Conditions of registration: 1 The age range of Service Users will be over 65 years. Date of last inspection 24 January 2005 Brief Description of the Service: Woodlands Care Home is a converted Edwardian Manor House providing accomodation and nursing care. The home provides a reasonable standard of accomodation for up to 26 older people. This comprises of 18 single rooms, 8 of which have en-suite facilities. The home has 4 double double rooms, one of which has en-suite facility. Residents have a choice of communal areas with a large main sitting room, a conservatory area and a dining room. The upstairs accomodation is accessed by apassenger lift. The top floor accomodates administration staff, the staff room and the homes laundry. The home also has a basement which houses the central heating boiler and futher laundry equipment. This is an overview of what the inspector found during the inspection. This was the first announced inspection for the year 2005/2006 and was undertaken by one inspector and took five hours. The inspector will refer to the service users as residents throughout the report as the manager stated that they preferred this. The inspector spoke with trained staff, carers, residents, relatives, the administrator and the laundry assistant. The inspector found a relaxed unhurried atmosphere with both staff and residents willing to speak openly. The inspector would like to thank the manager and staff for their cooperation during the inspection. What the service does well: What has improved since the last inspection?
The home has now employed an activity organiser who not only organises group activities but also provides one to one attention. Staffing numbers have improved and maintained with the manager always supernummery allowing him flexibility to organise all the other administrative duties. The manager has been able to reduce agency staff in the home as he has recruited his own work force that is becoming more reliable. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 3 & 5 Al residents have a pre-admission assessment and relatives/representatives are encouraged to visit prior to admission. EVIDENCE: The manager stated that all prospective residents have a pre-admission assessment prior to admission. The manager or his senior staff nurse does these assessments. The Surrey assessment form is used which is, the manager stated, comprehensive. This is then kept in the residents file after admission. The manager stated that families or representatives are encouraged to look around the home prior to admission and so they can see the room that was empty. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7,8,9 & 11 The health needs of the residents are well met with evidence of good multi disciplinary working taking place on a regular basis. EVIDENCE: The home has developed a good system of paper work for their nursing notes. Risk assessments are carried out and all are reviewed on a monthly basis. Although the care plans themselves are not signed by the resident or service user the manager stated that they are all discussed and signed for on a separate sheet with a list of the topics covered in discussion. This form is then signed. The G.P. holds a surgery every other week but will also call in between if needed. Residents may keep their own doctor if they wish and infact her family G.P visits one resident on respite care. The home also has a NHS chiropodist that calls every three months and on demand. The manager also stated that this chiropodist would also do training sessions about care of feet and how to cut nails. The community opticians will visit twice a year and there is also a community dentist who will visit when requested. Woodlands Nursing Home (Caterham) v218795 h09 h58 s13371 woodlands (caterham) v218795 020605 stage 4.doc Version 1.30 Page 10 The inspector did not go into detail with the medication but on looking at the MAR sheet it was noted that if a resident refuses a medication for any reason it was not noted on the back of the MAR sheet and this was a recommendation. The inspector spoke with the manager and trained nurses about the wishes of the services users at death and they all told her that this was a subject that had not been addressed formally. The inspector suggested that discussions should take place around death and dying and also their wishes if the resident has a medical emergency. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12,13 & 15 The home now has planned activities and has recruited both an activities organiser and chef since the last inspection. EVIDENCE: The home has recruited an activity organiser since the last inspection and she stated that they now have a planned weekly events calendar. The organiser said that she is adaptable and can change the activity as to the resident’s wishes. There is one to one activity for the residents that do not like to leave their room and also group activity. The activity organiser was helping a resident to make a cup of tea and to do some toast when the inspector arrived. The resident stated that she was planning to go home and wished to maintain her independence. There is a church service every two weeks and the hairdresser also visits on a two-week rota. The manager stated that he had tea parties that the relatives and residents are invited to and these usually took place twice a year. This allows time for general discussions and the chance for relatives to meet one another. The manager stated these were normally well attended. One resident was happy to speak to the inspector about life in the home and sat in his armchair playing an electric piano while the resident sitting next to him sang along. He told the inspector he was happy here and enjoyed his music and playing cards and the staff was seen to sit with him and play.

