Conditions of registration: None Date of last inspection 29 March 2005 Brief Description of the Service: Woodlands is a three-bedroomed detached house in a quiet cul-de-sac in a small village; it is registered for three people with a learning disability, although only two residents are currently accommodated at the home. CareTech is the provider organisation and is a national company providing accommodation with personal care to people with learning disabilities. The service provides long-term, rather than respite or emergency accommodation and Woodlands has been home to the current residents for many years. This is an overview of what the inspector found during the inspection. This unannounced inspection by a regulation manager and a regulation inspector from CSCI took place in the afternoon of Wednesday, 3 August 2005. The purpose of the visit was to examine specific records kept in the home following concerns raised relating to the protection of vulnerable people. The visit took two hours. The regulation manager and the regulation inspector spent this time with the registered manager, a CareTech area manager and a member of staff. They also met both residents and another member of staff. They also looked at records kept in the home; these included care notes, financial and medication records. The regulation manager and the regulation inspector would like to thank the residents, the registered manager and her staff for their assistance during the inspection. The intended outcomes for Standards 18 - 21 are: 18. 19. 20. 21. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 20 The systems for the administration of medication are poor and potentially place residents at risk. EVIDENCE: Records of medication administered to residents viewed during this inspection were seen to have errors, the use of correction fluid and handwritten entries that were unclear by whom those entries were made. It is strongly recommended that the home follow guidance issued by the Royal Pharmaceutical Society of Great Britain. JUDGEMENT – we looked at outcomes for standard(s) 23 Decisions made by CareTech senior management not to follow guidance from Oxfordshire Multi-Agency Codes of Practice for the Protection of Vulnerable Adults from Abuse has potentially placed residents at risk. The system used in this home for the safekeeping and accounting for residents’ personal finances is good. EVIDENCE: The systems expected of all care services operating in Oxfordshire relating to the protection of vulnerable adults are clear; these have not been followed. The responsible individual of CareTech has been asked to provide full details of how decisions were made and how residents in this home are protected from potential risk of abuse. Records of how residents’ finances are managed in this home were examined during this inspection. It was found that the system used in this home was robust and all records seen were well kept. JUDGEMENT – we looked at outcomes for standard(s) 37 The systems for reporting specific incidents to CSCI as expected under Care Standards legislation are poor. EVIDENCE: From records kept in the home viewed during this inspection and from discussions with a senior CareTech manager prior to this inspection it was found that two separate incidents that must be reported to CSCI without delay under the Care Homes Regulations 2001, Regulation 37 had not been made.
