Balmoral is a large detached house, located in the Mottram area of Tameside in a semi-rural setting. The home is registered to provide both nursing and residential care. Accommodation is provided in 31 single rooms, 21 of which have en-suite facilities. A separate dining room and lounge areas are available to service users on the ground and first floors of the home. The home has recently completed an extension to increase the number of single rooms and en-suite facilities. Pleasant garden areas are provided to the front of the home. The rear of the property has a secure enclosed patio leading from the main lounge which is accessible to service users. The home provides adequate toilet and bathing facilities with appropriate aids and adaptations to meet the assessed needs of the service users. Care Homes for Older People Page 3 of 9 What we found: The reason for this inspection visit was to look at some concerns we (the commission) had following an Annual Service Review carried out in December 2008. Those concerns related to lack of feedback from people who use the service about the care they are receiving and because information in the Annual Quality Assurance Assessment (AQAA) completed and returned to us by the previous manager of the home was limited. For example, there was very little information about how complaints and safeguarding are managed. We decided to look at four areas of practice and these related to care, complaints, safeguarding and staffing. Each person using the service has an individual file containing all information relating to care planning and practice. We looked at the file of one particular person and saw that information was detailed and contained enough information to enable staff to meet peoples needs in the most appropriate way. At the time of this visit the care plan formats had just been reviewed and updated and included relevant strategies for managing identified risk and all have been reviewed on a monthly basis. Instructions for staff in the care plan were very clear for example, While in bed use a slide sheet for positioning and follow the detailed photographs supplied by the physio. Things such as baths and showers were being recorded separately for each person and we saw these were taking place on a regular basis. Watching staff working with people who use the service indicated that good relationships had been developed and those people we spoke to told us Staff are good and You get well looked after here. We discreetly observed staff responding to call bells and verbal requests from people for assistance. The call bells were responded to quickly in a courteous and pleasant way with good and positive interactions taking place. Further examination of records demonstrated that regular appointments with the chiropodist, speech therapist and other health care professionals were taking place. We looked at how complaints were managed and found that a number of complaints had been recorded in the complaints register since we lasted visited the home. Visiting health care professionals, staff, people who use the service and relatives had made these complaints. The manager fully investigated the concerns raised and provided information to us to show how she had done this. We were told that the manager and deputy manager are due to attend specific training on dealing with complaints on 25/03/09. In the AQAA returned to us by the previous manager of the home details regarding safeguarding referrals was limited. We spoke to the current manager who was very clear about what action she would take in the event of any allegation of abuse being made. We saw that the home had a copy of the Safeguarding Adults in Tameside Inter Agency policy and their own policy and procedure regarding dealing with allegations of abuse. These documents will be looked at more fully at the next key inspection. The manager told us that no staff had received any recent training in safeguarding people. Care Homes for Older People Page 4 of 9 It is important that all staff receive training in this subject to make sure they know what to do in the event of an allegation of abuse being made. On the day we visited the home sufficient staff were on duty to meet the needs of the people who use the service. We looked at the files of two recently employed members of staff. On one file there was only one reference and this was in the form of a tick sheet and was not signed by the person providing the reference. It is important that all pre-employment checks are thoroughly carried out before the person starts work to minimise the risk of unsuitable people being employed to work in the home. We saw evidence that the staff had received various courses of training and this will be looked at more fully at the next key inspection. The manager of the home told us that she had been employed there a number of months but is still not registered with the Commission for Social Care Inspection. It is important that the service is managed by a permanent manager whose experience and qualifications meets the requirements to become the registered manager. This will help to minimise the risk of unsuitable people being employed to work in the home. 2 31 9 The service must appoint a 26/06/2009 permanent manager whose experience and qualifications meet the requirements to become the registered manager. To ensure the operation of the service is delivered effectively so that people receive the support they need at the time they need it.