Another granddaughter was visiting and bringing some lunch for her grandmother as a special treat as she is Chinese and enjoys their cooking occasionally. The chef has been with the home since December 2004 and showed the inspector the four-week rota. Every Sunday the residents are treated to a full English breakfast if they would like and the chef stated a few took up this offer. Service users’ legal rights are protected. Service users are protected from abuse. JUDGEMENT – we looked at outcomes for standard(s) 16 & 18 The home has a satisfactory complaints system with some evidence that residents feel that their views are listened to and acted upon. EVIDENCE: The home has a complaints policy, which is displayed on the wall in reception and is contained in the service user guide, which is in every bedroom. The manager stated that he has commenced a complaints book which al staff are encouraged to fill in if they have a complaint made to them. This allows the manager to quickly see what the complaint is and what has been done about it. There have been no recent complaints. The manager showed the inspector the recent Surrey multi agency abuse folder. He stated that staff had recently attended a course and more staff was about to attend another session. This is also covered in NVQ training, which most staff had now completed. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19, 24 & 26 The standard of the décor in some of the communal areas and bedrooms are poor with little evidence of improvement through maintenance or future planning. EVIDENCE: The inspector visited a number of the resident’s bedrooms and saw that quite a few needed decorating and in some cases new furniture to make the rooms more homely. In one room a windowpane was missing and waiting to be replaced the hole was covered with cardboard. Wallpaper was peeling off the walls and in some of the corridors light bulbs needed replacing to improve the brightness on the first floor. The manager stated that some decorating had taken place but with only one maintenance man and all the other routine checks that he needed to do time was limited. One assisted bathroom had a heated towel rail which was very hot it was difficult to cover as the purpose of it was to hang towels on to it therefore the maintenance man was going to remove it and this could cause a burn to a resident or member of staff.

Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27,29 & 30 Staffing numbers have been maintained at a reasonable level resulting in less agency use. EVIDENCE: The manager told the inspector that staffing had improved and he required less agency staff ad that this improved the quality of care delivered to the residents. The manager is always supernummery and the inspector agreed this was a good policy to adhere to. Apart from the manager there are always two trained nurses on in the morning and the inspector agreed that this was necessary due to the size of the home and the layout. The trained staffs are supported by carers on each shift and most have level 2 NVQ. The manager had introduced a policy where all staff now works a variety of day and night shifts on a rota basis. The manager stated this had improved working relationships between the staff, as each understood the different duties required for each shift. There was now no blame culture between day and night staff. The staff files are very untidy and the inspector told the manager these needed to be tided up so that documents could be found easily. The manager and administrator explained this was a job they had planned and that they would now start to organise this. One staff members CRB had not yet been returned and are therefore always working under supervision. Staff had undergone a variety of training relevant to their roles and the carers told the inspector that this was enjoyable. Most staff the inspector spoke with

Another two carers were about to leave to commence an adaptation course they were both trained nurses in India. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 31 & 38 The manager is fit to be in charge and is supported by his staff. EVIDENCE: The manager is a RGN and RMN with a management qualification. He has worked hard to improve the resident’s life by completing all requirements from the last inspection and building staff moral. He holds a coffee morning weekly for the staff to attend which is a general discussion group where, the manager stated. Problems can also be aired. The maintenance man does the water temperature checks and all taps have thermostatic valves. Except for one hot surface all radiators are covered. The fire alarms are tested weekly and fire training happens twice a year and the manager showed the inspector all the records. The fire officer has recently visited but the report was not yet available. The manager also holds fire drills monthly and he has devised a fire quiz for the staff to complete.
