The inspector found there to be outstanding requirements from the previous inspection report but made no statutory requirements on the home.

What the care home does well

The home was found to be clean and bright with no odours at this time and service users were observed to be appropriately dressed, appearing cheerful and bright. The environment, although not totally redecorated in all areas, ensures that service users are living in clean and bright surroundings that are gradually being improved. There is a good choice of food and the menu was on display at this time, presenting choices of meals and those wishing to eat in their rooms are supported to do so. Service users spoken to at this time stated they are well cared for and are comfortable in the home.

What has improved since the last inspection?

The ongoing redecoration and refurbishment programme continues to enhance the environment for service users. The continuation of this programme will further develop the building to offer a comfortable and well maintained environment for those living in the home. The ordering and stock control of medication has improved and is regularly checked by an allocated member of staff.

What the care home could do better:

While there are currently two lounge areas available for service users, one is a very small room that offers quiet time and cannot accommodate many more than 3 people in armchairs. The proposed conservatory will provide a valuable area that will enable activities to be undertaken in space that will be freed up. Many service users use specialist chairs and additional space is needed to comfortably accommodate all service users and support chosen activities. All areas that are used for oxygen must have clear signs to identify this area for health and safety. Staff must use notice boards with due consideration for confidentiality and dignity, particularly as the current nursing station is in the main entrance hall. All care plans are to be reviewed regularly to ensure appropriate support and care is provided at all times. Records must evidence that this has been fully completed with outcomes identified.

