This inspection was carried out on 26th September 2005.

CSCI has not published a star rating for this report, though using similar criteria we estimate that the report is Adequate. The way we rate inspection reports is consistent for all houses, though please be aware that this may be different from an official CSCI judgement.

The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

The grounds are well stocked and maintained to a very high standard. The home is tastefully decorated and maintained and cleaned to a very high standard providing a very pleasant environment for service users to live in. Service users are encouraged to retain their independence; their bedrooms are furnished with personal items. Service users are able to visit in the community, some go out with friends, some have their own cars and a choice of activities is available for those wishing to participate. Staff were observed assisting service users respectfully throughout the day. All service users have care plans and those seen were satisfactory and had been reviewed regularly. The service users spoken with said that the "staff are good" and "if they did have a complaint the manager deals with it". A visitor said that they are satisfied with the care. Service users made positive comments about the meals provided, bowls of fresh fruit were seen in all lounges and on dining tables. The arrangements for service users to receive health care are satisfactory; specialist care is arranged if needed.

What has improved since the last inspection?

Some service users` records seen showed that care plans are now being completed on a regular basis. The manager said that eleven staff have achieved NVQ qualifications and staff supervision is now being provided.

Can provide care and accommodation for three named service users with physical disabilities, category PD(E) 21st March 2005 Date of last inspection Brief Description of the Service: Walton Manor is a privately owned care home for older people situated in Walton village on the outskirts of Wakefield. The home is set back from the road in large well-maintained grounds, having gardens to the front and the rear. There is sufficient car parking space available. The home offers accommodation for 36 people in single and double bedrooms over two floors most of which have en-suite facilities. There are communal areas of different sizes that are comfortably furnished for the service users. Service users are registered with several local GP practices and district nurses support the care team at Walton Manor in the care of the people who live there. This was an unannounced inspection, which took place on 26th September 2005 over a period of 6.5 hours. The inspection process included discussions with some service users, a visitor and staff, a walk round the building and grounds and checking some of the records held at the home. Thanks are extended to the service users, staff, manager and all concerned for their hospitality and cooperation throughout the day. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 3 Pre-admission assessment arrangements are satisfactory. EVIDENCE: Service users’ files checked showed that pre admission assessments are completed and they are informed in writing that the home can meet their needs. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7,8,9,10 Care plans ensure that service users needs are met and the provision of health care is satisfactory. However service users health could be at risk due to the poor standard of medication records and administering practices. EVIDENCE: Care plans seen had been completed from assessment of needs and reviewed monthly. The records seen showed that GPs, district nurses and other health providers visit as required. Risk assessments, nutritional assessments and records of some weight checks had been kept, however the manager said that a type of weighing scales recommended at the last inspection had not been purchased. The medication and records seen showed some inaccuracies in the number of medications in stock and the records kept. The manager said that accredited medication training was being arranged. Staff were observed interacting with the service users, no incidences were seen that could have compromised service users’ privacy or dignity. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12,13,14, Service users are able to exercise choice and control over their lives and maintain links with the community. EVIDENCE: Two service users were seen strolling round the grounds during the day. Service users are served their breakfast in their bedroom. Many of the service users are independent, the manager said some choose not to join in with activities and make their own arrangements; some service users have a car and go out for a drive. A service user said that they had been out for a meal to a local restaurant with friends. The manager and deputy manager said they had taken two service users away for a weekend break. They said a craft person visits twice weekly and they are considering recruiting an activities person. The service users were not joined for lunch on this visit, those spoken with said the meals are very good, fresh fruit was seen in bowls in the lounges and on dining tables, the kitchen was clean and the cook said that he has plenty of provisions to ensure service users have good meals. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 16,18 Complaints appear to be dealt with, however recruitment procedures do not ensure service users would be fully protected from abuse. EVIDENCE: Two complaints seen in the records had been dealt with. A service user said that if they have a complaint the manager deals with it. The assistant manager said staff had been shown a video as part of their adult protection training. The staff files seen showed that two staff members had started work at the home before a CRB and POVA first check had been completed. Service users live in a safe, well-maintained environment. Service users have access to safe and comfortable indoor and outdoor communal facilities. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19, 20,23,24,26 The grounds and building provide a high standard of accommodation for service users to live in. EVIDENCE: The grounds seen were very well maintained and provide and excellent environment for service users to view from their rooms or sit or stroll in. The provider said that a new maintenance person has been recruited to work on the grounds. The communal areas seen are decorated, furnished and cleaned to a very high standard and provide a very pleasant comfortable environment for service users to sit and relax and dine in. The service users bedrooms seen were clean, very well decorated and furnished with many of their own possessions. The kitchen was seen to be clean and tidy. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27,28,29 Staffing levels are considered to be adequate, however reduced staffing levels in the evenings at weekends do not ensure that service users choice to bathe would be met. The recruitment procedure does not always ensure that service users are fully protected from abuse. EVIDENCE: An anonymous concern had been brought to the attention of the Commission before the inspection. The staff rosters seen showed that a carer was on duty from 7pm to 9pm on weekdays to assist with bathing however not at weekends. The manager said that carers only provide personal care and are not designated to others duties in the home. The manager said that there are: 3 carers and 1 senior on duty during the morning. 2 carers and 1 senior on duty in the afternoon until 7pm and then 1 extra carer works from 7pm –9pm Monday to Friday. Some service users said that there had been a “lot of staff changes lately”; the manager said that some new staff recruited had left for personal reasons and other had not been suitable. The manager said that six staff have NVQ level 3 & five have NVQ level 2 Walton Manor DS0000062365.V251877.R01.S.doc Version 5.0 Page 14 The files of the most recent staff recruited to the home were seen; application forms, references and proof of identity had been completed. Although a Criminal Records Bureau & Protection of Vulnerable Adult checks had been requested it was found that these staff had commenced employment prior to a POVA First check being received. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 33,38 The arrangements for safeguarding service users finances appear to be satisfactory. Attention to Health and Safety issues appear to be generally satisfactory, however the broken areas on the floor covering in the kitchen could lead to incidences of cross infection occurring. EVIDENCE: Discussions with some service users, staff, observations during the inspection and information found on service user’s records showed that the home is run in the best interests of service users. The records of monies held by the home on behalf of some service users were seen, they had been well maintained and no inaccuracies observed. The manager said that some service users have advocates to help them manage their finances. The fire report for the fire service inspection completed in November 2004 was seen and no recommendations were noted. The floor covering seen in the kitchen was torn in places.
