The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

Most residents said they liked living at the home and that staff were very pleasant and friendly. When asked what the residents liked about living at the home, comments included "Staff listen to you" and "Staff do errands for you". Visitors are made welcome and several visitors have regular meals with their relatives at no extra cost. Residents said they liked the food and during the inspection the food looked appetising and portions were generous. Residents generally felt that staff knew them well and understood their preferred routines. The new manager is enthusiastic and committed to improving the service.

What has improved since the last inspection?

It is disappointing that few improvements have been made at the home since the last inspection. Medicine records were slightly improved but continue to need attention to detail. The patio area was tidier and the manager had plans to purchase extra garden furniture before the summer.

Date of last inspection 25th November 2004 Brief Description of the Service: Thorncliffe Grange provides accommodation for up to 50 service users requiring personal and nursing care. The accommodation is within two converted Edwardian houses that are connected by a purpose built extension and link. The home retains many of the original features of both properties. The home is owned by a partnership and is under the control of a manager who is also a qualified nurse. Accommodation is provided over two floors. Twenty-two of the bedrooms are single en-suite whilst a further 18 single rooms, without en-suite facilities, are provided with washbasins. Five rooms are double rooms, two being en-suite and three not. A small lounge is provided on the ground floor with a larger lounge/dining room also available. On the first floor service users can utilise a large lounge/dining room. The home is located off the main A57 in the town of Denton. It is on the main bus route and is close to Denton town centre. It is also close to the M60 and M57 motorways. There is ample parking for those who choose to travel to the home by car. This unannounced inspection took place over two days. Time was spent talking to six residents and one visitor. Four residents were looked at in detail, looking at their experience of the home from their admission to the present day. Staff duty rotas, records of care, medicine records, staff personnel files and residents personal allowance records were examined. Since the last inspection a new manager has been appointed, who must apply to be registered with the CSCI. During this inspection one complaint was also investigated, which had not been properly addressed by the previous manager. What the service does well: What has improved since the last inspection? Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 3 and 4 Residents’ needs are not fully assessed before admission; therefore residents are not assured that the home can meet their needs. EVIDENCE: Four residents were looked at in detail. In all cases assessments had not been completed properly and failed to give enough information for the reader to be certain about what care they needed. Some files did have care assessments from social services but staff had not used all the information from them in determining the care needs of the residents. Staff were aware of residents’ physical needs but were less sure of their social needs and there was little written information about residents’ interests, family contacts and life before coming into the home. Staff were vague about the things residents enjoyed doing and were therefore less able to help them find ways in which to pass the day. Most residents however, said they felt staff knew them well, understood their needs and usually met them. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7,8,9 and 10 Care plans do not fully identify the needs of the residents. Failure to follow the correct procedures in the storage and administration of medicines puts residents at risk. Residents felt they were treated with respect. EVIDENCE: No progress has been made since the last inspection to ensure that care plans identify the actions to meet all aspects of health, personal and social care for residents. One resident who had been admitted to the home prior to the inspection had no care plans and no risk assessments. Another two residents had no risk assessments for falls, moving and handling, pressure sores or nutrition. Where risk assessments had been written they failed to identify suitable interventions to reduce the risk, although there was some evidence that staff had in practice taken steps to minimise the risk, for example by contacting the resident’s GP to review their medication. The manager was aware that the documentation was poor and had already arranged meetings with the staff to discuss the improvements that were needed. Records showed that residents had been seen by chiropodists, dieticians, GP’s, community physiotherapists and diabetic nurse specialists and residents confirmed that such services were accessed for them. However, one resident with a pressure sore had been seen by the tissue viability nurse who had advised treatment and it could not be evidenced from the records if the treatment had been delivered to the timescale specified by the specialist nurse. Therefore it was unclear if advice from other healthcare professionals was followed correctly. Staff were observed to be taking residents to their own rooms to help them with their hygiene needs and the interaction between staff and residents was courteous, helpful and kind but a number of residents had long dirty fingernails, uncombed hair or stained clothing. Oxygen cylinders were incorrectly stored in the hallways and appropriate safety signage was missing. Some improvements were noted to the medicine records but signatures for the administration of medicines were still absent so it could not be certain that medicines had been administered as prescribed by the resident’s GP. These shortfalls were identified at the last inspection and have not been addressed. