Care Homes for Older People Page 2 of 10 Date of last inspection Brief description of the care home Yohden Care Complex is a seventy seven bedded home situated in a quiet residential area of Blackhall, which is a village close to Peterlee. The home provides residential and nursing care. There is a separate unit for people with dementia who have been assessed as needing residential care. The home is split over two floors with lift access. Some of the bedrooms have en suite toilets with sufficient additional facilities located throughout the home. Communal bathing facilities are located throughout the home. These have specialised bathing equipment to support people who are less able. The home has a large central dining and lounge area as well as small lounge and dining areas within each unit. Fees range from £436:60p and £549:50 weekly, which does not include the free nursing care element. Additional charges are made for hairdressing, chiropody and personal newspapers. The reason for this inspection was to assess compliance with medicine requirements made in a Statutory Requirement notice dated 6 July 2010 and with a compliance date of 23 July 1010. During the inspection we spent seven hours in the home, and the inspection involved looking at medication storage, stock control, administration and procedures relating to record keeping and medicine management in the home. We looked at the Medication Administration Record (MAR) and talked to the manager and staff about the systems in place. Records are kept of fridge and room temperature however medication in some units is stored at temperatures higher than those recommended by the manufacturer. For one person medication with a limited use once open was used past the expiry date recommended by the manufacturer. The record keeping had improved since the last inspection, however there were still some gaps in the MAR records. There were also some medicines which were prescribed with a variable dose where the actual dose at each administration was not recorded. There was inconsistency in the recording of the quantity of medication supplied and date received. The quantity of medication from one monthly cycle to another is not recorded on the new MAR. This makes it difficult to confirm that medication is given as prescribed and when checking stock levels. An audit of current stock and records showed that some medication had been signed for but not given. Records for inhalers and liquids showed that staff would benefit from addition training to cover this type of administration. The code N was used for non administration on a number of MARs. This code is only appropriate when medication is prescribed when required. For many of the entries this was not an appropriate code. It is important that a clear reason is given so there is accurate information on how a person is taking their medication. The prescriber, who may wish to review the medication, may also use this information. Medication for two people was recorded as not in stock and therefore had not been administered as prescribed, however for two medicines they were available in the home but not stored on the medication trolley. For one person who was on a medication which required regular monitoring good records were kept. Two people who were self medicating inhalers and creams had good risk assessments in place. From this visit it is clear that action has been taken to meet the requirements in the Statutory Requirement Notice but there is still some requirements that have not been fully met. Some requirements issued at the last inspection remain unmet. What the care home does well:
