What the care home does well

Service users are well supported on a day-to-day basis by a committed and trained staff group. Health and personal care needs are identified and met and service users benefit from the opportunity to exercise choice over day-to-day activities. Service users enjoy a varied and nutritious diet.

What has improved since the last inspection?

Service users` privacy has been enhanced through the fitting of suitable locks to bedroom doors and comfort improved through the continuing upgrading of communal and bedroom accommodation. Some bedrooms have been redecorated and new carpets have been laid in communal areas.

What the care home could do better:

There needs to be a manager registered with the Commission for Social Care Inspection. Records required by regulation, such as the fire book, need to be kept up to date at all times so that the home can demonstrate it is protecting service users` interests.

The home is situated in Woodlands on the outskirts of Southampton City with easy access to the New Forest. Accommodation is provided on two floors served by a passenger lift. The home has eighteen single bedrooms and three double bedrooms with adequate numbers of bathrooms and toilets. It also benefits from having a large well-maintained garden accessible to wheelchair users. Fees range from £603 to £767 per week. Six hours were spent visiting the home, during which time the opportunity was taken to look around the home, view records and policies and to talk to the acting manager. The inspector also spoke privately with two members of staff. Most of the service users were observed making use of communal areas and their bedrooms and a number were spoken with briefly. Three service users were spoken with at greater length, two in their bedrooms in private, and one in a communal area of the building. Comment cards had been received before the visit from six service users and two relatives of service users. What the service does well: What has improved since the last inspection? Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 3 and 6. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The home has assessed the needs of its current service users well. These needs are clearly recorded and known to staff. EVIDENCE: Three service users’ files, one relating to a more recently admitted person, were inspected and needs assessments seen within these files contained a good level of detail. There were, for example, individual assessments relating to areas such as mental health, daily activity and prevention of pressure areas. The inspector also saw evidence that assessments were reviewed. For service users admitted through Care Management arrangements, a summary of the Care Management assessment was available. Care plans had been developed from the needs assessments and were available to guide staff in meeting needs. One service user said, for example, that she preferred spending time in her room rather than the communal areas of the home, and staff enabled her to do this. The home does not admit service users for intermediate care. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7, 8, 9 and 10. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The home has an adequate care planning regime which addresses identified personal and social care needs. Health care needs are well recorded although the home still needs to dispose of unnecessary stock medication. EVIDENCE: Care plans contained a good level of detail, and included assessments of daily living, mental state and cognition. They also included risk assessments relating to falls. Care plans are regularly reviewed and service users involved in this process. Clear information was available regarding the use of specialized equipment such as peg feeds, and a good level of information on meeting health care needs was also evident. The manager confirmed that service users have a choice of doctor from within three local practices, and that other health care professionals such as the dentist and optician are accessed as necessary. The inspector was informed that no service users were able to manage their own medication. There is a policy and procedure for the dispensing of medication. The inspector examined two medication records which were in Woodlands Ridge Nursing Home DS0000066797.V291037.R01.S.doc Version 5.1 Page 11 order and up-to-date and drugs, including controlled drugs were securely stored. It was pointed out to the acting manager that there was still some medication, prescibed to service users no longer at the home, that needed to be disposed of. One service user said that the staff were nice, and helpful, and staff were observed to be providing discrete assistance to service user when necessary. Service users are addressed by the name of their choosing Staff were observed to knock on bedroom doors and await a response before entering and spoke to service users in a respectful way. Screening was available in double bedrooms to promote privacy. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12, 13, 14 and 15. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Service users enjoy varied lifestyles and undertake activities of their choice. Visitors to the home are encouraged. Service users enjoy attractively presented meals in congenial surroundings. EVIDENCE: Each service user has a daily activities record and the inspector was informed that residents enjoy indoor activities such as bingo and playing cards. Church services were also arranged ‘in-house’. The occasional trip out or outing was arranged for service users who wanted this although two service users said to the inspector that they preferred to spend time quietly in their rooms. One service user did however confirm to the inspector that he had recently enjoyed attended the ‘residents’ meeting’. The local parish council was said to have provided information on the local area and some service users were able to visit a nearby pub. Shopping trips over the Christmas period had been undertaken and one service user was able to attend the local Womens Institute. Woodlands Ridge Nursing Home DS0000066797.V291037.R01.S.doc Version 5.1 Page 13 One service user said that she was ‘looking forward to the meal’ and another that the ‘food was good’. A choice of meals is available which are served in a large and attractive communal dining room, and staff help those service users who need this in eating. The inspector noted that there was a choice of two meals for lunch, which could be taken in the dining room or privately if this was preferred. Hot and cold options were available for tea. Swallowing assessments had been undertaken where services users had difficulty in swallowing and foods prepared accordingly The inspector was not able on this occasion to speak with any visitors to the home, who are encouraged to visit as they wish. JUDGEMENT – we looked at outcomes for the following standard(s): 16 and 18. