Conditions of registration: None Date of last inspection 7/10/04 Brief Description of the Service: The Limes is a care home providing personal care and accommodation for up to 32 older people. It is located in a residential road in Bembridge village; a few minutes walk from a range of local shops and the nearby beach. There is some off road parking for visitors or on road parking is to the front of the premises. The accommodation offers a range of mostly single rooms on 3 floors, all with en-suite facilities. Access for residents is via a passenger lift to all but 4 rooms on the mezzanine floor, which are not suitable to be occupied by people with mobility difficulties. This is an overview of what the inspector found during the inspection. The inspection took place unannounced over 6 ½ hours. Residents were variously located in the lounges, conservatory or in their own rooms. The atmosphere was friendly and very relaxed. A tour of the premises took place and a selection of records was inspected. Two staff and two visitors were spoken with. Three residents were spoken with in the privacy of their rooms and several others as a group in the dining room. Comments about the service were extremely positive and no concerns were raised. On 25 January 2005 an additional inspection of The Limes was undertaken to look at standards relating to specific issues following an accident in the home. All standards inspected at that time were found to be met. What the service does well: What has improved since the last inspection?
Since the last inspection progress has been made with a programme of covering the radiators in residents’ rooms, with the majority now covered. A new bright and airy laundry has been built at the rear of the building with new industrial appliances installed. The home has achieved the Investors in People Award. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 3 and 4 The manager ensures that the care needs of the people who live at The Limes will be met by undertaking a proper assessment prior to them moving into the home. The home ensures that residents’ needs will be met with appropriate equipment, access to specialist services and the skills and experience of the staff. EVIDENCE: The newest resident admitted to The Limes three weeks before the inspection had come from the mainland. While a personal visit for a pre-admission assessment was not practical the manager undertook the assessment by way of telephone consultations with all interested parties. Records showed that the assessment covered all relevant areas of need. During the inspection there was an opportunity to speak with this resident’s son who said that he chose The Limes after having viewed several others. He and his mother were very positive in light of the fact that they had had only three weeks to experience the service.

Records showed the assessment was used to ensure that appropriate equipment was in place to meet her change in needs. The Limes has an experienced and stable staff group with no turnover of care staff since the last inspection. 75 of care staff are qualified at NVQ level 2 with others undertaking the training. The home has a range of equipment to enable staff to transfer residents safely including a standaid, hoist, two Parker baths and a wheel in shower. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7,8 and 9 The home has a system of care planning with an individual plan for each resident. They demonstrate that residents’ health care needs are identified and met. Medication is securely held and appropriate records are maintained. EVIDENCE: The principal of case tracking was used in a sample of care plans. The intention was to look at the outcomes for residents in general by assessing all areas of care for those sampled. The sample included a resident with complex needs, a resident who had recently returned to the home from hospital, and the newest admission to the home. Plans were seen to be comprehensive. They contained admission details, a long-term needs assessment, psychological assessment and plan of care. Risk assessments were in place and reviews were up to date. Plans showed that a range of health care professionals was consulted where appropriate, including psychiatry, GPs, and the infection control nurse. Additionally, they contained information on the progress made with mobility and pressure areas for one, and how the home was coping with the deteriorating health of another. Only

Four residents and a relative expressed, in written comments, satisfaction with the care provided. While controlled drugs are held under secure conditions it is recommended that the present cabinet be replaced with a metal one. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 14 and 15 Residents are supported to manage their own financial affairs for as long as they are able. Where this is not possible family or advocacy are brought in to assist. The promotion of choice extends to all aspects of daily living including personalisation of rooms, and meals. EVIDENCE: The manager explained that some residents manage their own financial affairs but most have family to assist. One resident is under the Court of Protection. She said that while currently advocacy is not required she has accessed the service for a resident in the past and is familiar with the procedure. Information about independent support is available to residents and their representatives in the service users’ guide, a copy of which can be found in the resident’s room. Some of the rooms visited during the inspection showed a good level of personalisation. One very articulate resident was very clear about choices in the home, saying that there are no regimes and she can do whatever she wants. As at previous inspections there was a high level of satisfaction with the food provided. One resident described the food as ‘exceptional, home cooked’ and another said it was ‘very good indeed’. While the home operates a five week menu the manager said that basically residents can have whatever they like. Menus showed food to be varied and nutritious. Lunch appeared to be a very sociable

JUDGEMENT – we looked at outcomes for standard(s) 16 The home treats residents’ complaints seriously and responds appropriately. EVIDENCE: The home has a complaints policy and procedure which is included in the service users’ guide. There was a complaints register in which the last recorded complaint was April 2004. The manager said that minor issues are dealt with immediately but no formal complaints about the service had been made in the past year. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19 and 26 All areas of the home are kept clean and tidy and free from unpleasant odours. The location and layout of the home is generally suitable for its stated purpose in providing a safe and comfortable environment for those who live and work there. Some improvements have been made since the last inspection. EVIDENCE: Residents at The Limes are accommodated in spacious single rooms, all with en-suite facilities. A passenger lift provides access to rooms on the first floor except four on the mezzanine level. These rooms are only occupied by people who are able to use the stairs without assistance. There is ramped and level access to the rear garden in which there is seating for residents during the summer. There is an ongoing programme of maintenance and decoration. Since the last inspection radiators in many of the rooms have been covered on a risk assessment basis. There was evidence that the process was ongoing.

Residents spoken with expressed general satisfaction with their accommodation. Two made special mention of the pleasant outlook they had and one was particularly pleased with her room having the sleeping area separate from where she sat. While there were no unpleasant smells detected the manager pointed to one room where a change of floor covering is planned to better manage a problem with incontinence. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 29 and 30 Staff at The Limes have the necessary skills and experience to meet the needs of the people who live there. Staff turnover is low and a robust recruitment procedure ensures residents are protected. EVIDENCE: There have been no new care staff recruited since the last inspection when recruitment records were checked and found to be in good order. 75 of care staff are qualified at NVQ level 2 and others are undertaking the training. Some staff have achieved the qualification at level 3. Each member of staff has a training profile. A selection was viewed. Training needs are identified at staff supervision and as well as statutory training dementia awareness, coping with Parkinson’s disease and adult protection are included. Two members of staff were spoken with. They confirmed that training is ongoing and the manager arranges for courses as and when they become available. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 31, 35 and 38 The registered manager has the experience and qualifications to run the home and meet its stated purpose, aims and objectives. The home provides a sound system to ensure residents’ finances are safeguarded. While policies, procedures and staff training generally are in place to ensure so far as is reasonably practicable the health, safety and welfare of residents and staff, records of weekly fire alarm tests must be regularly maintained. EVIDENCE: The manager Mrs Preston had several years’ experience as the deputy manager at The Limes before becoming the registered manager during 2003. She has since achieved the NVQ Level 4 in care and has completed the training for the Registered Managers Award. She was able to demonstrate that she has undertaken periodic training in various care related subjects. One of the

It was understood from the manager that the majority of residents have family to represent them. Others are able to manage their own affairs or have a solicitor to help. The integrity of the system for administering residents’ monies was examined by way of dip-sampling. Receipts were kept of transactions and records and monies balanced. Records showed that a recent fire risk assessment had been carried out for the whole building. There are policies and procedures in place to ensure safe working practices in the home. A sample of records was viewed including accidents, COSHH assessments, TOPSS induction training and the electrical certificate, all of which were in good order.
