This inspection was carried out on 20th May 2005.

The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

Several residents told the inspector that they were well looked after, and a relative praised the personal care and the high level of housekeeping at the home. Visitors are made very welcome at Winterbrook.

What has improved since the last inspection?

The leisure interests of residents are recorded.

What the care home could do better:

The home needs to involve residents and relatives in planning the daily care of individuals, to make sure that all the needs are met in the way preferred by the resident. The home`s complaints procedure should include timescales.

Date of last inspection 10 February 2005 Brief Description of the Service: Winterbrook Nursing Home is a 26 bedded care home owned by a partnership of two individuals. The home is situated in a residential road in Cholsey, Wallingford, Oxfordshire, and the house was converted for use as a nursing home in 1990. The town centre is nearby, and the home is easily accesible by public transport. The private accommodation is arranged over two floors with a passenger lift and is a mixture of single and double rooms. Communal rooms include two sitting rooms and a dining room, and residents have access to the gardens. The home is managed by the registered manager, with support from the owners who are in close touch and visit regularly. There is a nurse on duty over the 24 hours, supported by a team of carers, housekeeping and administrative staff. This unannounced inspection was carried out one afternoon in May 05 by one inspector. The registered manager was on annual leave so the scope of the inspection was limited as not all the information needed was available. The inspector saw several areas of the home including private rooms and communal areas, spoke to residents, staff and visitors and discussed issues arising with the nurse in charge. A registered nurse and four carers were caring for the 24 residents during the afternoon. What the service does well: What has improved since the last inspection? Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7 and 8 Good systems are in place to manage health and welfare. EVIDENCE: The inspector saw care plans for three individuals based on assessments. These contained instructions for staff on how to care for the three residents. Care plans are reviewed regularly. Although the inspector understood that relatives are consulted on care planning, there was no evidence of this, such as agreement signatures seen in the records. Residents can register with a local GP of their choice, and risk assessments are carried out on care issues to determine the appropriate action. Residents are referred to health specialists when necessary. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 13 Visitors are welcomed to the home. EVIDENCE: Residents told the inspector that visitors were welcome to visit the home at any reasonable time. One visitor said that she was able to spend time with her relative every day, and could have refreshments during her stay. She appreciated being able to perform some of the daily tasks for her relative, such as making a drink. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16 Complaints are easy to make and are acted on. EVIDENCE: The home’s complaints procedure is displayed in the entrance hall, but is placed above seating so that the inspector had difficulty reading it. A relative said that the home’s manager is very responsive to complaints and a resident told the inspector that it is easy to make her wishes known to staff. The procedure needs to be amended to include timescales for responding to complaints. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19 and 26 The home’s environment is clean, safe and well maintained. EVIDENCE: Standard 19 was partially assessed at this inspection, as limited information was available due to the registered manager being away. Winterbrook is located in an accessible area of Wallingford, and the grounds are well maintained. The inspector is aware that improvements are planned for the home, and noted that an area of one living room was being used for storage. One resident likes to sit in the same area, and the stored items were preventing her from fully enjoying the area. The inspector was later informed that the items have been removed to an area not used by the residents. The home’s laundry is sited away from the main building and has appropriate facilities, including hand washing and drying facilities. Some of the carpets in individual rooms had been cleaned on the day, and the home was pleasant and clean. The home uses the local health authority control of infection policies and procedures, and the registered manager needs to consider how these can be used to develop procedures more specific to the home.
