This inspection was carried out on 2nd February 2006.

What the care home does well

Trevelyan Road offers a homely environment for clients and the activities that clients undertake are similar to if they lived in the community on their own. Clients are able to voice their opinions about what they do and where they go and are supported by staff to achieve their aims within a risk assessment framework.

What has improved since the last inspection?

The front and back gardens have been tidied up; gravelled seating areas have been provided in the back garden, to enable clients to use these areas safely. Clients are protected from harm by the recording of known allergies on their medication records. Minor repairs noted on the previous inspection have been dealt with, providing a safe environment for clients. Religious needs when appropriate are recorded and support is given for clients to maintain their faith.

All four clients are male and are supported by male members of staff. The home is a two storey domestic property with a garden to the rear. The home provides a lounge, four bedrooms, kitchen/diner and bathing and toilet facilities. Trevelyan Road is situated close to local bus routes; Tooting Broadway underground and Tooting Bridge mainline station are within five minutes walk. The inspection was undertaken unannounced by one inspector and lasted a total of two hours. JUDGEMENT – we looked at outcomes for the following standard(s): 2 Clients are supported to achieve their individual aims and aspirations. EVIDENCE: The three clients spoken with indicated that they are able to choose what they do and are supported to develop their skills. One client was positive about the life skills they had learnt and was hoping at some stage in the future to live in the community independently. Two clients plans indicated that they are involved in the reviews of care given and supported to make their views heard. The staff reported that they were currently accessing relevant college courses for clients, in practical and educational skills to enable them to develop. Service users know their assessed and changing needs and personal goals are reflected in their individual Plan. Service users make decisions about their lives with assistance as needed. Service users are consulted on, and participate in, all aspects of life in the home. Service users are supported to take risks as part of an independent lifestyle. Service users know that information about them is handled appropriately, and that their confidences are kept. The Commission considers Standards 6, 7 and 9 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 6 and 9 Clients are able to develop independent living skills in a safe environment and are supported to take appropriate risks as part of an independent lifestyle. EVIDENCE: Clients are enabled to develop independent living skills within a risk assessment framework and care plans were developed from assessments of the client. Care plans examined had individualised goals and there were risk assessments relating to activities such as cooking, venturing out in the community, using public transport and smoking. Care documentation was noted to be routinely reviewed and changes noted, this verifies that clients’ conditions are monitored and appropriate care planning strategies are put into place. Annual mental health review documentation was in place and detailed progress in the placement of clients. Risk assessments relating to clients’ were noted to be individualised and covered areas such as behaviour, arson, alcohol consumption and shopping independently. Service users have opportunities for personal development. Service users are able to take part in age, peer and culturally appropriate activities. Service users are part of the local community. Service users engage in appropriate leisure activities. Service users have appropriate personal, family and sexual relationships. Service users’ rights are respected and responsibilities recognised in their daily lives. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 11, 15 and 17 Clients are able to have appropriate relationships with family members and friends. Clients are able to choose what and where they eat. EVIDENCE: Clients are enabled to maintain contact with significant others. Two clients spoke of visiting their families and friends at regular intervals. Care is taken by the staff to make sure that a client is stable prior to visiting their relatives and there are emergency procedures documented in the individual plan and these are communicated to the family if the need arises. The home has a set menu but this is deviated from if clients chose. On the day of inspection one client went with a member of staff to get some food for lunch, as the clients did not want what food there was available within the home. One client said that they enjoy take away food at least once a week and lunch out when they go shopping. There was discussion with staff regarding religious needs of the current clients. The staff reported that clients had not expressed any particular views, but would be supported if they wished to attend religious services. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 18 Clients benefit from individualised support. EVIDENCE: Clients are supported with personal care if necessary. Staff make sure that they are appropriately dressed for the weather if going out. There was open discussion about compliance with medication and how the clients might feel if they do not adhere to medication. One member of staff explained that one client now attends their outpatient appointments for medication on their own after initially receiving support from staff. Service users have the specialist equipment they require to maximise their independence. The home is clean and hygienic. JUDGEMENT – we looked at outcomes for the following standard(s): 24, 25, and 26 Clients benefit from a homely environment and are able to choose items for their own rooms. EVIDENCE: Clients are able to personalise their rooms and are encouraged to choose their own furniture and bedding. One client kindly allowed the inspector to view their room. The client had adequate furniture for their needs. The client had their own audio system and CD’s. The walls had pictures and posters of the client’s choosing. The client was happy to discuss photographs of family and friends that they had in their room. Improvements have been made to the garden areas to make them more accessible for clients and to ensure that safety is maintained. The minor repair needed to the kitchen unit, noted at the previous inspection has been done. The lounge had comfortable seating for all clients and the dining area was large enough to accommodate all clients of they wished to take a meal together.
