What the care home does well The service has complied with the requirements made at the key inspection of May 2009. It is clearer now how peoples health needs are identified, reviewed and monitored . Plans for staff training are more explicit and the service can now demonstrate what training they are providing and when. The manager and staff team work well with health care professionals to ensure that peoples care needs are managed in a multidisciplinary way. Records regarding recruitment are more robust so that the service can demonstrate that they complete satisfactory pre employment checks on staff. Care Homes for Adults (18-65 years) Page 3 of 9 What we found: This random unannounced inspection was carried out by one inspector on 25 June 2010. The visit lasted for three hours. The purpose of the inspection was to establish how the service has addressed the outstanding requirements, made at the last key inspection of 18 May 2009. Evidence for this inspection was gathered in a number of ways. We spoke to the new manager who is in the process of applying for registration. We talked with three staff. We looked at records for three service users and at records of two staff members. We spoke to an involved health care professional over the telephone. We also spent time in the company of service users who were at home during the visit. At the last inspection we made a requirement that: The responsible individual must ensure that residents are able to access dental and chiropody care as required. The health care needs of residents must be regularly reviewed to demonstrate that health care needs are met all of the time. The needs of service users are such that they are unable to provide verbally their views about the health care they receive. We looked at records for three people which showed that each service user has a health action plan that is reviewed regularly. These are written in a person centred way and identify what action is necessary, and by whom, to keep each individual healthy. Records show that any identified need is properly monitored, for example, one person whose weight was a concern was being weighed regularly and any changes had been reported promptly to health care professionals. Files also contained guidance for staff about how to work effectively with people, for example staff were provided with clear written guidance about how to monitor and record one persons blood glucose levels. Staff we spoke to said that they felt confident that they had the skills and understanding to manage peoples health care needs. One healthcare professional said that staff were accommodating and that they were able to gather information from them needed to make an assessment. They said that staff were generally knowledgeable about peoples needs. Records we saw indicated that the service liaises when necessary with relevant health care professionals. We saw that service users had their eyes tested in 2009 and that a podiatrist makes regular visits to the home. Service users who were able to attend dental appointments had done so. For some service users the issue about dental care was more complex and where this was the case, multidisciplinary best interest meetings had taken place where a plan had been devised to ensure that service users receive the most appropriate dental care possible. This requirement has therefore been met The next requirement made at the last inspection was that: The responsible individual must ensure that people working in the home receive training appropriate to the work they do (including structured induction training) and to meet the specific needs of the residents. A training plan must identify how staff training needs will Care Homes for Adults (18-65 years) Page 4 of 9 be met with timescales for action. The manager described what training is available to staff and we saw a training and skills audit that she had drawn up to detail what training each staff member had completed and when this had happened. This showed that most staff had completed four training sessions in communication and maketon in 2010. Records showed that staff have been provided with training in 2010 in first aid, safeguarding, food hygiene, moving and handling and infection control. Renewal of fire safety and health and safety training is scheduled for later in the year. Records highlight which staff have missed training so records are now clear about who needs to do what, and when. Staff members we spoke with described training as Good and Ok and neither could think of any particular training needs that were not already being addressed. We spoke with a staff member who records reflected was overdue in renewing their safeguarding training. They were aware that they needed to update this course, felt that training was in hand and they were able to demonstrate that they had a good understanding of the issues around keeping people safe. The records show that regular medication training is provided to key staff and that all staff had been offered, and most had attended, Timian, (a course to help in safely managing challenging behaviour) Most had also completed deprivation of liberty and mental capacity training. The skills audit did not detail and courses for staff in epilepsy or in understanding autism, but the manger was able to confirm that dates for both these courses had just been arranged for July 2010. As stated earlier, the service also liaises closely with specialist health care professionals who have provided advice and information about individuals needs, for example, suggestions about appropriate activities for a particular service user so that the staff can work with them more effectively. This requirement has therefore been met The third requirement made at the last inspection was that: The responsible individual must ensure that new staff have satisfactory pre employment checks that meet regulatory requirements. The responsible individual must ensure that residents are protected by thorough recruitment practice and demonstrate that new staff are suitable to work with the residents. The manger said that the staffing team was stable with a core group of staff members who have been working at service for a long time. She said that they use agency staff from time to time and have found them to be very reliable. We spoke with one agency worker on shift at the time of visit. They said that regular staff provide enough support and information to them to enable them to assist service users appropriately. We looked at the files for recently recruited staff to establish whether pre employment checks had been thorough. Both files we saw contained all checks necessary, including evidence of a CRB check completed before they started work, two satisfactory references, evidence of identity, completed application form , a copy of the job description and evidence of training that they have already completed. There was also evidence on file that staff had attended recent supervision sessions with the manager. This requirement has also been met.