Of the 33beds registered for nursing patients 10 are also registered for the admission of residential clients 1st November 2005 Date of last inspection Brief Description of the Service: Woodlands Care Home is a purpose built two-storey building set in gardens next to Fairfield Manor Care Centre, both of which are owned by the same company. The Home offers personal, nursing and palliative care, it is also registered to care for 3 people with a physical disability. Accommodation comprises of 28 single and one double room all with en-suite facilities. The well-kept garden and patio area are available to service users at the front of the building. The Home stands in extensive grounds, with ample parking facilities and is located in a residential area on the outskirts of Broadstairs. The fees for support from the home are set during the assessment period and are very individual to the needs of the service user, depending on the level of support required and the staffing numbers provided. The manager stated that the average fee levels at this time are between £386.00 to £698.00. This was an unannounced inspection and the process consisted of speaking with the manager, staff members, service users, visitors and also included a review of records, documents and undertaking a tour of the premises. Questionnaires were also sent to families, regular visitors and health care professionals. The responses received through questionnaires provide valuable feedback that helped with the completion of this inspection. At the time of this report being completed, questionnaires had been received from service users, three family members and two GPs. All stated they are happy with the support provided and families explained how staff are ‘like friends’. What the service does well: What has improved since the last inspection? Pre admission assessments are carried out to ensure any decision to move into the home is appropriate. Intermediate care is undertaken with appropriate support to maximise independence and facilitate a speedy return home. EVIDENCE: All new admissions are undertaken following a pre admission assessment to ensure the placement is appropriate. Files seen at this time contained pre admission assessments that are undertaken by the manager or deputy manager. All care is undertaken to enable a full recovery and to ensure independence is achieved and maintained. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7,8,9,10 Quality in this outcome area is adequate. This judgement has been made using available evidence including a visit to this service. All the appropriate information is recorded within the service user care plans, however, these are not always reviewed regularly. This is therefore not ensuring the appropriate support and safety of service users. Medication practices are satisfactory and appropriate. Service users are treated with respect and the right to privacy is upheld. EVIDENCE: Care plans are to be reviewed on a monthly basis and files must evidence this to ensure care needs are appropriate at all times. Staff have been working to review care plans, but those seen at this time were not all up to date or reviewed regularly. New formats for files is currently being implemented and this will support regular reviews but staff must take responsibility to ensure this is undertaken and evidenced on files. Woodlands Care Home DS0000065784.V301246.R01.S.doc Version 5.2 Page 11 Some staff are still not taking full responsibility for this practice and it is hoped that the current move to a new format may support regular review. Records have improved and do, in the main, identify the actual mood of each person and how the day was spent. New formats for recording support on care plans will include charts to show when a person has been checked, spoken to, turned or has received any form of care. This provides a full picture at all times and ensure no person goes unattended in when spending time in their room. Healthcare professionals had no areas of concern and stated that staff do work in partnership and ensure the privacy of service users. All other questionnaires received confirmed that staff provide appropriate care and support. Alternative therapies are being discussed to assist with those showing any signs of dementia. Full assessments will be undertaken and additional support is to be given by a member of the local psychiatric team and also from GPs. Medication records contain photographs and clear information regarding each service user to support the correct administration of medication. Medication is appropriately stored and only staff who have been trained and assessed as competent administer medication. The ordering and stock control of medication has improved and is regularly checked by an allocated member of staff. Staff were seen to be demonstrating good attitudes towards the service users and have clearly established a good rapport. Service users stated that staff are caring and always ready to help and assist when needed. Staff were speaking to service users in an appropriate way and people were approaching staff in a confident and comfortable manner. Observations, conversations with service users and questionnaires received confirm that staff do consider privacy and treat service users with respect. All areas that are used for oxygen must have clear signs to identify this area for health and safety. There is alcohol hand scrub throughout the building to control infection and provide a hygienic environment for those living in the home. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. Choice and autonomy are supported and encouraged, ensuring individuals have control over their own routines and life. All aspects of service user needs are met by the home and family, friends or advocates are encouraged to maintain contact. Service users stated they enjoy the food in the home and there is always a choice. EVIDENCE: Discussions with service users and visitors at this time, confirm that staff do support visitors and encourage continued contact. Questionnaires received also confirm this. One visitor explained how well one service user is doing since moving into the home and staff are both caring and encouraging. Any matters needing attention are brought to the staff directly and service users stated they are confident with approaching staff with any matters that need their attention. The kitchen area was found to be clean and orderly, alternatives are offered at each meal and various meals were being prepared at this time. There is a large dining room that is used by those choosing to eat in this area and those wishing to eat in their rooms are supported to do so. Service users stated they have plenty to eat and have good choices, with snacks being available if requested. A gold award has been achieved in the home this year. There is a record sheet for all service users that clearly identifies what activities have been undertaken, if outings have occurred and any books/films that have been obtained for a person. This will ensure appropriate support is provided, will support full follow up in all areas and also inform where options have been offered and refused by the service user. Service users and family members are confident their complaints will be listened to and acted on. Service users stated they feel safe and cared for in their home. EVIDENCE: Questionnaires received and discussions with service users confirm that family/visitors are kept fully informed and are welcomed into the home at all times. Service users explained that they are confident when talking about any areas of concern with members of staff or with the manager. A copy of the service user guide is available to each service user in their room and this is currently being updated following new ownership of the company. This provides support for all service users, explains routines in the home and sets out how to complain. Policies are in place to support staff and service users and these are again gradually being replaced with Southern Cross policies and procedures. Service users are protected by the recruitment practices and all money is appropriately secured. This judgement has been made using available evidence including a visit to this service. Rooms, communal areas, bedding and bathroom areas were all found to be clean and odour free at this time. While redecoration and some refurbishment has enhanced areas of the home, other areas remain in need of total refurbishment before the home reaches an acceptable standard throughout. The external grounds are also being developed with a sensory garden and pathway for those in wheelchairs or just getting mobile. One service user enjoys this walk with family and is becoming more mobile from this exercise. This area will also have additional seating areas, raised flower beds and will also have wind chimes. Consideration is being given to an appropriate smoking area and also to covering the current seating area to the front of the building. This will enable those who enjoy sitting outside to maintain their routine during the winter months. Again, this provides a much needed use of space and choice for service users. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27, 28, 29, 30 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Staff are caring and consider the dignity of service users, creating a relaxed and calm environment. Staff numbers do support the current needs of service users. Training is now monitored throughout the year and training needs are being identified and booked. EVIDENCE: Staff on shift at the time of this inspection consisted of the manager, three senior carers, one senior registered nurse plus five carers. There would usually be one domestic but due to sickness, care staff were covering the domestic work. There was also one cook, an assistant cook and a maintenance man on duty. The main working station has been opened up at the front of the home to create an open reception/working area. However, attention must be paid when sharing information and especially when using the notice boards. Confidentiality and privacy must be recognised when working and using the notice board in this open area. Methods must support the dignity of service users and appropriate methods must be developed to ensure this is met Woodlands Care Home DS0000065784.V301246.R01.S.doc Version 5.2 Page 20 Staff supervision has been formalised and is regularly undertaken. This process was evidenced when reviewing staff files. Staff are to undertake training in dementia during October with support from a specialist in the field of dementia and awareness packs are already in place. Staff files were reviewed at the last inspection and clearly evidenced orderly files with appropriate recruitment procedures and safety checks being undertaken prior to staff commencing work in the home. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31, 33, 35, 38 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The manager and staff have addressed previous areas needing attention and do ensure the best interests of service users are met. In the main, staff and practices in the home due support the safety and well being of service users. EVIDENCE: The current manager has worked in the home for some years and has also undertaken acting manager duties on previous occasions. Staff work together with full support from the manager to support service user needs. Staff have all worked hard to meet previous requirements and have been very keen to improve routines in the home. All fire extinguishers, service records, fire drills and fire log are maintained and up to date. Fire exits are appropriately signed and COSHH items were securely locked. There is an emergency call system in place and security measures in the home are appropriate to ensure the full safety of service users. Magnetic closers are to be installed on all service user doors for fire safety, bed rails are inspected regularly and are maintained and these actions are recorded. Maintenance records also evidence checks on temperatures, emergency call system, fire doors and door releases. .
