Conditions of registration: Date of last inspection 10th February 2005 Brief Description of the Service: The Old Rectory is registered to provide care and accommodation for up to 24 older people. Mr. And Mrs. Blundred are the registered providers and Mrs Blundred is the registered manager in day to day control of the home. In addition to the Proprietors and Head of Care the Home employs, 12 care staff who work a rota which includes staff on wake duty at night. There are a further 3 who undertake meal preparation and domestic tasks. Administration is undertaken by the proprietor and management team.. Many of the staff have worked at The Old Rectory for a number of years. According to its aims and objectives the Home provides an environment in which dignity is maintained, independence encouraged and privacy respected. The Old Rectory is a detached three storey Victorian house situated in a residential area in the village of Dymchurch. A substantial extension to the home was recently completed and provides additional bedrooms, communal areas and additional facilities for service users and staff.. There is bedroom accommodation on all three floors There is a passenger shaft lift and stair lift for those who cannot manage the stairs. The Home is located in a village but with easy access to public transport. The nearest amenities (church, shops, post office and public house) are within walking distance for ambulant residents. The Home has facilities for car parking to the side of the property. The enclosed garden is well maintained and accessible for residents. This is an overview of what the inspector found during the inspection. This was an unannounced inspection visit undertaken on 14/9/05 at 10:30 am. The inspector spoke with the registered provider, staff members on duty and a number of service users. He observed staff interaction with service users, viewed a number of records and undertook an accompanied tour of the premises The Home appears to comply generally with the requirements of the NMS. Any areas for development were discussed and are identified in the body of the report. What the service does well: What has improved since the last inspection? What they could do better:
Staff shift changeovers are conducted in small groups, appraising practice and the day’s events. It was felt however that there could still be changes to the system that would improve communication and be more inclusive of the night staff. It was suggested that a photo of each service user is placed in their care plan and medication documentation as well as the misper. sheet. Environmental risk assessments are in place but need up dating to reflect the increased capacity and improved facilities. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 1,2,3,4,5, The home provides written detail of the services it offers so that prospective service users can make an informed decision about admission to the home. The admission procedure provides for pre placement assessment to ensure that the home can meet the service user’s needs. EVIDENCE: A statement of purpose relating to the service and facilities offered to existing and prospective service users. The home also has an introductory brochure/welcome pack. The Home provides Service Users with a contract / statement detailing the terms and conditions under which accommodation and personal care is provided Many of the existing staff team have worked with this service user group for a number of years. Nine have level2 NVQ qualification or above and the home has an active training programme which helps keep up to date with current practice. . Prospective service users are offered the opportunity to visit the home, take a meal, meet and spend time with the existing residents and staff and view the accommodation before any decision is made about moving in. All

One service user got to know the home through respite stays and decided then to make the arrangement permanent. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7,8,10, Personal support is offered in a way that promotes service user privacy, dignity and independence. EVIDENCE: All service users have an individual plan of care, drawn up by the management team with the support and involvement of staff, service users and relatives. These, and associated risk assessments, developed from the initial daily needs assessment, are subject to regular and frequent review. Service users access a range of community health care services as and when required. Staff are expected to monitor and record any changes in condition and alert line management and medical services when required. Staff members were observed interacting with residents in a non-patronising and respectful manner in what appeared to be a generally relaxed and friendly atmosphere. The majority of service users are accommodated their own room; screening is used in shared rooms to afford privacy. Service users may fund a phone connection to their own rooms Service users may seek solitude in their own rooms or congregate in the communal areas as they wish. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12,13,14 There is an open and relaxed feel to the home which fosters a culture of mutual respect and support. Service users are consulted as to their wishes and are encouraged to participate in the running of the home. EVIDENCE: A range of activities is offered at the home e.g. reminiscence afternoons, word games cards, visiting entertainers, singing & dancing, visits from local church group, etc. In addition, weather permitting there are outings for shopping and visiting local amenities. Service users are encouraged but not compelled to participate. One member of staff has undertaken a course to lead a ‘seated mobility exercise ‘ programme for the service users. Two service users confirmed that they may go out unaccompanied to do their own thing if they so wish and are deemed competent and safe to do so. The daily routine and lifestyle within the home appears to be generally flexible. Residents may rise and retire when they wish and bathe at their preferred time. Breakfast may be taken in bed, the service users are encouraged however to congregate in the dining room for the main meal of the day. . Residents are encouraged to express choices collectively at the service users meetings and individually.

They may be received in the communal areas, the visitors’ room or the residents own rooms. EVIDENCE: Service users spoken to said that they would approach staff and management without hesitation if they had any concerns or complaints. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19-26 Recent investment has significantly improved the appearance, accommodation and facilities. The home provides a comfortable and varied environment in which to work and live. EVIDENCE: The home is located in a quiet residential area and is relatively close to public transport. Service users have reported that it is a level walk to the village shops, the road however, is quite busy and only a few venture that far. The accommodation is on three floors with stairlift and shaft-lift access. As noted earlier the recently completed extension provides 10 bedrooms; lounge, bathrooms, toilet staff facilities, treatment room and conservatory. The Home appeared to be generally well maintained and in good decorative order. The communal areas contain a range of varied and comfortable furnishings arranged in a non-institutional manner Private areas are decorated and furnished according to service user choice and preference. .

Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): Not inspected EVIDENCE: It appeared from conversations with staff and service users and from observation that the Manager is approachable and supportive. She operates an open door policy and frequently meets with staff individually and collectively on a formal and informal basis. An open and inclusive atmosphere appears to have been established within the Home. The Inspector was assured that the service users views are actively sought in all matters relating to their lifestyle and daily routine at the Home. Service user and management meetings are a regular feature and service users are encouraged to voice their opinions at any time. The home’s ‘quality policy’ is described in the Statement of Purpose. It was reported that the Home has a limited involvement in the financial affairs of the service users. The Manager or staff members do not act as appointee for any of the residents. The Home encourages the management of finances to be undertaken by the service user or their representatives.
