Statement of Purpose

Bramley House Residential Home. STATEMENT OF PURPOSE Bramley House is a Care Home operated by our Private Company – Bramley House Westcott Street Westcott Dorking Surrey RH4 3NX Bramley House provides support, care and accommodation for ‘Older People’ both men and women. The current registration enables adults to live here from the age of 65. Some service users may have Dementia and a moderate sensory disability – this does not present an issue, providing that their needs can be met. The home is currently registered with the Care Quality Commission for Sixteen people. The house is a 3 storey detached building set in a large garden with 6 bedrooms on the ground floor and 10 bedrooms on the first floor. Eight bedrooms have full en-suite facilities, four bedrooms have ensuite toilet facilities and 4 have vanity units. The home is able to provide ‘respite care’ for people who normally live in their own home. If an existing client is diagnosed with Dementia they may be able to continue living at the home providing the appropriate support is offered to the client and staff, and their needs can be met without creating difficulty for others. Registered Provider/Manager Bramley House Residential Home Mrs Fiona Collins C/O Bramley House Westcott Street Westcott Dorking Surrey RH4 3NX Bramley House was first set up in 1984 by Mrs Rita Stappard who suddenly passed away in June 2008. Bramley House has always and will continue to be a home for the Elderly. Mrs Fiona Collins wishes to carry on the good reputation that her late Mother built up over the years, offering those who wish to live at Bramley House are provided with the highest possible care and continue to be as independent and active lives as they possibly can. Bramley House Residential Home. Staff Team The provider is supported by 2 Senior Carers and a range of full time & part time carers. All have experience of the care sector and several have been employed in the care field for many years. All the staff have duties that involve contact with and direct care for the service users. Organisational Structure The home is managed by the Registered Provider. The Registered Provider is, in turn, supported by two Senior Carer Workers who report to, and who are reportable to her. Below the 2 Senior Carers Workers are the Carer Workers (Day Time and Night Time) who report to the Senior Care Workers but are reportable to the Registered Provider. The home is not split into any units. When on duty, staff are not allocated to any service users specifically but look after the needs of the whole group. The Senior Care Workers and the Care Workers do act as Key Workers for individual service users though. Shifts are in operation to cover the working week. Morning shifts are typically from 08.00 until 14.00 and the Afternoon shifts are typically from 14.00 until 20.00. Each night one member of staff is on a waking duty from 20.00 until 08.00 with a second member of staff ‘sleeping in’. Apart from the basic shift pattern, some staff may work different hours according to service users’ needs. An ‘on call’ system operates whereby staff working in the home are able to obtain advice or actual physical presence of a more senior person as required. This ‘on call’ system operates each evening and all the weekend. Staff recruitment, induction, training and supervision are in accordance with Government guidelines. Admissions Under Government regulations, potential service users must have their ‘needs’ thoroughly assessed before entering into any service of care. This is intended to provide any potential service user and their families with the best possible information with which to make their choice. The initial assessment will normally be undertaken by a Care Manager from a Social Services Department as part of the care management process. We will also carry out our own assessment of any potential service user, which should have similar findings to the Care Manager’s assessment. Any significant difference in assessments will need to be resolved at this time. Bramley House Residential Home. Should any families of potential service users approach the home directly with no need for Social Service funding there will be a full assessment carried out by ourselves calling on specialist advice where considered necessary. Whichever kind of assessment is made, it will cover the range of health and social needs set out in Department of Health guidance and all information gathered will be treated in confidence. The assessment process helps in ensuring that the home and its staff can meet a potential service user’s needs and also forms an initial ‘Care Plan (Plan of Care)’. All prospective service users and their families will be provided with as much information as possible to help them to make a decision about whether or not they want to live with us. We offer opportunities for prospective service users to visit the home, join us for a meal or a weekend and normally move in on a trial basis at a rate suitable to them. Should we feel that the home is not suitable for a prospective service user then we will try and give advice as to where else to look. Social Activities, Hobbies and Leisure Interests We try to make it possible for our service users to live their lives as fully as possible. In particular we 1. Aim as part of the assessment process to encourage service users to share with us as much information as possible about their social cultural and leisure interests, as a basis for helping them to live happily in the home. 2. Try and help service users to enjoy as wide a range of individual and group activities and interests both inside and outside the home, to develop or maintain hobbies, pursuits and relationships. All service users are entitled to use any communal space inside and outside the home but those who wish to make a positive choice to remain in their rooms will be enabled to do so. 3. Encourage service users to personalise their own rooms and we try to follow individual preferences in matters of décor and furnishings. 