Conditions of registration: 1. The service user category (LD) (E) is specifically for one named individual. If this person leaves the service user category will revert to (OP). 27th October 2005 Date of last inspection Brief Description of the Service: Wentworth House is a home that provides residential care for up to 19 service users over the age of 65. The home is situated on a quiet residential road in the Swinton area of Salford. The home is situated close to local amenities and close to public transport routes. Bedrooms are situated on both the ground and first floor of the accommodation. A passenger lift serves both floors. There are several communal lounges and two communal dining rooms that are situated on the ground floor of the accommodation. The home continues with on-going refurbishment to provide a pleasant environment. Wentworth House DS0000008339.V278660.R01.S.doc Version 5.1 Page 5 SUMMARY
This is an overview of what the inspector found during the inspection. This inspection was unannounced and took place on 24th February 2006. During the course of the inspection time was spent talking to a number of residents, a member of staff and the managers of the home. Examination of records, care plans, medication records, polices and procedures relating to the home took place. The home continues to provide long stay accommodation. Throughout the inspection the inspector observed a pleasant atmosphere around the home with lots of conversation taking place between residents, staff and visitors. The home was very clean and tidy and was pleasantly furnished to meet the needs of the residents. Not all of the standards were inspected during this inspection. It is strongly advised that this report is read in conjunction with the previous report. What the service does well: What has improved since the last inspection?
Further improvements had been made in the content of the homes Service Users Guide and Statement of Terms and Conditions. Further development of these documents is still required to ensure that they fully demonstrate the service that is offered by the home. Since the previous inspection a member of staff had taken on the role of full time cook. This action will now ensure that the staff on duty can fully dedicate their time to care duties within the home.

Two staff had attended a health and safety course and two staff had completed Salford’s training partnerships induction programme. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 1,2,3 The service user guide does not reflect the service delivered in the home. Once in use, the pre-admission assessment should collate all the needs and wishes of perspective residents. EVIDENCE: The home’s recently revised service user guide was in draft form. The majority of the information that is needed to be contained in the document was there. The manager needs to ensure that all the information listed in schedule 1 of the Care Homes Regulations is included, for example, the staffing structure in the home, a copy of the fire evacuation procedures, what arrangements the home has for residents to attend religious services of their choice. The manager has recently revised the terms and conditions given to residents when they commence living at the home. This document explains what accommodation is offered by the home and any additional charges. However, the document does not state in full what room is to be occupied, the rights and obligations of the resident and registered provider and who is liable if there is a breech of contract, the period of notice required when a placement ceases or notice is given. The document also needs to contain specific information about

Following a requirement made in the previous report the manager of the home was in the process of devising a new pre-admission assessment for use when assessing if the home can meet an individuals needs. The inspector viewed some of the new documents and the content appeared detailed. The manager stated that the assessment format would be introduced and in use within the near future. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7,9 Resident’s needs and wishes are not fully reflected in individual care plans. The staff and management of the home ensure that residents receive the appropriate healthcare support from local care professionals. EVIDENCE: Individual care plans were available for each resident. The majority of care plans contained personal information, residents daily routines, information on the residents life prior to moving into the home, risk assessments, physical and mobility needs etc. However, not all care plans contained consistent up to date information and therefore did not demonstrate what care and support needs were being met by the staff team. Following a requirement made during the previous report the manager stated that a new format for documenting residents needs and wishes was being devised. The requirement that care plans and risk assessments are reviewed and updated on a regular basis and contain all the information required about the individuals needs and wishes is reiterated in this report. Records and documents contained on files clearly demonstrated that residents had access to health professionals when required. There was evidence of
Wentworth House DS0000008339.V278660.R01.S.doc Version 5.1 Page 11 regular visits from the peoples GP and the local district nursing team. An optician visits the home on a regular basis. The home had a policy for the storage, handling and administration of medication. Medication within the home was stored appropriately. Medication Administration Records (MAR) were supplied by the dispensing pharmacist. However, some of the MAR sheets did not contain all the information required regarding the resident’s full details or information relating to the dosage of medication. The manager of the home said that they would address this issue with the pharmacist. Some MAR sheets also contained handwritten entries onto the sheets that did not contain all the required details. Medication Administration Records demonstrated that residents were being administered their medication by staff. However, some staff had ‘ticked’ and not signed to say that they had applied a cream to a service user. Prior to taking on the role of administering medication staff receive training from the managers of the home. Staff are also required to complete a written workbook on the subject of medication. At the time of the inspection the manager of the service was discussing with the local joint agency to access training in the administration of medication. The dispensing pharmacist carries out an annual visit to the home the assess their management of medication. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12-month period. JUDGEMENT – we looked at outcomes for the following standard(s): 13,14,15 Visitors are welcomed into the home. Residents are happy with the meals that are served at the home. EVIDENCE: Residents confirmed to the inspector that they were able to receive visitors at all times. The visitor’s book demonstrated that some residents received visitors on a regular basis throughout the day and evening. Information for relatives and visitors on what services are offered by the home is contained in the homes residents guide that was in draft form at the time of the inspection. Information was available to residents, their families and their representatives relating to how to access local advocacy services for an independent person who will act in their interests. This information was clearly displayed in the hallway of the home. Meals are served in communal dining rooms on the ground floor of the accommodation. Service users spoke positively about the food that was prepared and cooked at the home. Staff demonstrated a good knowledge of residents likes and dislikes relating to food and drinks. There was a selection of fresh, frozen, tinned and frozen foods available in the home to meet resident’s needs.

The intended outcomes for Standards 16 - 18 are: 16. 17. 18. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12-month period. JUDGEMENT – we looked at outcomes for the following standard(s): 18 Information is available to support staff should they need to deal with an allegation of abuse. However, staff would also benefit from further awareness training in this subject. EVIDENCE: The home had a policy on the protection of vulnerable adults along with a copy of Salford Social Services joint agency adult abuse policy. A requirement was made in the previous inspection report that all staff receives awareness training in adult protection. The manager stated that since this requirement was made all staff had watched an awareness video on adult protection and it was planned that all staff would watch this training and information video again. It is strongly recommended in this report that all staff receive further awareness training that informs them of how to raise a concern using the Salford Social Services joint agency policy on adult abuse. Service users have access to safe and comfortable indoor and outdoor communal facilities. Service users have sufficient and suitable lavatories and washing facilities. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12-month period. JUDGEMENT – we looked at outcomes for the following standard(s): 25 Although hot water temperatures are tested on a regular basis there is some evidence that further work is required regarding the monitoring and maintenance of hot water temperatures. EVIDENCE: A requirement was made in the previous inspection report that hot water temperatures were monitored and recorded on a weekly basis. During this inspection there was evidence that hot water temperatures were being monitored on weekly basis, however, the entries to the record were not dated on some occasions. The temperature on some hot water provisions had been recorded as low as 31.1c. The inspector randomly tested several hot water provisions around the building and found that several were running too cool. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12-month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27,28,29,30 The failure of staff not having regular formal supervision and updated training may result in them being unable to maintain the quality of service they currently provide. EVIDENCE: At the time of the inspection there were two carers and a manager on duty to meet the needs of the residents. A requirement was made following the previous inspection that any member of staff who was carrying out cooking/kitchen duties was supernumerary to the staff carrying out care duties. During this inspection the manager stated that current member of staff was about to commence fulltime cooking duties in the home. The home had a recruitment procedure that demonstrated that an appropriately completed application form was completed by any candidate and that two acceptable written references and Criminal Records Bureau checks were received prior to any person commencing employment. A requirement has been made in recent reports that all staff files must be updated to include all the information listed in Schedule 2 of the Care Home Regulations. This requirement had not been met and is reiterated in this report. The home had several vacancies and were waiting for four newly recruited members of staff to commence employment in the near future. There was evidence that some but not all staff had received training to help them carry out their role. This training included watching videos and completing workbooks on emergency 1st aid, moving and handling and food hygiene in a care home. The manager stated that he was in the process of arranging further training for staff with Salford Training Partnership with regard to moving and handling. Two staff had completed Salford Training Partnerships induction training programme and two staff had completed a CIEH foundation training course in health and safety. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 35,36,38 Policies and procedures do not fully reflect the standards of the service delivered in the home. EVIDENCE: The homes policy is that it does not involve itself in managing any financial accounts for residents. The home does support people their weekly personal allowance and any money transactions are fully documented on an individualised record sheet. A requirement has been made in previous inspection reports that all staff receives formal supervision on a regular basis. During this inspection the manager confirmed that formal supervision sessions were still not taking place and therefore this requirement is reiterated in this report. The manager stated that regular staff meeting took place, however, these meeting had not been recorded. The home purchases a health and safety management system that provides advice and support on all aspects of health and safety. A requirement was

The manager stated that he was still in the process of reviewing health and safety policy within the home. There was evidence that recent portable appliance testing had taken place and that the passenger lift and bath hoist had been inspected/serviced. STATUTORY REQUIREMENTS This section sets out the actions, which must be taken so that the registered person/s meets the Care Standards Act 2000, Care Homes Regulations 2001 and the National Minimum Standards. All medication must be signed for at the time of administration and all medication administration records need to contain the appropriate details of the resident. It is required that hot water temperatures are monitored and recorded on a weekly basis and that residents and staff have constant access to hot water heated to a temperature of 44oc at all times.