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12,13,14 and 15 The home did not satisfy residents’ social and recreational needs. Contact with residents’ friends and relatives was encouraged and there was recognition of residents’ rights in respect of having control of their lives. Residents receive an appealing and nutritious diet. EVIDENCE: One visitor said he was made welcome at any time and visited for several hours each day. Several visitors have meals with their relatives. An advocate from the Citizen’s Advice Bureau was visiting a number of residents at the time of the inspection, ensuring that their views about their lifestyles were heard and made known to the home. Many residents had brought small items of furniture and their own possessions into the home. Residents said they were bored and there was little for them to do. Residents were observed sitting in the lounges for long periods of time with nothing to occupy them. Staff said they had no time to arrange and provide activities, although one member of staff does try to offer some group activities during the afternoons. Staff were observed struggling to find sufficient time to deliver basic care and were therefore unable to sit with residents or provide social stimulation. Cheese and onion pie, chips and beans was served for lunch whilst some residents had omelettes. Portion sizes were generous and all the residents were seen to eat well. Tea consisted of jacket potatoes with various fillings, homemade vegetable soup, sandwiches and homemade cake. The kitchen staff serve the meals and therefore know the residents and can monitor who is eating well and what meals are popular. In this way although there is a fairly limited choice residents are provided with food they like. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16 Residents were not certain that their complaints were listened to. EVIDENCE: Most residents said they would be quite happy to see the “boss” or the nurse in charge if they wished to make a complaint, and the complaints procedure was clearly displayed in the reception area of the home. However, failure on the part of the previous manager, to follow the home’s complaints procedure led to one complainant not receiving a response to a letter written five weeks previously, and being dissatisfied with how the complaint was handled. This complaint was subsequently referred to the CSCI and was investigated during the inspection. There was no record of the complaint having been made at the home. Service users live in a safe, well-maintained environment. Service users have access to safe and comfortable indoor and outdoor communal facilities. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19,21,24 and 26 The majority of residents’ rooms and communal areas are not clean, poorly decorated and not properly maintained. Shortfalls in hygiene procedures put residents at risk of infection. Bathing facilities did not meet all the residents’ requirements. EVIDENCE: A requirement was made at the last inspection that four residents’ rooms per month must be redecorated and this work had not been undertaken. The standard of decorating in the small number of rooms that had been redecorated was very poor and had not significantly improved the appearance of the rooms. Most rooms had no lockable storage facility, bed linen was worn and thin and towels were in short supply and of poor quality. Hallways and communal areas had chipped and dingy paintwork and wallpaper was peeling in some places. Some residents’ wheelchairs were dirty and used incontinence products had not been correctly disposed of and were left in one toilet. Toilet paper and hand towels were not provided in all toilets and bathrooms. The Parker bath remained inoperable and another bath was not suitable for residents needs. The small courtyard looked tidier than at last inspection and the manager was planning to purchase some garden furniture. Two new cleaners were working hard during the inspection and were enthusiastic about improving the environment of the home. Although the home was not clean in all areas, no malodours were detected. Service users needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27,29 and 30 Insufficient numbers of staff are provided to meet the needs of the residents. Recruitment procedures protect the residents from abuse. Staff do not receive the correct training to be able to do their job. EVIDENCE: Observation of the routine of the day and the dependency of the residents provided evidence that insufficient staff were on duty to meet their needs. Many residents were still in bed at 10.30 am and some had not had breakfast or were still having breakfast at 11am (lunch was served at 12.30pm). One relative said that residents often had to wait to be taken to the toilet. Staff were struggling to supervise one resident properly, who presented a safety risk and the time spent with this resident further reduced time spent with other residents. Staff were unable to have breaks, leading to health and safety risks to them and residents. Staff said that they found it difficult to find time to review residents’ care plans and the untidiness of the home and presentation of some residents further indicated that staffing levels were insufficient to cope with the workload. Staff personnel files contained all the information and documents required to ensure that new employees were suitable to work in the home. Staff had received training in moving and handling but no training in other areas since the last inspection. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 33,35 and 38 Residents are not consulted on how the home is run. Procedures in the home protect residents from financial abuse. Staff practices do not protect the health, safety and welfare of residents. EVIDENCE: Since the last inspection a new manager has been appointed who must apply to be registered with the CSCI. The manager was enthusiastic about implementing changes within the home and felt that the inspection would be useful in identifying areas for improvement. Many of the residents had a small amount of money in safekeeping at the home. Records are maintained of all transactions. The financial records for the four residents who were case tracked were examined and in good order. The temperature of one freezer was too high, the records showed that the temperature was consistently borderline and the freezer was extremely old and in poor condition. This freezer must be replaced. Cleaning products had been left in the shower on the first floor. Training in first aid was planned for May 2005 There had been no resident/ relative meetings and no residents could recall having received questionnaires asking their views about the service being delivered. Staff meetings had take place and staff said they were able to put forward their suggestions and ideas. Visits to the home from the registered provider, required at the last inspection, had not been undertaken. STATUTORY REQUIREMENTS This section sets out the actions which must be taken so that the registered person/s meets the Care Standards Act 2000, Care Homes Regulations 2001 and the National Minimum Standards.