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The home has a suitable complaints procedure, which service users are aware of and feel able to use. Service users are protected through an adult protection policy and procedure known to and understood by staff. EVIDENCE: The home has a complaints policy and procedure, a copy of which was on display in a public area of the home, and a complaints log. Both service users spoken with privately said that they ‘no complaints’, and one explained that he had received a copy of the complaints procedure with a contract when entering the home. Service users all appeared to have a good degree of confidence that any issues raised would be dealt with, and four service users returning comment cards confirmed that they knew who to speak to if they were unhappy with their care. The home also has a policy and procedure relating to adult protection, with information produced by Hampshire Social Services being available for staff to consult. Staff have been trained in this and when interviewed confirmed their understanding of what to do in the case of suspected abuse. Both members of staff interviewed said that they would report anything they needed to. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19 and 26. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The home provides a safe and comfortable environment, which is suitably furnished and adequately maintained. Some bedrooms are in need of refurbishment to enhance service users’ enjoyment of these. EVIDENCE: The tour of the building showed this to be clean and tidy throughout and there were no undue odours. Communal areas were acceptably furnished and adequate bathroom and toilet facilities were available, although one bathroom had some items of equipment stored in it. Several bedrooms, which were all adequate in size, were inspected and had clearly been personalised, to considerable degrees. The inspector was informed of plans to provide better screening in double rooms. Woodlands Ridge Nursing Home DS0000066797.V291037.R01.S.doc Version 5.1 Page 17 Service users were observed to be freely making use of communal areas, such as the lounge and dining room and several had chosen to spend time in their bedrooms as they wished. Since the previous inspection the home has provided ramping in one area and also purchased equipment to enable a previous problem regarding access to a part of the building to be addressed. Suitable locks have also been provided to service users’ bedroom doors, enhancing privacy. Various maintenance certificates were seen and were in order and up-to-date, the person in charge confirming that she received good support in dealing with maintenance of the building. Since purchasing the home some eight months previously the provider has made several improvements to the décor, in particular the dining room and several bedrooms. The inspector noted that the home’s programme of refurbishment was continuing with some new carpeting being laid on the day of the visit. The home’s laundry was inspected and was fit for purpose, with the washing machine capable of reaching the required temperature for disinfection purposes and being automatically fed with detergent. The home has a Health and Safety policy and the members of staff spoken with were clearly aware of Health and Safety issues. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27, 28, 29 and 30. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Staff are appropriately trained and aware of their responsibilities, and are employed in sufficient numbers to meet service users’ needs. Service users are protected through the soundness of the home’s recruitment practices. EVIDENCE: The inspector examined three staff files, one relating to a more recently appointed person. These contained evidence of written references being obtained following the completion of an appropriate application form and interview. There was evidence of Criminal Record Bureau (CRB) disclosures having been obtained. Staff have job descriptions and there was evidence in files of induction training having taken place. Likewise files contained evidence of the various short courses undertaken which included Dementia Awareness, First Aid, Food and Hygiene, Health and Safety, Pain Control and Moving and Handling. The home has a yearly training plan and staff interviewed said that the new owners of the home provided good training opportunities. Both longer standing members of staff interviewed were undertaking National Vocational Qualifications at Level 3. Woodlands Ridge Nursing Home DS0000066797.V291037.R01.S.doc Version 5.1 Page 19 The person in charge said that it was intended that a more formal staff supervision and appraisal system be introduced. During the inspection the inspector observed staff members interacting with service users in a friendly yet professional manner. It was explained to the inspector, and confirmed by the rota, that there were usually five members of care staff on duty in the mornings and three in the afternoons who were backed up by ancillary staff. There are two members of staff on waking duty at night. Staff were observed to be attending to service users’ needs in a calm and unhurried manner, although said that they were busy at times. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31, 33, 35 and 38. Quality in this outcome area is adequate. This judgement has been made using available evidence including a visit to this service. The lack of a registered manager and omissions in record-keeping may compromise residents’ health and safety. EVIDENCE: The inspector was informed by the person in charge that it was intended that she apply to become the registered manager and that an application form had been obtained. It is understood that she has had a considerable number of years experience. A Residents Meeting, attended by some 13 to 14 service users had been held recently and it was intended that in future these would be held every two to Woodlands Ridge Nursing Home DS0000066797.V291037.R01.S.doc Version 5.1 Page 21 three months and also involve relatives. The inspector was informed that a more formal Quality Assurance mechanism was to be developed, although an audit of service users regarding the food provided had been undertaken on 2nd March 2006. Service users’ financial interests are safeguarded and there was clear information regarding the level of fees and any necessary third party ‘top-up’. There is a Health and Safety policy in place and information available to staff who were clearly aware of health and safety issues. The home’s laundry was inspected and it was noted that the washing machine was capable of reaching an appropriate temperature to disinfect items. A sample of policies, procedures and records required by regulation were inspected. Whilst most were in order and up to date the fire book was not and the person in charge also needed to attend to the accident book and to remove details of individual accidents to place in personal files to ensure that confidentiality was maintained.
