What the care home does well

The service users are cared for in a clean, warm and friendly environment. The staff receive training to help them provide good care. The management of the home work together to ensure a quality service is provided and are competent in the management of the home. The cook takes pride in the food that she prepares and all the service users spoken to state, " the food is excellent here." Staff are patient and kind when dealing with service users.

There are 24 places for older people who do not have personal care needs requiring specialist help and 5 places for older people who have dementia care needs. The home was purpose built about 40 years ago and provides accommodation in four separate flat lets. Service users have single bedrooms and share small lounge/dining areas. The upper floor is accessible via passenger lift. There is a large day-centre located on the ground floor of the home. Outside there is ample parking and gardens. The unannounced inspection was carried out on 29 November 2005 lasting 3 hours, following a half a day preparation prior to the inspection. A tour of the premises was completed this included the kitchen and laundry. A number of staff and service users were spoken to during the inspection. A number of records inspected including menus, care plans, pre admission assessments, staff rotas and personal allowance account records. Team Manager, Vicky Bushfield was in charge of the home and assisted in the inspection process. The registered manager Mrs Hazel Lynn Shepherd came on duty later that morning and also took part as required. The inspection concluded with a feedback session, involving Vicky Bushfield Team Manager and Mrs Hazel Lynn Shepherd who attended towards the end of the feedback. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): Standards 3 (Intermediate care is not provided) Sufficient information is gathered prior to admission in order that the home can make an informed decision that the needs of prospective service users can be met. EVIDENCE: The registered manager ensures that a pre admission assessment is carried out for prospective service users, before a decision is made that their needs can be met. If an emergency admission occurs, the up to date assessment is received from social services prior to the individual being admitted and the home ensures that they can meet the needs of the new service user. The information gained at the pre admission assessment formed the basis of care plans and risk assessments. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): Standard 15 Service users are provided with a good choice of food, which is well presented and caters for special dietary needs. EVIDENCE: The kitchen was clean and well organized. All the food open in the fridges was labelled and dated to ensure that it would be discarded within a short time frame if it were not consumed. There were up to date records of meat, fridge and freezer temperatures. All chemicals utilized within the kitchen were adequately stored and safety data sheets were available pertaining to COSHH requirements. Stock received is rotated upon delivery, home baking within the kitchen looked appetizing and well presented. The hot trolleys used to transport the food to each unit were clean and the temperatures of each trolley were recorded and monitored to ensure the food was transported at the correct temperature. The Cook informed me of service users who required puree diets and those who were diabetic. Menus were displayed on each unit and within the communal dining room and were current for the day of inspection. A five-week rolling menu is provided and the cook changes this as required. The service users on each unit may eat in the communal or units dining room. They may also eat in their own room. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): Standard 27 28 30 Service users needs are met by trained and competent staff. EVIDENCE: The building comprises of four units, each staffed separately. The dementia unit had a high staff ratio for three service users. The staff rota was examined and showed adequate numbers of staff were on duty at peak times of the day. Service users needs are met by a highly trained workforce, of 25 care staff all but two have the National Qualification in Care award at level two or three. There were adequate numbers of staff on duty at the time of the inspection. Agency usage is limited at the home, and if required they try to utilize the same agency staff to provide continuity of care. The home has a good bank of staff that are flexible and willing to come in at short notice. The Team Manager stated that “ the service does not suffer, we have a very valuable bank of staff who come in at the drop of a hat” All staff complete in house and external training, three staff files were examined and evidence of courses attended were seen. New staff all have a mentor appointed to them and for the first week are supervised. During this time the induction training is carried out and documented. Induction foundation training is commenced within six months .In house training takes place for moving and handling, this occurs when the new staff member commences at the home. Training in regard to dementia and challenging behaviour also occurs. This gives care staff relevant knowledge to help them deliver appropriate care to service users. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): Standards 31 33 35 The registered manager ensures that the home is run well and does not put service users at risk. EVIDENCE: The Team Manager and registered manager who were observed during the inspection were competent and experienced and managed the home well. Staff stated that the registered manager was good at her job and was approachable. Evidence of this was seen during the inspection as staff came to speak with her and issues were raised and appropriately managed by both members of staff. The registered manager has finished her Registered Managers Award and is awaiting her certificate. The home is run with the best interests of service users at the forefront of all decisions made. There are monthly Resident and relative meetings held on each of the units. The minutes are recorded and available for everyone to read. Staff meetings, which are also held monthly. There is a Quality Champions system in place where the home management carries out self assessment of the National Minimum Standards, the outcomes are documented and action where required takes place to ensure that the standards are being met. This information is then shared with all the other council homes in the area. A newsletter called Quality Matters is circulated monthly for all the staff to read on the notice board. A yearly questionnaire is also sent out to the residents and relatives, these are reviewed and any necessary actions taken to address shortfalls in service are undertaken. Three service users personal allowance accounts were checked, all were correct. Receipts for transactions and purchases were kept at the back of the individual’s statement. Service users were encouraged to have individual bank accounts for their personal allowance money.
