Conditions of registration: That a maximum of two places can be used at any one time for accommodation of persons who are 55 years and over with a physical disability. Date of last inspection 23 September 2004 Brief Description of the Service: The Mains is a thirty-one bedded care home registered to care for residents requiring nursing or personal care. the home is located in the small village of Redmarshall. The accommodation consists of 21 single bedrooms, 17 of which have en suite facilities and four double bedrooms also with ensuite facilities. There is a stair lift for those residents who have need of it. There are facilities available for the residents to see family, friends and visiting professionals in private. This is an overview of what the inspector found during the inspection. This unannounced inspection commenced at 10am and lasted for four hours. Three residents, two relatives, a proprietor and the manager were spoken to during the inspection. Numerous records including care plans, policies/procedures and menus were examined. A tour of the premises was carried out. Three residents and relatives said they were happy with the way care was given. A relative talked about how when her family member had not been feeling very well and ‘off her food’ staff had gone out of their way to provide special treats for her. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 2 & 3 Terms and conditions of business for residents living in the home are in place. Residents’ needs are assessed prior to them moving to the home. EVIDENCE: Evidence was available to show that each resident has a written contract/statement of terms and conditions with the home. Three resident’s files were examined and they each contained an assessment, which had been carried out prior to the person moving into the home. The manager also carries out her own assessment, meeting with the prospective residents, either in their own home or in hospital. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7, 8 & 10 Resident’s health and personal care needs are met, however more detail is required for social needs. Residents are treated with respect and privacy is maintained. EVIDENCE: Three residents files were examined and they each contained an individual detailed plan of care. However whilst personal and health care needs of the residents were being met there needs to be more detail regarding social care and how residents spend their days. Three residents and two relatives said that the staff were always helpful and gave assistance when asked. They always knock on bedroom doors and wait for an answer before entering. One resident said she had originally come to stay in the home for respite care, however because of the kindness of the staff she wanted to stay and not look at other homes. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 13 & 15 Visitors are free to visit at any time and are made welcome. Dietary needs of residents are well catered for. EVIDENCE: During the inspection two relatives were spoken to. Both said they visit every day and were always made welcome. There is a small kitchen in the home specifically for relatives/staff to make drinks and both relatives said they make good use of it. The inspector noted the good relationship between staff and the visitors and the fact that visitors appeared to feel ‘at home’. Menus showed a variety of food with choices if residents did not like the main meal offered. A resident spoken to said staff came to tell her what meals were available to choose from. She described the meals as ‘tasty and nicely cooked’. Two of the residents spoken to said they preferred to have meals in their bedrooms and that staff were always pleased to help. JUDGEMENT – we looked at outcomes for standard(s) 16 The home provides effective arrangements to ensure that residents are enabled and supported to make any complaint they consider necessary. EVIDENCE: The home has a clear complaints procedure and information about how to make a complaint is displayed around the home. Two relatives spoken to said they knew how to make a complaint but had not had to do so. They both said that relationships with the manager and staff are very good and any day to day issues are sorted out quickly. Two residents spoken to said they had no complaints and that the home’s manager and staff are very good and listen to their views. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19 & 26 The standard of the environment within this home was good. The home was clean and odour free. EVIDENCE: The home is well maintained by the proprietors who are in the home every day during the week and available for emergencies at the weekends and bank holidays. If a repair is needed staff write in the maintenance book which is checked daily by the proprietor. Certificates were in the place showing up to date servicing of equipment in the home. At the time of this unannounced inspection visit the home was clean and hygienic with good facilities and procedures for handling laundry and preventing odours or the spread of infection.
