Conditions of registration: 1. On admission service users must be 60 years of age and over. Date of last inspection 20th May 2005 Brief Description of the Service: Winterbrook Nursing Home is a 26 bed care home owned by a partnership of two individuals. The home is situated in a residential road in Cholsey, Wallingford, Oxfordshire, and the house was converted for use as a nursing home in 1990. The town centre is nearby, and the home is easily accessible by public transport. The private accommodation is arranged over two floors with a passenger lift and is a mixture of single and double rooms. Communal rooms include two sitting rooms and a dining room, and residents have access to the gardens. The new manager is in the process of becoming registered with CSCI, and manages the home with support from the owners who are in close touch and visit regularly. There is a nurse on duty over the 24 hours, supported by a team of carers, the activities coordinator, and housekeeping and administrative staff. This is an overview of what the inspector found during the inspection. This announced inspection took place over one day when the inspector saw all areas of the home, spoke to staff, residents and relatives, and saw records and documents relating to the care of the residents. The inspector also met with the home’s proprietors, who are involved with the management of the home. As the inspection was planned in advance with the home, the inspector had received the home’s completed pre-inspection questionnaire, giving details of all aspects of the home’s management. Some relatives and health and social care professionals completed the CSCI questionnaire about the care at the home, and their comments were also taken into account when completing the report. What the service does well: What has improved since the last inspection?
Broadband Internet services are available in the office. More administrative help has been provided. A television and video recorder have been provided to improve social opportunities. The maintenance arrangements have improved at the home. The complaints procedure has been improved. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 3. Residents’ needs are fully assessed before moving into the home. EVIDENCE: The manager usually conducts assessments in the individual’s own home or in hospital if necessary, and referral information from care managers is recorded. The inspector saw the assessments conducted for two residents covering all the health and welfare needs of the individuals. Care plans are developed from the assessments and the registered nursing input is appropriately assessed. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 9 and 10. Medication is well managed. Residents are treated with respect and can see and talk to others in private. EVIDENCE: The home has a medication policy and procedure, and only nurses manage the medication. Medication is ordered monthly and is supplied in blister packs. The inspector checked the procedures, including the controlled drugs, and was satisfied that medication is well managed. The inspector discussed privacy and respect issues with residents and relatives, and was satisfied that residents are respected and that all personal care is delivered in private. Residents can easily contact friends and relatives on the phone, and can see visitors in private. New members of staff receive training on how to treat residents with respect during induction. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12, 14 and 15. Residents value the lifestyle provided at the home, including the range of activities available. Residents are encouraged to exercise control over their lives. Good quality food is provided to residents. EVIDENCE: Personal choice is seen as important for the resident, and the timing of breakfast, for example, is decided by the resident. Residents can bring personal possessions with them if they wish. Ministers of religion regularly visit and some religious services are conducted in the home. The activities coordinator usually documents the life stories of the residents with help from relatives and friends, and residents’ interests and hobbies are integrated into the activities programme. The dedicated activities coordinator works four days a week, and provides a good mixture of individual and group activities. This service is much valued by the residents. Relatives and care managers usually manage the residents’ financial affairs. The inspector saw the home’s menu, and spoke to the cook, relatives and residents about food. Several residents said that the food has improved and that the presentation is better. The inspector was satisfied that good quality home cooked meals are provided to the residents.

The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 18. The home’s adult protection policy and procedure needs to be improved. EVIDENCE: The home’s policy on protecting vulnerable adults is not sufficiently detailed to help staff address allegations of abuse. The policy should be amended to include contact details of the Oxfordshire adult protection support worker. All staff should have basic training in how to protect vulnerable adults. JUDGEMENT – we looked at outcomes for the following standard(s): The key standards were assessed at the last inspection and were not assessed at this inspection. The intended outcomes for Standards 27 – 30 are: 27. 28. 29. 30. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27, 28, 29 and 30. It is not clear that sufficient staff are available at all times to meet residents’ needs. The numbers of NVQ trained staff will soon be appropriate. The home’s recruitment procedures need to be improved to protect residents. New staff should be provided with better induction and foundation training. EVIDENCE: Several residents told the inspector that staff are caring and kind, and one relative commented that they are ‘more than helpful’. Some relatives expressed unease about the staffing levels at night, when one nurse and one carer look after the needs of the residents from 21.00 hours to 07.30 hours. The inspector noted from checking the staffing statement agreed with the previous regulatory authority in 2000 against the staff rota provided, that the minimum staffing levels were not as agreed. The staffing rota showed one carer on duty between 06.00 hours and 07.30 hours, and also between 21.00 hours and 22.00 hours, when the staffing statement showed that the minimum levels should be 2 carers. The inspector understood from the manager that the residents went to bed early in the evening and rose late in the morning, so that an extra carer was not needed at those times. The proprietors should inform the inspector in writing why the agreed staffing statement is not being met, and should propose an amended staffing statement for discussion with CSCI.
Winterbrook Nursing Home DS0000047824.V259729.R01.S.doc Version 5.1 Page 14 Nine carers are currently undergoing the NVQ Level 2 in Care training, and six have almost completed the training. When all nine have completed the training the home will be meeting the standard of 50 trained care staff, excluding the registered nurses. The inspector checked the recruitment files of staff, and found that not all the necessary information was available about staff. All information required by the regulations, including proof of identity, a photograph and references must be available about all staff working at the home. The inspector saw the induction programme for new care staff, which is in the form of a checklist, and recommends that an appropriate induction programme which meets the Skills for Care standard be made available for the home. There is no foundation training programme at the home, and the proprietors should consider how best to provide for the training of new staff who do not undertake NVQ training. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31, 35 and 38. The manager has started the registration process with CSCI. Residents’ financial interests are safeguarded. EVIDENCE: The registered manager left the home in December 2005, and a new manager has been appointed. The new manager has previously worked at the home as the deputy manager, and has started the process of registering with CSCI. The home takes responsibility only for small amounts of petty cash in exceptional cases, as relatives or care managers usually manage residents’ financial affairs when they cannot manage themselves. The inspector checked the records kept and was satisfied that the records are appropriate and that safe storage is provided. The inspector discussed health and safety issues with the manager, and saw policies and records relating to maintaining a safe environment for residents

It is not clear from the records that all members of staff have been provided with appropriate fire training including fire lectures in the last twelve months, or that enough staff with first aid training are available on each shift. The proprietors must investigate and inform the inspector of the situation regarding appropriate fire training for all staff.
