This inspection was carried out on 4th July 2005.

The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

The service users said that they liked the home and in particular their relationship with staff, the food and activities. `Good food, clean home.` `I think I`ll settle here.` `I like my room.` `I have a special person, we understand each other.` Visitors were made welcome and obviously know the manager and her team well. Since December 2004, 7 compliments were received from relatives. A range of activities are available such as keep fit, yoga, art group and bridge. Service users can have newspapers, friends over for drinks or meals, garden, cook and help in the home. Service users join in the community events such as a recent Rose Festival and Celebration of Music. Local schools visit and a BBQ is planned for 15/7/05. The standard of accommodation meets the needs of the service users and is being improved so that there are more bedrooms with ensuites, usable space and more communal space. The new kitchen, laundry and hairdressing room provide better facilities and demonstrates the commitment of Wellburn Care Homes Ltd to improve standards for the benefit of service users. The Fire Safety checks, drills and instructions to staff by the competent person are comprehensive. Service users needs are assessed by the home and care plans are in place to meet them. Relationships between service users and staff are good. Staff described the needs and wishes of the service users they care for.

What has improved since the last inspection?

One requirement was made at the last inspection regarding the new building works and has been met. Wellburn Care Homes Ltd continue to make improvements to the home.

What the care home could do better:

The medicines were checked with the manager and some areas require improving. Care plans need to record the preferences of service users such as personal care.

Ovingham has a local shop, 2 public houses and bus services operate. The home has a pleasant garden surrounding the building and many bedrooms have rural views. The unannounced inspection took place on 4/7/05 at 10am and lasted until 4pm. At the time of this inspection the Registered Manager Mrs Margaret Armstrong was on duty. Wellburn Care Home is being refurbished and building works are in the process of improving the facilities. There were 27 service users at the time of the visit and staffing levels were appropriate to meet the needs of the service users. Time was spent looking around the home to check the cleanliness, maintenance and decoration. Service users shared their views about the home. Time was also spent observing the contact between service users and staff. Three Care Plans for service users care were examined. The home’s Fire Log and maintenance systems were checked. Arrangements for the administration and management of medication were audited. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 3 Service users needs are assessed before they are admitted to the home. EVIDENCE: Three care plans were examined and detailed assessments are in place. Staff and management explained their approaches to the care of service users. Staff have good relationships with service users and their families. Comprehensive assessment procedures ensure that the home does not offer places inappropriately. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7,8,9 Care plans are in place and staff have the information to meet service users needs. However care plans do not reflect the personal preferences of service users regarding personal care. Staff demonstrated that they know the resident’s needs and wishes and have good relationships. The health needs of residents are met and multi disciplinary working is taking place. The systems for the administration of medication require improvements. EVIDENCE: Three care plans were examined and residents and staff spoke about their needs. The documentation regarding the care of residents is positive and the staff know the residents well. Personal preferences should be recorded for such tasks as personal care. Residents said that they liked the staff and felt well looked after. A risk assessment must be completed for one bedroom with consultation with the service user. An audit of the medication arrangements was undertaken in the presence of the manager and some areas require addressing. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12 & 15 Service users have access to a variety of activities. Dietary needs of residents are well catered for with a balanced and varied selection of food available that meets resident’s tastes and choices as well as special dietary needs. EVIDENCE: A social activities programme is displayed. Service users gave positive comments on being able to participate in activities and extensive social occasions. These ranged from a weekly art group, yoga, keep-fit and bridge club. Service users join in the community events such as a recent Rose Festival and Celebration of Music. Local schools visit and a BBQ is planned for 15/7/05. The cook demonstrated his knowledge regarding nutrition for older people and service users spoke positively about the food. Lunch was unhurried and service users are able to have alternatives to the menu at anytime. Wellburn Care Homes Ltd now have a Nutritionist to develop the menus of the residents. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16 The home has a satisfactory complaints system in place. EVIDENCE: No complaints have been made since the last inspection in December 2004. Service users spoke of their good relationships with staff and the manager. Any concerns are dealt with immediately. Seven compliments were received from relatives since December 2004. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19,21,22,24-26 The standard of the environment within this home is good providing service users with an attractive, safe, clean and homely place to live. Current refurbishment and building works are not having a detrimental effect on the service users. EVIDENCE: A tour of the home was completed and the home was clean, well maintained and decorated. Service users said that they like their home and feel comfortable in their bedrooms. The Manager and her team ensure that the current building works has minimal disruption to service users during this period. The location and layout of the home is being refurbished and improvements to the kitchen, laundry, hairdressing and bedrooms are already being valued. Adequate numbers of bathrooms and wc’s are provided for the service users living in the home. However more ensuites are being provided. Bedrooms are spacious, homely and personalised by the service users, relatives or staff. One carpet must be replaced as it is stained despite cleaning. One bedroom must also be risk assessed in consultation with the service user due to storage safety concerns. Service users needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. JUDGEMENT – we looked at outcomes for standard(s) 27 Staffing numbers are appropriate to the assessed needs of the residents, size and layout and purpose of the home at all times. EVIDENCE: The home maintains the level of staffing in accordance with previous agreements with the local authority and this reflects the size and layout of the building and the needs of the residents currently living in the home. The current levels of staffing are a minimum of 4 care staff on duty during the day and 2 waking night staff. The staffing levels will be reviewed once the building works are completed and the registration increases to 35 service users. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 38 The health and safety checks are good and the safety of service users and staff are protected. EVIDENCE: The Fire Log is maintained and staff receive comprehensive instruction and drills by the competent person. A Fire Risk Assessment is in place but needs to be dated. Maintenance checks are in place. These include Control of Substances Hazardous to Health (COSHH), Risk assessments, hot water temperature and Legionella checks.