4. Go out into Dorking and the surrounding areas i.e. Shere & Gomshall to shop or visit other community facilities when possible and attend appropriate events as they occur. 5. Provide recreational equipment, games, puzzles, art materials, etc. for use by any service user(s) to use individually or in groups as they wish. 6. Recognise that food and drink play an important role in the social life of the home. We try and provide a welcoming and attractive environment in the dining area and ensure that meals are pleasant and unhurried occasions providing opportunities for social interaction as well as more basic nourishment. Service users are encouraged to eat together. Meals can be served in a service user’s own room if they wish. Three meals a day are provided based on a rotating 3 weekly menu, which is changed frequently and to which service users have an input. Bramley House Residential Home. 7. Ensure that snacks and hot and cold drinks are available at all times. We aim to make all food and drink provided attractive and appetising and we also mark special occasions and festivals. 8. Ensure that the home is part of the local community. We welcome visitors to the service users and we respect the views of service users as to whom they want to see or not to see. Service users are able to meet with their visitors in private as they wish. 9. Recognise that some risk taking is necessary to aid selfdevelopment. It is an important and sometimes an enjoyable part of life and we recognise that some service users will wish to take some risks (actively or by default) despite their ability. We do not aim to provide a totally risk free environment, but we will take care to ensure as far as we are able that service users are not subjected to unnecessary hazards. Where a service user wishes to take part in an activity that involves some risk to themselves or to others, then we will carry out a thorough risk assessment with that service user (and others involved) and we will agree and record such action that will appropriately balance the factors involved. Such risk assessments will be regularly reviewed with all parties in the light of experience. 10. Ensure that all of Bramley House is a no smoking area and cigarette smoking is only allowed outside of the actual building. 11. May make additional charges for some social activities and services and, where this applies, details will be made clear to the service users or their families where possible in advance. 12. Try to enable service users to participate in all aspects of life in the home. In particular, service users are regularly consulted as individuals and together about the way that things are done. Service users meetings are held monthly for example and all service users are encouraged to join in and contribute. It is our aim to make the managing and running of Bramley House has a transparent as possible and we strive to ensure that the home has an open, positive and inclusive atmosphere. Daily Respite Care Bramley House Residential Home offers daily respite care. This service is for people who live in their own home and like to visit Bramley House for a day or for a few hours, where they can meet other service users, have a cooked meal, have a bath and enjoy the entertainment. Some service users who live in their own home find that going to a day care centre is not to their liking, where as coming to Bramley House Residential Home they feel more comfortable as it is like a home from home. Consultation with service users All service users are consulted on a daily basis regarding their wishes and needs, and on an ad-hoc basis as required. More formal Service users meetings are held each month, to which all Service users are invited to contribute. Bramley House Residential Home. Fire Precautions, Associated Emergency Procedures, Safe Working Practices All service users are made aware (as best as we are able) of the action to be taken in the event of a fire or other emergency. The home conforms to all government advice on promoting and protecting the health & safety of service users, staff and visitors. Arrangements for Religious Observance Service users who wish to practice their religion / faith will be given every possible assistance. We will contact any minister or religious leader on behalf of a service user to arrange for home visits if this is preferred. We will do our utmost to meet the needs of any service user from a minority faith. The local minister visits the house on a monthly basis to carry out holy communion. The home celebrates all festivals in the communal areas of the home, which are appropriate to the current service users’ backgrounds and faiths, but service users have the right and opportunities not to participate if they wish. Contact with Others All Service users are encouraged and supported to maintain contact with families and friends. All clients have the choice of having their own telephones in their bedrooms for private calls, staff will assist Service users to write letters if they wish. Families and friends are encouraged to visit. Concerns