Conditions of registration: 1. 2. Sliding scale of staff agreed by the CSCI to be adhered to at all times, as agreed on 2/2/04. 2 named female out of category service users, under pensionable age. The variation applies to the named service users only, should they leave the Home then the variation will cease to apply. Not accommodated in Willow View Unit. The service should employ a suitably qualified and experienced Manager who is registered with the CSCI. The age range between service users in the Willow View Unit should be no more than 25 years at any one time. Three places within Willow View Unit for service users between the ages of 18 and 64 years. Service users to include up to 36 OP N Up to 10 PD N accommodated within the unit known as Willow View. Date of last inspection Brief Description of the Service: Willow Gardens is situated in Bootle area of Liverpool. It provides care to 36 older persons and 10 younger disabled adults (known as Willow View). The home is a purpose built care home, which provides nursing care. Willow Gardens is built on 2 floors. There are two Lounges a large lounge on the ground floor and a smaller lounge on the first floor with a separate smoking area. Dining facilities are provided in two separate areas. All rooms are single rooms with 14 rooms providing en-suite facilities and 32 rooms without en-suite facilities. Willow Gardens has gardens to the rear and sides of the home that provide space for service users to sit outside in good weather. The gardens are easily accessible from the large ground floor lounge. The home is privately owned and the company has a variety of other homes within the UK. They provide care for a number of residents with a wide range of differing needs. This is an overview of what the inspector found during the inspection. This inspection was an unannounced inspection undertaken by on inspector over a day. The total duration of the inspection was 6 hours. CSCI inspects “core” standards over 2 inspections. The core standards not covered in this report were covered in the previous report undertaken on 21/04/05. The inspector undertook this inspection by reviewing on-site records such as care plans, resident’s finances, daily records, staffing rotas, staff training and staff recruitment records. Other records reviewed were previous reports and CSCI records such as notifiable incidents and provider visit reports. Discussions were held with 10 residents, 2 relatives and 6 staff. The inspection was undertaken with the support of the manager and all areas were discussed with the manager as they occurred. Full feedback was given to the manager at the end of the inspection. All requirements from the previous report were addressed and one requirement was made in this report. None of the good practice recommendations had been utilised. What the service does well: What has improved since the last inspection?
Medication records were clear and reflected that residents were receiving their medications in accordance with the GP’s prescription. Arrangements for fire prevention have continued to improve and resident’s health and safety needs are treated as important and managed appropriately. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): None of the standards in this area were fully reviewed at this inspection. EVIDENCE: Several care and individual plans were viewed, all contained assessments of residents needs. One resident spoken with discussed how staff had visited them before they moved into the home and helped them feel “welcome”. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 10 Residents are treated in a dignified manner and with respect at all times. EVIDENCE: Three residents spoken with said that staff were “always very, respectful and took the time to find out what I want”, “staff are very good, they have such a lovely manner about them” and “they help me to make sure that I’m dressed nicely and always have my hair done”. Relatives detailed that staff are “courteous”, “polite”, “calm” and “very happy to help”. Staff were able to detail how they maintain the residents privacy and the importance that they place in maintaining the residents dignity. Staff observed during the day spoke in a respectful manner and to demonstrate a kind and warm attitude towards all the residents that they dealt with. Medicines were looked at briefly and were managed in accordance with the homes policies. They were clearly recorded and a brief audit detailed that medicines were accurately given. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12, 13 Residents have contact with family, friends and the community and the staff supports this, there are a variety of activities within and external to the home suitable to the residents needs. Further development regarding daily living activities is needed for residents living in Willow View. EVIDENCE: Daily on-going activities were available such as ensuring that newspapers were available and residents were able to access their favourite television programmes. One resident said “I prefer to spend my time in my bedroom, staff make sure that I get everything I want”. An activities co-ordinator has a robust programme of activities for the residents in particular for those on living in Willow View, these have included ten pin bowling. However daily living activities in accordance with the standards for younger adults are not explored. These were not available as part of the daily routine of the residents or detailed within the individual plans for all the residents. Families detailed that they are always welcome to come to the Home. One relative said, “staff treat me as though I’m a member of their family, its lovely”. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 16 All complaints are dealt with in an appropriate manner, they are taken seriously and acted on promptly. The Home has a procedure in place to ensure a proper response to any concerns. EVIDENCE: Three residents spoken with said “I have no need to complain, but if I did I’m sure that it would be sorted”, “if I have a issues they are fixed” and “ I’ve told the staff if I’ve a problem and they’ve made sure it was all okay”. The manager keeps clear records of any concerns when raised. None have been raised since the last inspection. Staff spoken with were aware of how to deal with any complaints including allegations of abuse. Copies of the policy on reporting concerns are given to staff when they start working in the home. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19 The home is need of maintenance in a number of areas. EVIDENCE: Over the last few months Willow Gardens has operated a maintenance system that was identified by the organisations as not working appropriately. The manager raised concerns with the Head Office and the system is to be changed. The following maintenance issues were noted during the inspection. 1. 2. 3. 4. 5. Shelves in the manager’s office are unsafe. The main corridors are in need of redecorating. The kitchen floor has gaps that present a hazard to staff. Four chairs in the day room were torn and stained. There is no lighting in the main car park as this has broken and not been repaired. 6. The cooker in the kitchen is in need of servicing. 7. The woodwork in the home is in need of repainting or re-staining.

Lighting on the top corridor is dull in particular this is a hazard to residents with poor eyesight. 10. Bedrooms 3, 5, 38 and 43 are in need of redecoration. The manager anticipates that recent delays in attending to maintenance issues will now cease and that the home will then be maintained to its usual good standards. Residents spoken with were generally complimentary about the environment. Two said that their bedrooms were “lovely” and “very nice”. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 29, 30 Residents are protected by good recruitment policies. Staff have received a variety of training to enable them to do their jobs competently and plans are in place to meet the gaps in staff training. EVIDENCE: Staff detailed that they have a variety of training available and are supported to undertake training as they need or request. The Home has recently put into place senior carer assistants to undertake more of a management role. These staff receive training to a higher level and have specific training to meet their job role. Training is available and has been undertaken by staff in medication and control of substances hazardous to health. However staff had not managed either of these two areas in accordance with their training or the Homes policies and procedures. The staff records viewed show that the staff receive full and proper employment checks prior to commencing working, this includes, a Criminal Records Check, 2 references, proof of identity and evidence of their experience and training. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31, 35 The manager is registered with CSCI and makes sure that residents are protected, well cared for and that their views are the top priority. Resident’s financial interests are safeguarded. EVIDENCE: The manager is registered with CSCI. She has worked in the home for several years and has been the registered manager for the last three years. She is a registered nurse and has management qualifications. The manager makes sure that she maintains her own learning and undertakes a number of courses. Residents spoken with were very confident at her ability to manage the home, comments included “always happy to help”, “she makes sure that the home runs properly” and “very supportive to everyone”. There is a full policy regarding resident’s finances that is included in the information given to residents when they arrive in the home. Clear records are

Residents are supported to manage their own personal allowances as appropriate. Residents can request that the home looks after their money and records such as receipts are kept to make sure that the money is managed in accordance with the resident’s choices. An expansion of the good practice regarding more personal centred care planning on Willow View unit should be considered for the Home as a whole. Staff should be supported to move away from medically based care plans and base plans on the clear needs of residents. The manager should formalise the monitoring of residents needs in order to clearly monitor that there is always staff to meet the identified dependency needs of the residents. The manager should ensure that the training needs discussed in the inspection are undertaken within the training programme developed by the Home and further training needs are identified and planned for.
