are aware of the needs of residents and how these are to be met. During the inspection staff were seen spending time with residents and talking to them. Residents and relatives said staff are helpful and caring. Relatives said their concerns/worries are listened to and acted upon. A number of staff have worked in the home for some years and are encouraged to attend courses to further develop their caring skills. Residents are offered a choice of meals. Advice is sought from dieticians on the variety and nutritional value of the food. Daily activities for individual residents are included in their care plans. These can be organised by staff from the home or from the organisation’s daily options scheme. As well as attending community day care residents have been enrolled on college courses. One resident has an allotment which has been adapted so that people with a disability can attend to it. Two files are kept on each resident. One has detailed information on the resident’s background, care needs, risk assessments, financial information and other essential information. The second file contains detailed information on the residents’ healthcare and the level input from doctors, nurses and other healthcare professionals. Residents’ opinions and views are sought on issues such as food, decoration and furnishings. They are also consulted on issues such as the day-to-day running of the home. Prospective users’ individual aspirations and needs are assessed. Prospective service users’ know that the home that they will choose will meet their needs and aspirations. Prospective service users have an opportunity to visit and to “test drive” the home. Each service user has an individual written contract or statement of terms and conditions with the home. The Commission consider Standard 2 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 1/2/3/4/ Residents’ care needs are identified, monitored and action taken to meet these needs. The statement of purpose and service user guide ensures information is available on the services offered by the home. EVIDENCE: The current service user group have lived in the home for a number of years. Residents plans of care showed that staff have identified their individual care needs and how these needs are to be met. Because of their limited verbal skills the care offered to service users is closely monitored and any changes recorded. A statement of purpose and service user guide was available which showed the services offered by the home. It is the policy of the organisation that prospective residents are able to visit the home before making a decision about moving in. The two relatives spoken with said the home continues to meet the residents needs. Also that the home is looking at various courses/activities that will help fulfil the residents. JUDGEMENT – we looked at outcomes for standard(s) 6/7/8/9/ Residents’ care needs have been identified and included in their plans of care. The home is able to meet these needs. Risk assessments identify possible dangers to residents and safe guards are in place to protect them. EVIDENCE: The plans of care contain a wide range of information on residents and how their assessed care needs are to be met. Included was the level of staff support required by residents in their daily lives. The care files seen were well laid out and contained information which was up to date. Assessed care needs of residents showed that they need staff support with every aspect of daily living. This includes using the bathroom, toilet, eating and moving about the home. Staff were observed helping residents with these tasks in a sensitive and caring manner. Wherever possible residents are encouraged to take responsible risks as part of their independent lifestyle, for example, when they go out in the garden, moving about the home and using local shops. Risk assessments are in place which identify health and safety issues for service users. They said in the years they have been visiting the home they have always seen staff help and support residents. Examples given by the relatives include asking staff asking residents to choose the décor for their bedrooms. JUDGEMENT – we looked at outcomes for standard(s) 11/12/13/14/15/16/17 Residents are offered a varied and stimulating lifestyle and are appropriately supported by staff. The menus reflect the residents’ preferences and choices. EVIDENCE: Plans of care showed that residents are involved in appropriate activities. This can include hydrotherapy, attendance at a community day centre and day care services provided by Social Services and attendance at college. On the day of the inspection residents, supported by staff, were playing board games. Also one resident was out with her parents. The manager said that service users are supported to use local shops and other amenities. Included in the plans of care was the level of staff support required by service users to access these amenities. Relatives said the home has arranged holidays for the residents. This was confirmed in the care plans. The relative spoken with said they were renting a specially adapted allotment for the resident. The record of menus showed that choices are offered to residents which reflect their personal preferences. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 18/19/20 Residents’ health and personal care needs are set out in their plans of care. The home ensures these needs are met. Staff give full support with residents’ health and personal care needs EVIDENCE: Staff were observed helping residents with personal care tasks, for example, using the bathroom, having a drink. They were also seen talking with residents and responding to their individual personal needs. The plans of care showed service users’ preferences on how they wished to be cared for are recorded. Because the current resident group have lived in the home for some time staff have developed an awareness of their care needs. A separate file is kept on each resident in which their healthcare needs have been assessed and recorded. Plans of care seen showed that residents receive visits from doctors, nurses and other health professionals. The reasons for the visits and the recommended treatment were also recorded. The manager confirmed that providing a comprehensive level of personal and healthcare is a priority. Staff support residents to attend hospital, dental and other appointments. Satisfactory arrangements were in place for the administration of medication to residents. Residents require full assistance with the administration of medication. They also said they are kept informed of any incidents/accidents involving the resident. The intended outcomes for Standards 22 – 23 are: 22. 