This inspection was carried out on 1st August 2005.

The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

The first impressions of the home are that it is welcoming and comfortable. Many residents and their families know and are known by the manager and the home`s staff before they come to live at The Laurels, so the home is truly part of the local community. The atmosphere in the home open and welcoming, during the inspection 2 people visited looking to accommodate their relative, the manager and staff were observed to welcome them warmly and to take time to show them round the home and give them the information they need to make an informed choice.

What has improved since the last inspection?

One of the lounges has a new carpet and has been re-decorated since the last inspection. Residents have been provided with a portable ramp to enable people with mobility problems to go out of the patio doors and into the patio area more easily. The manager has organised a new care planning system, which gives clear accurate and detailed guidance for staff about the care to be delivered. The manager has completed the training and qualifications required by the National Minimum Standards [NMS] for Older People.

Date of last inspection 27/10/04 Brief Description of the Service: The Laurels is a care home providing personal care and accommodation for 28 older people. It is owned by Superior Care Homes Ltd a privately owned limited company.The home is situated close to the centre of South Kirkby, a small former mining community.The premises a former vicarage that has been adapted and extended. Service user accommodation is arranged on two floors and there is a passenger lift.There are twenty bedrooms for single occupancy of which one has en-suite facilities. There are four bedrooms designed for shared occupancy of no more that 2 people. The home, which is close to local amenities, has a car park to the front and gardens and a patio area to the rear. The home was running well on the day of inspection. People spoken with during this unannounced visit said that they liked living at the home and that staff are kind and helpful. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 3&4 Resident’s are assessed before they are admitted so that they know that the home can meet their needs. EVIDENCE: Resident are only accepted into the home following the receipt of an assessment of need from the placing agency on which the home then bases their assessment. The manager undertakes pre-admission assessments for all prospective service users. This process ensures as is possible ensure that the home is confident prior to admission that the needs of service users can be met. Records show and people said that they were invited to visit the home prior to admission so that they and their family/representative are sure that the home can meet their needs. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7 &9 Detailed care plans are in place for all residents. Residents are protected by the homes policy and procedures for dealing with medication. EVIDENCE: The care plans of 3 most recently admitted residents were inspected this included 1 resident on respite care. All plans were set out clearly and gave detail of the care to be provided how it would be delivered and they when reviews would be undertaken. There is clear evidence and resident/relatives and staff said that all relevant people are involved in the formulation and review of care plans. Individuals have a risk assessment in place that shows presently, no residents take responsibility for their own medication. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12 &13 Resident’s lifestyles match their expectations and people keep in contact with their relatives and friends. EVIDENCE: Residents spoken to said that they and the staff recently had a lovely day in Bridlington, the photographs displayed were evidence that all people enjoyed themselves. Residents and family members said that the home has activities and that lots of things ‘go on’ in the home. From direct observation it is clear that The Laurels is a vibrant home with much relaxed communication between residents, staff and visitors. The home’s Statement of Purpose and Service User Guide show that people can have visitors at any time. The Inspector met with a range of people visiting the home during this unannounced inspection. Service users have access to the local community through their family/representatives and through local church/ community group visitors. Service users where possible go into the local community either independently or with the assistance of staff. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16 Complaints are taken seriously and acted upon. EVIDENCE: Residents spoken with said that they haven’t had reason to complain and went on to say that if something is ‘not right’ they only have to tell staff and it will be dealt with. Records show that serious complaints are few and that all concerns and complaint are recorded and dealt with appropriately. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 20 & 26 Residents have safe access to all communal areas of the home. The home is clean. EVIDENCE: From direct observation it is clear that the private and communal areas of the home are decorated and furnished to a high level. One sitting room has recently been re-carpeted and decorated. There are large televisions, radio, stereo equipment and DVD players provided. A wellmaintained garden to the rear and a safe accessible patio area gives residents access to the external areas of the home safely via a large, bright conservatory. Overall the environment is clean, bright and pleasant. Service users needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27 &28 There are sufficient experienced and qualified staff to meet the needs of people accommodated. EVIDENCE: On the day of inspection there were sufficient staff to meet residents assessed needs. The manager maintains a staff rota, which shows that there are at all, times sufficient staff. The manager said that 43 of care staff have achieved the National Vocational Qualification [NVQ] level 2 or above, 2 further staff are presently being trained at Level 2. Staff undertakes regular mandatory training required by the care industry such as Health & safety, moving and handling, first aid, Food hygiene etc. The manager maintains a staff training matrix which highlight where training or re-training is required. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 31,32 & 38 The home is managed by a qualified and competent person whose leadership skills benefits the home. The health & safety of residents and staff are protected. EVIDENCE: The home manager has achieved an NVQ at level 4 and a regsitered manager’s award. It is clear from observation during this visit and from speaking with residents, visitors and staff that the manager’s leadership style and management approach benefits the home and that at all times the home is run in the best interests of residents. Records show that the registered person ensures the health and safety of residents and staff at all times.
