Conditions of registration: Date of last inspection 28th June 2005 Brief Description of the Service: The home is situated near the coastal road at Minster and offers accommodation for 10 older people. The ten single bedrooms are located on both the ground and the first floor. There is no shaft lift access between floors and the stairs are narrow and incorporate a bend, so the home may not be suitable for anyone with very limited mobility. However, there is a sitting room on both floors, with the one on the first floor having access to a large balcony, with views over the sea.The home is on a bus route, so provides easy access to all the local amenities in the town of Sheerness, which is roughly two miles away. There is limited parking to the front of the property, but unrestricted on-road parking at the side. Service Users mostly come from the local community and retain their own GPs when moving in. This is an overview of what the inspector found during the inspection. Graham Cummings carried out the Announced Inspection on the 22nd November 2005. The Inspection consisted of speaking with the Manager, Deputy Manager, Residents, staff, relative, looking at care plans and information received in the Pre Inspection Questionnaire, 3 relative and 6 Resident comment cards were returned. The Requirements from the last inspection were discussed, the Inspector was informed that the home will have 50 of staff achieving NVQ Level 2 by the end of the year. Each Resident now has an inventory of belongings that is located at the back of the care plan. Since the last inspection, the Manager has informed the Inspector of 2 notifiable incidents. The Inspector will send a Managers application pack to Kelly Southfield. During discussions with a relative and Resident the Inspector only heard positive comments about the running of the home and the quality of care provided. The Inspector left having no concerns for the health, safety or welfare of the Residents. What the service does well: What has improved since the last inspection? What they could do better:

The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 1,2,3,6 Prospective Residents have the information available to make an informed choice. Residents have a written contract with the home. The needs of Residents are assessed prior to coming into the home. The home does not take intermediate care Residents. EVIDENCE: The homes Statement and Purpose reflects the way the home is run and gives any interested parties the required information to make an informed decision about any placement. The Inspector looked at a Residents file a saw a pre placement assessment and a written contract that contained the information set out Standard 2. The home does not cater for intermediate care. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7,8,9,10,11 Residents needs are set out in a care plan. Residents health needs are fully met. Residents are protected by the homes policies and procedures for dealing with medicines. Residents feel they are treated with respect. Residents are assured that they and their family will be treated with care and respect at the time of death. EVIDENCE: The Inspector saw a Residents file that had a care plan setting out the individual needs and care required. All of the Residents had been registered with a G.P. and have access to Opticians and Dentists. None of the Residents presently living at the home self medicate but the home does have a policy to cover this should it be needed. One Resident spoken to told the Inspector that ‘I am treated so well, the staff are always available and come quickly if I press the help button’ ‘they are so helpful and polite and nothing is too much for them’. The wishes of the Resident at the time of death was written and included in the care plan, the information was gathered from the Resident. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 14,15 Residents are helped to exercise choice and control over their lives. Residents receive a balanced and nutritious diet. EVIDENCE: Residents are able to choose if they wish to participate in any of the activities that take place, the clothes they wear, having the hairdresser, chiropodist and newspapers. The Residents are not woken in the morning but allowed to wake naturally. The Residents receive a wholesome and nutritious diet and the Inspector looked at the menu and this was confirmed. JUDGEMENT – we looked at outcomes for the following standard(s): 17,18 Residents legal rights are protected. Residents are protected from abuse. EVIDENCE: The Inspector was informed that Residents legal rights were protected, any post was always given to the individual to open, access to a phone, family, friends, Doctors and others was available. The Deputy Manager had just attended a 2 day Adult Protection training course and they are going to run short training sessions within the home to pass down the information to other staff. Service users have access to safe and comfortable indoor and outdoor communal facilities. Service users have sufficient and suitable lavatories and washing facilities. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19,24,26 Residents live in a safe well maintained environment. Residents bedrooms are safe and comfortable and included personal possessions. The home is clean, pleasant and hygienic. EVIDENCE: The Inspector toured the home and found it to be well maintained clean and tidy. The home has a cleaner who is very dedicated and proud of her role and she maintains a high standard of cleanliness. The Inspector saw a large number of the bedrooms and all were well furnished and personalised with the Individuals possessions. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27,28,30 Residents needs are met by the number of staff on duty. Residents are in safe hands. Staff are trained and competent to do their jobs. EVIDENCE: All Residents have a needs assessment carried out by staff prior to any placement taking place. The home employs 9 full time staff, the Inspector was given a rota for a 3 week period and this averaged out at 250 staff hours per week, on using the Residential Forum guidance on care hours this estimated a need for 232 hour per week. Over the last year staff have attended Adult Protection training also First Aid, Medication, Moving and Handling, Basic care and Nutrition. Plans have been made for further training in the near future to include Fire Protection, Infection Control and Adult Protection. The Inspector hoped that the Managers position would be clarified by the early part of 2006 and a fit person interview held by March 2006. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31,32,34,35,37,38 A person fit to be in charge runs the home. Residents are safeguarded by financial procedures of the home. Residents are safeguarded by the homes record keeping, policies and procedures. The health, safety and welfare of Residents and staff are promoted and protected. EVIDENCE: The day to day running of the home is carried out by the Deputy Manager and overseen by the Provider until the outcome of the Deputies Registered Managers Award which they have completed and submitted. The Residents finances are dealt with by relatives and there are policies and procedures in place should any finances need to be administered by the home. There are no indications for the Inspector to have any concerns regarding the financial future of the home. The Inspector looked at a Residents file and daily notes and found these to be fully up to date and informative. The Inspector was satisfied that the health, safety and welfare of the Residents and staff were protected. The home has notified the Inspector of 2 incidents within the home.
