Conditions of registration: Date of last inspection 25th February 2005 Brief Description of the Service: Willow House is a care home run by City of York Council and is registered to provide a service for 33 older people of either gender aged over 65 years who do not have any specialist requirements. Willow House was purpose-built approximately 40 years ago and is located in Walmgate within the city walls. The centre of York is about 20 minutes walk away. There are nearby shops and public houses. Accommodation is provided in single rooms on two floors. The upper floors are accessible via passenger lift. This is an overview of what the inspector found during the inspection. This inspection was carried out on 8 November 2005. It was unannounced and took 6 hours to complete. The majority of time was spent with the service users in the home at the time of the inspection and with the staff on duty. There were opportunities to observe staff interaction with the service users. The relative of one service user and a care manager also spoke with the inspector. A sample of service user files was examined and a check was made as to whether the home had complied with recommendations from the last inspection. Feedback was given at the end of the inspection to the Registered Manager. What the service does well: What has improved since the last inspection? What they could do better:
The home’s line manager is planning to review the way in which the home responds to adult protection issues to make sure the procedures are followed. This should ensure that any issues are investigated in the best interest of service users. The care plans are currently adequate and the manager has identified this as an area for further improvement. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 3&6 Prospective service users have information about the home prior to admission. EVIDENCE: The manager visits prospective service users in their own homes or in hospital. This was confirmed in the case notes of the service user most recently admitted to the home. Where possible they would be invited to spend some time in the home prior to admission as part of the assessment process. Some service users said they had visited the home before making a choice to move in. An initial assessment is recorded. Intermediate care is not provided. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7 - 10 Service users’ health and social care needs are met and the systems for the administration of medicines are good. EVIDENCE: A sample of service user plans was inspected. They have been completed for all service users since March 2005. The plans are adequate and the manager has identified this as an area for further improvement. On the day of inspection the care needs of one service user were being reassessed with a view to securing an alternative placement. There are two other service users requiring more appropriate placements and the manager has this in hand. All service users are registered with a GP of their choice and are able to see other health care professionals as the need arises. Service users confirmed this and evidence was also available in case files. The systems for the administration of medication were inspected and were found to be in good order. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12 – 15 Service users said that the daily life at Willow House was what they expected. EVIDENCE: On the day on inspection, service users said they were happy living in the home. Most are content to watch television in the small lounges or spend time in their own bedrooms. There are some organised activities and one member of staff is employed specifically for this purpose. Service users said that they are able to see their relatives and friends at times convenient to them. They also said that the food was good and there was evidence that choices are available at each meal. JUDGEMENT – we looked at outcomes for the following standard(s): 16 & 18 There are policies and procedures in place to protect service users from abuse but some work is required to ensure that they are followed. EVIDENCE: There have been no complaints about the quality of the service provided at Willow House. At the time of inspection, the manager was dealing with allegations requiring investigation under the Protection Of Vulnerable Adults (POVA) procedures. She spoke with her line manager following the inspection and later confirmed that appropriate action had been taken to ensure the safety of service users. The line manager has identified the need to follow POVA procedures as an area for discussion in supervision with the registered manager. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19 & 26 Service users live in a home which is well-maintained. EVIDENCE: Service users’ bedrooms are comfortably furnished and have been decorated with personal items such as photographs and ornaments. The communal rooms are maintained in good order. Bathrooms and toilets were clean. Service users said that staff respond promptly to the alarm call system. There were no health and safety issues arising during the inspection. Improvements to the premises include the installation of new windows throughout and redecoration and repair in the kitchen. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. JUDGEMENT – we looked at outcomes for the following standard(s): 27 - 30 Service Users’ needs are met by adequate numbers of staff EVIDENCE: There are adequate numbers of staff on duty and staff. Any gaps in the rota are generally covered through overtime but there are occasions when relief staff are used. Agency staff are not used at present. Staff confirmed that there are arrangements in place for supervision and team meetings. Staff records are not currently held in the home. This has been a longstanding requirement. The manager confirmed that new staff would not be employed in the home prior to confirmation from human resources that CRB (Criminal Records Bureau) clearance has been received. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31, 33, 35 & 38 Service users live in a home, which is managed in their best interests. EVIDENCE: The registered manager has achieved the Registered Managers Award and has many years experience working with older people. She is supervised on a regular basis and Regulation 26 visits are carried out on behalf of the registered provider. The staff said that the manager is both approachable and knowledgeable. A sample of monies held on behalf of service users was checked against the record and tallied. The home has a health and safety policy and maintains appropriate records including fire safety.