23. Service users feel their views are listened to and acted on. Service users are protected from abuse, neglect and self-harm. The Commission considers Standards 22, and 23 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 22/23 Satisfactory procedures are in place to respond to complaints and adult protection issues. Relatives know who to see if they have any complaints about the home. EVIDENCE: A copy of the complaints procedure was available on the day of the inspection. Details on how to contact the Commission for Social Care Inspection were included in the procedure. The home has received one complaint since the last inspection. This was from a neighbour and was concerning work carried out by staff on the garden. The issues raised were resolved satisfactorily. CSCI have not received any complaints about the home. The Trust has produced an Adult Protection procedure, a copy of which is kept in the home. Also available was a copy of the government guidelines ‘No Secrets’. Staff spoken with knew about the complaints and adult protection procedures and what action to take if a problem arose. Shared spaces complement and supplement service users’ individual rooms. Service users have the specialist equipment they require to maximise their independence. The home is clean and hygienic. The Commission considers Standards 24, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 24/25/26/27/28/29/30 A safe and comfortable standard of accommodation is provided for residents. There is enough shared space to meet the needs of residents. EVIDENCE: The home is a domestic type building that provides residents with a wellmaintained, comfortable and homely environment. Bedrooms are individually decorated and furnished, hoists/lifting aids are also provided to assist residents with mobility problems. Bedrooms also contain residents’ personal possessions such as photographs and ornaments. One resident said she chose the colour scheme as well as the furniture for her bedroom. Another resident was happy to show the changes that had taken place in his bedroom Sufficient toilet and bathing facilities are provided for the residents. These areas have doors that can be locked which ensures the privacy and dignity of residents is protected. There is a secure, well-maintained garden area at the back of the home. The home was clean, bright and free from unpleasant smells. Relatives spoken with said the home has always been well decorated, clean and tidy. They also said staff ask residents’ opinions about any changes/maintenance work to be carried out in the home. JUDGEMENT – we looked at outcomes for standard(s) 31/32/33/35/36 Staff are employed in sufficient numbers to meet the needs of residents. Residents are cared for by appropriately trained and supervised staff. EVIDENCE: The staffing rota showed that there are normally two staff on duty during the day/evening and one waking plus one sleeping in staff during the night. These numbers can include the registered manager. The Lady Verdin Trust provide on-going training for staff including NVQ training in care this will help staff further improve their skills in caring for service users. A number of staff have achieved NVQ Level 2/3. The Trust also provides induction training for staff called Supported Assistant General. Staff spoken with said they receive supervision and support from the manager which ensures they are aware of their responsibilities in caring for residents. Staff meetings are held where staff roles and responsibilities are also re-enforced. Details on staff are kept in the home as required. During the inspection staff were observed providing personal care to residents. They also spent time with residents playing board games and talking with

Residents were comfortable in the company of staff and were able to approach them for help. Residents, and relatives, said the staff are helpful and will do what they can to improve their quality of life. For example, when one resident was recently admitted to hospital staff visited every day to ensure he was being cared for. Staff were concerned that, because of the resident’s limited communication, he may not have received the level of help he needed. JUDGEMENT – we looked at outcomes for standard(s) 37/38/39/42/43 The arrangements for managing the home are satisfactory. The health, safety and welfare of service users underpins the overall management of the home. EVIDENCE: The manager has worked for the Trust in a senior capacity for a number of years. She is also the registered manager for another home that is run by Lady Verdin Trust and is located in Crewe. Training on the day-to-day skills required to manage a care home is provided by the organisation’s training manager. The manager has the experience and knowledge to ensure the home is run in the best interest of residents. Staff confirmed that the management approach encourages input from residents, relatives and staff on the services offered. They also said the manager offers leadership and direction to ensure the assessed care needs of

The staff spoken with said they receive support and supervision from the manager. The Trust have provided a comprehensive range of policies and procedures, copies of which are kept in the home and are available to staff. These and other records have been provided to ensure residents’ rights and best interests are protected. A partial tour of the building was carried out which showed that health and safety issues are addressed. The fire safety record seen showed that regular fire checks, drills and staff training is carried out. A letter was received from the Borough of Crewe and Nantwich (Environment and Development Department) which said that at their last inspection ‘no major contraventions of’ the Food Safety (General Food Hygiene) Regulations 1995 were found.
