Conditions of registration: Date of last inspection 25th January 2005 Brief Description of the Service: Treeton Grange is a care home with nursing situated on the outskirts of the village of Treeton that is near both Rotherham and Sheffield. Buses pass by the home, though access to the village is limited for people with disabilities. The company no longer has its own minibus; it now accesses community transport for outings. The home is set in large gardens overlooking the countryside, and has ample car parking. The building was converted to a home from offices, and has had a large purpose built extension. It now accommodates up to fifty elderly people on two floors, accessed by shaft lifts and stairs. The rooms are mainly single rooms and all have en-suite toilets and washbasins. Five lounge areas and four dining areas are available, with one of the lounges having access on to the garden. Treeton Grange Nursing Home DS0000003092.V251325.R01.S.doc Version 5.0 Page 5 SUMMARY
This is an overview of what the inspector found during the inspection. This was an unannounced inspection that took place between 09:00 and 14:30. It involved, reading of records, discussions with staff, discussions with residents and relatives, sampling a lunch, a tour of the building, and discussions with the proprietor. The manager of the home had resigned and there were plans in place to appoint another manager by the end of October. The residents and staff were very willing to discuss issues and there was a very happy, friendly atmosphere throughout. What the service does well: What has improved since the last inspection?
The number of staff who are qualified to National Vocational Qualification level 2 has now exceeded 50 of the total care staff employed in the home. Some of the staff are now in the process of completing National Vocational Qualification level 3. There are improvements in the way money paid in by families and friends is recorded so that the staff are now consistent in the way these records are maintained. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 6 Intermediate care is not offered at Treeton Grange. EVIDENCE: The home is not registered to offer intermediate care. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 10 The service users are treat with respect and have their dignity maintained when personal care is provided. EVIDENCE: The staff were heard using the resident’s name and the residents confirmed that they had been asked what they wanted to be called. Friends and relatives were visiting residents in their bedrooms and the staff were keeping out of the way to preserve privacy. Residents said that there were no restrictions on when their visitors could call, and that they could choose who they wanted to see. The residents said that there was not any problem with the laundry loosing clothes. Extra staff are coming into work next week to spend time with the residents ensuring that all their clothes are labelled to ensure that none go missing.

Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12,15 There is a good program of activities that can be changed according to resident’s wishes on the day. Meals are attractively presented, well cooked and enjoyed by the service users. EVIDENCE: There is an activity plan on display in the reception area that indicates two different activities each day from Monday to Friday. Residents confirmed that there were activities offered both morning and afternoon and that they were asked if they wanted to join in or not. Residents said that the staff often asked them if there was anything special that they wanted including in the activity program. The residents said how much they enjoyed their meals and that they were always given o choice of what to eat. The transporting of the food to the dining room in the extension took a long time and ways of speeding this up should be investigated. There are menus available that show a good range of food is offered.

Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 16 There is a clear complaints procedure that is available to anyone who has a concern. If complaints are made these are recorded, along with details of the investigation, and used as a learning tool influencing future care in the home. EVIDENCE: The complaints procedure is kept where staff and residents/relatives can easily get to look at it. The complaints register is kept in the reception desk. A complaint has been received by the Commission for Social Care Inspection prior to the inspection that was dealt with under a separate letter. All the recommendations made from this have been implemented. Where a complaint is identified appropriate action is taken records kept and the incident can be used to highlight issues that can be learned from. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 26 The home is very clean and tidy, with large corridors that lets people in wheel chairs move around with relative ease. EVIDENCE: A tour of the building showed that all areas were kept clean and free of clutter. The staff confirmed that they had received training in the use of chemicals and that there was a good network for getting advice if it was needed. There had recently been new equipment bought to ensure that there were no reasons the home to be kept in first class order. There were no unpleasant smells noted, and the cleaning trolleys carried substances that would remove these if any were to be detected. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27,29 There are adequate staff on duty at all times of the day and night to meet the needs of the residents. There are recruitment policies that ensure proper checks are carried out before members of staff are appointed. EVIDENCE: The duty rota shows that there are more staff on duty than required by the National Minimum Standards. Staff sickness has been reduced and the staff confirmed that this had reduced the stress of having to cover shifts, and that this made the job more satisfying. Staff confirmed that they were able to go on training courses and that they were encouraged to do National Vocational Qualification training. Members of staff were seen wearing badges to prove they had successfully completed National Vocational Qualification courses. Extra members of staff are brought in to cover busy times or to carry out specific pieces of work. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 33,35 The post of manager was vacant at the time of the inspection, plans are in place to appoint someone to the post and they are scheduled to take up the role at the end of October 2005. When money is paid onto the home by families, it is recorded in a consistent way, and receipts are given to the family member. The residents and families are able to comment on the running of the home. EVIDENCE: Receipts were seen that recorded the money that had been paid in and the date of the payment. Receipts are cross referenced in a ledger that shows a balance and allows an audit trail to be carried out.

There has recently been a residents/relatives meeting that was attended by 60 people. Minutes were taken and the date of the next meeting set. When people are invited to the relatives meeting they are sent questionnaires to fill in that give different aspects of the care, such as meals, a rating from 1 to 5.
