11 Kilford Court is purpose built as a residential care home for people who have learning disabilities and associated physical disabilities. There are four bedrooms on the ground floor, which are fully wheelchair accessible, and two bedrooms upstairs for62009 service users who are more physically able. The home is situated on a small housing estate in Botley, approximately a quarter of a mile from local shops and pubs and access to public transport services. The home has a minibus that is equipped to provide transport for physically disabled service users. The home is managed by Choice Support. Service users are responsible for paying for rent, food, mileage for the home`s car when they use it, their own toiletries and items of a personal or luxury nature.

What the care home does well Care plans give staff guidance on how to deliver care and are in a pictorial format for easier understanding by the residents. Staff have access to a range of training courses. What the care home could do better: Greater attention is needed to ensure that records are available for inspection. Service users are responsible for paying for rent, food, mileage for the homes car when they use it, their own toiletries and items of a personal or luxury nature. Care Homes for Adults (18-65 years) Page 3 of 9 What we found: The inspection consisted of a 3 hour and 20 minute site visit to the home, during which 2 staff and the manager were spoken to. Staff were observed interacting and working with residents. Surveys were sent to residents and staff asking for their views on the service. These were returned by 7 staff and 6 residents. Residents were given help to complete the survey. Records, documents and policies and procedures were looked at. Care services are required to submit an Annual Quality Assurance Assessment (AQAA) to the Commission. This was returned to the Commission and contained the information we asked for. Information contained in the AQAA has been used for this report. Choice of Home The previous report required that each resident has a contract or terms and conditions of residence. At this visit copies of residents licence agreement to stay at the home were seen. Individual Needs and Choices Care plans were looked at for 2 residents. These include pictorial diagrams for easier understanding by the residents. Personal details and current medication are recorded. Details of specific care procedures are recorded such personal care, getting up and going to bed and the use of any specialist equipment along with diagrams for staff to follow in using lifting equipment. Communication needs are assessed and guidelines recorded for staff to use. Health needs are comprehensively recorded in pictorial format. Each persons care records has risk assessments. These refer to specific activities and needs where an element of risk to the individual is recognised. Guidance is recorded for staff to follow so that risks are minimised when supporting people to go out and take part in events. Care records include an assessment of individual residents cultural needs. Lifestyle Discussions with staff and care records show that residents have allocated time for social and leisure pursuits including meals at restaurants, visits to museums, pub lunches, attendance at church services, shopping excursions, and swimming sessions. Staff were observed interacting with residents providing stimulation by the use of games and a foot spa. A resident was seen using the homes sensory area. Residents confirmed that they are able to spend their time as they wish and are able to Care Homes for Adults (18-65 years) Page 4 of 9 make their own decisions. Personal and Healthcare Support Residents say that they are supported by staff. Staff report that residdents health and personal care needs are met. The homes medication procedures were looked at. The home uses a monitored dosage system of medication where medication is held in cassettes for each prescribed time medication is due. The cassettes and medication administration records show that residents receive medication as prescribed. There are written policies and procedures for the handling and administration of medication. Staff receive training in handling medication from the supplying pharmacist. Records show that 14 care staff have received this training. Medication was found to be securely stored. Concerns, Complaints and Protection Individual residents are charged a mileage rate when they are driven to social events. A record of each journey is maintained with details of the amount to be charged. There is also a record for any transactions of the mileage charge made to the individulal plus a record of any monies held on behalf of the person. Residents surveys state that staff and managers listen and act on what they say. Staff say that they know what to do if they have any concerns. Environment Two residents bedrooms, the communal areas as well as the toilet and bathroom facilities were seen. Bedrooms contain specialist lifting equipment such as track hoists. Rooms are personalised with residents own belongings and pets. There is a lounge with an adjoining sensory area. The lounge carpet was stained and it was confirmed that this is to be shortly addressed. There is a kitchen/dining area. Residents are able to access the garden via level access patio doors. The garden is well equipped and is accessible to wheelchair users. Specialist equipment is provided in bathrooms for bathing those with mobility needs. Staffing At the time of the visit there were 3 staff on duty in the home plus 2 staff escorting 1 person to a swimming session. The staff rota showed staff on duty for the day as follows: 2 staff 7am to 2.30pm, 1 staff 7am to 11am, 1 staff 9am to 5pm, 1 staff 12 noon to 8pm and 1 staff 2pm to 9.30pm. Night time staffing consists of 1 waking and 1 sleep in staff members. Staff said that the staffing levels are sufficient to meet the needs of the Care Homes for Adults (18-65 years) Page 5 of 9 residents. We looked at staff training. Staff reported that they have access to a wide range of relevant courses and that training is provided as residents needs indicate. Recent training in dementia care was recently provided for instance. Some training is mandatory such as moving and handling and first aid. Staff are also able to choose additional courses from a training prospectus. A copy of the prospectus was seen. The AQAA states that 11 of the 13 permanent staff hold National Vocational Qualification level 2 or above in care. Staff receive regular supervision, which was evidenced from training records and from staff themselves. Staff recruitment procedures were looked at for 4 staff. The home maintains a record to show that the required checks are carried out before the person starts work. These include a criminal record bureau (CRB) check and 2 written references. For 1 person these records were only available when the manager contacted the organisations head office who e- mailed the details to the manager during the visit. Conduct and management of the home Since the last inspection there have been several changes to the homes management. The current manager has been in post for less than a month before the visit and has not yet registered with the Commission. Where a care home is owned by a company, a representative of the company must carry a monthly visit and report on the conduct of the service as required by Regulation 26. Copies of these were not available at the home aside from the month of November 2009. Further copies were made available following the inspection for each month from November 2009 with the exception of April 2010. Only 2 reports were said to be available for the period from July to October 2009. This was a requirement at the last inspection.
