The company own another care home in a nearby town. Information about the home is provided to service users and others in the homes statement of purpose and service user guide. Fees paid are GBP375 per week and there is an additional charge for hairdressing, beauty therapy, private chiropody, toiletries and newspapers. Private accommodation is provided in 47 single rooms and two shared rooms, and there are various communal rooms available. The home is close to local amenities such as shops, banks, cafes, public houses and transport facilities. All areas of the home are accessible to service users via the

The garden has been specially designed to meet the needs of the people living at the home and is attractive and easily accessible. There is a small car park available for visitors and staff. The reason for carrying out this inspection visit was to assess the progress made against the medication requirements issued at the last full inspection of the home on 27th October 2009 and against the contents of the subsequent Statutory Requirement Notice (SRN) issued on 25th November 2009. A pharmacist inspector spent 5 hours in the home looking at the medication ordering, storage, and disposal arrangements, examining the current medication administration record charts (MARs) and speaking to the manager and senior care staff. The outcome is as follows. We found no significant administration gaps on the curent months MARs and all items checked in both medication trolleys and the controlled drugs cupboard were found to be correct. Together this indicates that people living in the home can expect to receive their prescribed medicines correctly as directed. Further improvements could be made by assessing, recording and regularly reviewing how and when people prefer to receive their medicines. This will help to reduce the number of times medicines are omitted or refused. Hand written MAR entries were clear, accurate and had been checked by a second carer. This helps to make sure that any changes to peoples medicines are correctly understood and consistently followed by all staff. Additional information on the use of medicines prescribed to be given when required is kept with the MARs. This helps to make sure that these medicines are given correctly by all staff as and when needed. Records of the use of prescribed skin care products e.g. emollients for dry skin, should be improved to show exactly how and when the products have been used. We found that the photographs and personal information used to help sure give medicines to the right person could become detached from that persons MARs. Permanently fixing this information to each persons MAR divider would help to make sure staff always identify people correctly when giving medicines. We found that all medicines were stored securely, including those kept in peoples rooms for self administration. There are good systems and records in place to support someone who wishes to look after their own medicines whilst living in the home. The medication storage areas are clean, tidy and well-organised so that staff can work efficiently and effectively. The medication fridge temperature is checked regularly but the store room temperatures are not yet monitored. All medicines must be stored securely at temperatures recommended by the manufacturer so that staff know they are safe to use when needed. Since the last inspection, the manager has put in place a range of medication checking systems including the checking of monthly prescriptions for completeness and for any changes. This good practice helps to make sure that medicines are given correctly and that no-one should ever be without a supply of their medicines. We were told that the local NHS medicines management team had reviewed the homes medicines management systems, that senior staff were carrying out medication safety training updates, and that the safety of staff work practices were being regularly assessed. Having well trained up to date staff handling and giving medicines helps to reduce the risk of any error occurring in the home. People requiring bedrails to 15/01/2010 prevent them from rolling out of bed must have thorough risk assessments completed in line with health and safety guidelines. This will ensure that all areas are covered and there is a correct match between the resident, the type of bed, the mattress and bedrail.
