This inspection was carried out on 23rd January 2006.

CSCI has not published a star rating for this report, though using similar criteria we estimate that the report is Adequate. The way we rate inspection reports is consistent for all houses, though please be aware that this may be different from an official CSCI judgement.

The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

The Inspector saw most of the residents in the lounge and they were listening to music. The activities co-ordinator was doing manicures. One service user told the inspector he had only been there for a short while but he said he had no complaints and that the food was good. Another told the Inspector that they look after us wonderfully and they do splendid work.

What has improved since the last inspection?

All requirements from the previous inspection have now been met. The home has employed a new administrator who has worked very hard tidying the paper work and putting the staff files in order. The home has just had an education audit and can now do adaptations.

What the care home could do better:

Following the last inspection the Inspector noted that some areas of the home looked very run down and needed refurbishment and the empty rooms needed decorating. It was also mentioned, following the last inspection in the body of the report, that work needed to take place in the bathrooms but this advice was not taken therefore there will be requirements at the end of the report for a re-decorating and refurbishment programme to take place this year. Most of the bedrooms need decorating, the bathrooms need refurbishing and the lighting needs improving. The conservatory is in a poor state of repair and it requires new furniture and also not to be used as a storage area. The gardens need attention as the paths at the side of the conservatory has moss and the flowerbeds need planting to make the area more appealing. The hard standing at the side of the conservatory needs clearing of all the rubbish.

The age range of Service Users will be over 65 years. Date of last inspection 2nd June 2005 Brief Description of the Service: Woodlands Care Home is a converted Edwardian Manor House providing accomodation and nursing care. The home provides a reasonable standard of accomodation for up to 26 older people. This comprises of 18 single rooms, 8 of which have en-suite facilities. The home has 4 double double rooms, one of which has en-suite facility. Residents have a choice of communal areas with a large main sitting room, a conservatory area and a dining room. The upstairs accomodation is accessed by apassenger lift. The top floor accomodates administration staff, the staff room and the homes laundry. The home also has a basement which houses the central heating boiler and futher laundry equipment. This unannounced inspection took place over three hours and was the second inspection to be undertaken in the Commission for Social Care Inspection year April 2005 to March 2006. Lesley Garrett, Lead Inspector for the service carried out the inspection and Titus Musee, Registered Manager represented the establishment. The deputy joined the Inspector and Manager for feedback. A full tour of the premises took place. The inspector saw most of the residents and spoke with some of them. The inspector would like to thank the residents, staff and manager at Woodlands Nursing Home for their time, assistance and hospitality during this inspection. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 10 The staff respects the privacy and dignity of their residents. EVIDENCE: The inspector saw the homes privacy and dignity policy, which also incorporates independence. This subject is also covered in induction. Shared rooms all have screens, which are fixed, to the ceiling and the inspector saw this on the tour of the building. It was also noted that staff were knocking prior to entering a room. The G.P. sees all residents in the privacy of their bedroom when a home visit is made. The personnel plans of all the residents contain the name they prefer to be known as. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 14 Residents are helped to exercise choice over their lives. EVIDENCE: The home has recently employed a new chef and all residents have a choice of menu. Most eat in the dining room but some residents have chosen to eat in their room. Sometimes, the manager stated a resident would choose to stay in the lounge to watch a particular television programme. When the residents have visitors the manager stated that they sometimes choose to sit in the conservatory but this is in need of refurbishment and will be a requirement at the end of the report. No resident manages his or her own financial affairs. All rooms have been personalised and the inspector saw this during the tour of the building. The manager did tell the inspector that there had been no complaints since the last inspection. Service users live in a safe, well-maintained environment. Service users have access to safe and comfortable indoor and outdoor communal facilities. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19 The standard of the décor within this home is poor with little evidence of improvement through maintenance or future planning. The home does not, therefore, present as a homely and comfortable environment for the residents. EVIDENCE: The lighting throughout the home is dull. The light is not adequate for the elderly frail and it will be a requirement at the end of the report to improve the lights in the corridors, communal areas and bedrooms. The conservatory needs totally refurbishing. Some of the window frames are rotting and the Perspex on the door is covering a broken window and therefore needs replacing. This room is also used as a storeroom for the wheelchairs not making this area an inviting place to receive visitors. The furniture in the home appears old. The garden to the side of the conservatory needs attention as there is moss growing over the paths; which could be slippery to walk on. The hard standing for the demolished shed is untidy and old wheelchairs and commodes are kept there. These must be removed. Woodlands Nursing Home (Caterham) DS0000013371.V276117.R01.S.doc Version 5.1 Page 13 The bathroom near the conservatory cannot be used, reducing the bathrooms that are available in the home. The remaining bathrooms are in need of refurbishment. Room 5 on the ground floor smells damp and this needs to be addressed. Most of the furniture is dated and looks worn and therefore a programme of replacement needs to be implemented. The home has purchased some electric beds and a programme of replacing the beds must continue. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 28 Staff are trained to do their jobs and National Vocation Qualification (NVQ) training is in progress. EVIDENCE: The manager stated that 50 of the carers now have level 2 NVQ and that a further four carers are doing the qualification now. All new members of staff are encouraged to do their NVQ following their induction and probationary period. The training takes place in-house with a tutor/assessor coming to visit in the home during the working hours. The home has not had to use agency staff for two years; which the manager felt was positive as there are adequate numbers of permanent staff available and willing to cover the shifts. The manager has accessed training from Surrey Care Home Association. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. JUDGEMENT – we looked at outcomes for the following standard(s): 35 The resident’s personal finances are looked after by the home adequately. EVIDENCE: The manager looks after the residents personal finances if they wish, which is kept in the office safe. The money is kept in individual moneybags for each resident. Each transaction is recorded on individual record sheets for each resident. The manager keeps all receipts and invoices so they can be given to the relatives or residents at the end of the month.
