This inspection was carried out on 28th June 2005.

The inspector found there to be outstanding requirements from the previous inspection report but made no statutory requirements on the home.

What the care home does well

The home is friendly and welcoming, most of the Residents have limited mobility due to their age but despite this they are encouraged to participate in lots of activities that can be done within the home, these include: skittles, netball, bingo, nail manicures and hair dressing. The Deputy Manager and staff member during conversations with the Inspector felt that the home met the needs of all the Residents presently living there. They felt the positives of the home were that they kept `the Residents safe` `listened to them` `befriend them` `read to them` `protect them` and keep the house clean`.

What has improved since the last inspection?

Since the last inspection the home has employed more care staff including a cleaner and the empty room has been redecorated. The Deputy Manager has taken a greater lead role in the running of the home and is becoming more confident in her management role, it is hoped that she will be applying for the Fit Person Interview in the October /November time this year. Training in Adult Protection has taken place with 4 staff attending the 1day course. The Deputy Manager is attending a 3 day Adult Protection course in September 2005. Four staff have started the NVQ2 in Care, once completed this should make at least 50% of the staff team qualified to NVQ levels 2/3. Both of the staff files inspected met with the standards

The ten single bedrooms are located on both the ground and the first floor. There is no shaft lift access between floors and the stairs are narrow and incorporate a bend, so the home may not be suitable for anyone with very limited mobility. However, there is a sitting room on both floors, with the one on the first floor having access to a large balcony, with views over the sea.The home is on a bus route, so provides easy access to all the local amenities in the town of Sheerness, which is roughly two miles away. There is limited parking to the front of the property, but unrestricted on-road parking at the side.Service users mostly come from the local community and retain their own GPs when moving in. Graham Cummings carried out the unannounced inspection on Tuesday 28th June 2005. The Inspector arrived at 09:35 and left at 12:45. The inspection consisted of meeting with the Deputy Manager and a member of staff, a discussion with a Resident and viewing 2 care plans, 2 staff files and a tour of the home. The home is registered for 10 residents and presently there are 8 placements made and 1 in hospital due to return soon. The Deputy Manager is essentially running the home under the guidance of the Registered Provider whilst completing the Registered Managers Award. The home had a relaxed homely atmosphere and the Resident spoken to said that they were ‘happy living at The Willows’. The staff member said that the Deputy Manager was ‘brilliant’, ‘approachable’, and ‘respected’. It is anticipated that the Deputy Manager will be applying for the Managers position in October/November 2005. The home did not raise any major concerns or worries for the Inspector. Of the 3 Requirements made at the last inspection, 2 have or are being completed and the third, making an inventory of Residents belongings will be completed by the due date of 30th August 2005. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 2,3 Residents have contracts/statements of terms and conditions on their file. The Residents have a needs assessment carried out prior to admission. EVIDENCE: The Inspector read the care plan and file contents of 2 Residents, the last Resident admitted which was 29/4/05 and one other. The files contained an assessment of need covering all items required in Standard 3.3, the only omission was that there was not a photo at the front of the file on the latest Resident to arrive. Both files contained a contract / Statement of Terms and Conditions. The Care Plans were detailed and would enable a new worker to carry out the care of a Resident in their usual routine. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7,8,10 Service Users know that their personal goals are reflected in their individual plans and that potential risks are managed. The health needs of Service Users are well met. Personal care is offered in a way protect Service Users privacy and dignity and promote independence. EVIDENCE: Care Plans were seen for two Residents, these were full and comprehensive, giving details that would enable a new staff member to carry out most tasks effectively. Both files had information regarding GP appointments and hospital admissions. When speaking to the Resident, they informed the Inspector that they ‘were happy with the staff’ and ‘they are very good and helpful’ ‘looked after her well’. The Resident was unsure why she was living there, they were living in semi-independent accommodation before going into hospital with a view to returning back home, she initially came to The Willows for recuperation, unfortunately it was assessed that she could return to Semiindependence and that she should remain at The Willows. The Inspector noted that when staff spoke to Residents they did so with respect and promoted independence and choice. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12,13,15 Residents lifestyles satisfy their social and cultural needs. Residents contact their families whenever they wish. The meals in this home are good offering both choice and variety and cater for special diets. EVIDENCE: The Inspector spoke with the Residents in the lounge and was informed that they were content with living at the home, one Resident said she would like to get out more but was aware of the difficulties due to her age and mobility. The staff are going to re-establish some of her links with the community where she lived in Sheppey. A vicar visits weekly and sits and speaks to the Residents who choose to see him. Family members are encouraged to visit whenever they can, it is preferred that mealtimes are avoided if possible but this is not essential. The home has a menu that rotates on a 3 week basis, this is changed with the seasons but the majority of Residents do like traditional English cooking of meat, vegetables and gravy. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16,18 Service Users know their complaints will be listened to and acted on. Staff have a good knowledge and understanding of Adult protection issues which protects Service Users from abuse. EVIDENCE: A new complaints book is available but has not had to be used yet, the Deputy is aware of the requirements regarding the complaints procedure, process of recording and timescales. Four staff have just completed Adult protection training course and the Deputy is booked to attend a 3 day Adult Protection course in September 2005. There is a call alarm system in every room and 1 Resident has a pressure pad alarm by the bed in case she tries to go to the toilet by herself, a risk assessment was available for this. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19,20,21,22,23,24,25,26 The standard of the environment within the home is good providing Residents with an attractive and homely place to live. The Residents rooms are comfortable and furnished with personal belongings. EVIDENCE: The Inspector toured the home and noted that the environment was well maintained, there is a Handyperson that calls into the home daily to check everything is in good condition and working order, the home has a maintenance book which is filled in with repairs that need attending to. The communal rooms were well furnished, safe and comfortable. There are suitable lavatories and washing facilities to meet the needs of the Residents, the home has a hoist and wheelchairs available should they be required. The bedrooms were all of good size and well decorated and furnished, Residents had photos of family and friends as well as personal possessions on display. The home was clean and tidy and free from any offensive odour. Service users needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27,29,30 The staff have a good understanding of Residents support needs. Residents are protected by the homes recruitment policies and procedures. Staff have access to training which enables them to be competent in their job. EVIDENCE: Four staff had completed an Adult protection course on Monday 20th June 2005 and have dates booked in the diary for staff to attend Moving and Handling and First Aide courses. The Deputy manager is attending a 3 day Adult protection course in September 2005. The Inspector noted the positive relationships, which have been formed between the staff and Residents. A total of 12 staff are employed by the home, the shift comprises of 2 staff on shift throughout the day and 1 sleep in. The Inspector had a discussion with the Deputy regarding the homes awareness of meeting the needs of the residents with the number of staff on duty and to be aware that the sleep in may at some point in the future need to be changed to a wake night. The Inspector also discussed the flexibility of the staff team between the companies other home in the local area and how they could support each other. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 32,33,36,38 Residents benefit from the leadership and Management of the home. The staff are supervised regularly. The health, safety and welfare of the Residents are promoted and protected. EVIDENCE: The Deputy Manager is responsible for the day to day running of the home under the guidance of the Proprietor and Home Manager of the other company home in the local area. It is planned that the Deputy will be putting her name forward to become Manager in September / October of this year having had more guidance in Management and almost completing her Registered Managers Award. The Residents were complimentary about the Deputy and staff team. The staff member spoken to informed the Inspector that the Deputy was ‘Approachable’, ‘caring’, ‘easy to talk to’ ‘considerate – but not a soft touch’.