and Complaints The management and staff of Bramley House aim to listen to and act on the views and concerns of service users and to encourage discussion and action on issues raised before they develop into problems and formal complaints. We welcome comments and suggestions from service users and their representatives, friends and relatives, etc. Positive comments help us to build on our success, but we can also learn from comments that are critical. Our policy is to look at and to respond to all comments, criticism or complaints as quickly and as objectively as possible. Service users Care Plan (Plan of Care) When a new service user is admitted to Bramley House, we will work with them, friend or relative - as appropriate - to draw up a written Care Plan stating the care we will aim to provide. This plan will set out the objectives for the care and how we hope to achieve the objectives, and the plan will also incorporate any risk assessment(s). Bramley House Residential Home. This will be reviewed every month setting out whatever changes have occurred and what still needs to occur. This will ensure that the care that we are providing is relevant and helping the service user to achieve their full potential. All service users have access to their Care Plan and each service user is encouraged to participate as fully as they are able in the process. All service users are to sign and agree the care plans. Rooms in the Home Bramley House has sixteen bedrooms for service users. The rooms for communal use are the Lounge, Dining room and conservatory. There are 3 bathrooms 2 have a bath hoist and the ground floor has a Parker bath. There are also 2 separate toilets within the home. There is a fully equipped kitchen on the ground floor. Service users’ Rights The ‘rights’ of our service users are important to our philosophy of care. We will seek ways of advancing these ‘rights’ in all aspects of the environment and the services that we provide and we will encourage our service users to exercise their rights to the full. Privacy – We recognise that living in a communal facility and the need to accept help with personal or intimate tasks is inherently invasive of a service users’ ability to enjoy their own company and being undisturbed. We will therefore try to retain as much privacy as possible for our service users in the following ways by – 1. Providing help and assistance in intimate situations as discreetly as possible. 2. Helping service users to furnish and equip their own rooms in a style appropriate to them. 3. Encouraging the service users to use their rooms as much as they wish for leisure, meals or entertaining. 4. Offering a range of locations around the home where service users can be alone or with others of their choosing. 5. Providing lockable facilities for service users to store personal items. To enable service users to be alone or with others. 6. Ensuring service users privacy in making and receiving telephone calls (unless actual practical assistance is necessary) receiving post (unless service users need practical assistance to open, read or understand post), and meeting with friends or relatives. 7. Ensuring that confidential information held in the home about service users is secure. Dignity – Bramley House Residential Home. As any disability may undermine a service user’s dignity we will try to maintain our respect for service users’ dignity in the following ways by 1. Treating each service user as a special and valued person. 2. Helping service users to present themselves to others positively through their own clothing, appearance, possessions and behaviour. 3. Offering a range of activities, which will enable service users to express themselves as a unique individual. 4. Tackling any stigma that any of our service users may experience through age or disability. 5. Endeavouring as much as possible to compensate for the effects of any disability, which service users may experience with their communication, physical functioning, mobility or appearance. Independence – We offer as much support as possible to be as independent in their living situations. We therefore feel that it is important to foster and develop opportunities for our service users to think and act more independently by – 1. Providing any physical or mechanical assistance necessary as tactfully as possible. 2. Making the most of the service users’ abilities to be independent and encouraging them to develop their skills and take reasonable risks – following full risk assessments. 3. Assisting service users to establish and maintain contacts outside of the home. 4. Enabling service users to have access to and contribute to their own care records. Security – Bramley House will provide an environment, which supports service users by – 1. Offering assistance with tasks and in situations, which would be perilous for service users. 2. Avoiding, as far as is possible, any dangers, which are commonly found with elderly people. 3. Protecting service users from all forms of abuse and from all possible abusers. 4. Providing easily accessible channels and forms of complaint in a variety of formats. 5. Endeavouring to provide an atmosphere within the home, which is open and positive for all service users. Civil Rights – Living in a care home and becoming older can all act to deprive service users of their rights as ordinary citizens. We will therefore Bramley House Residential Home. work to maintain our service users’ place in society as participating and benefiting citizens by – 1. Ensuring that those service users who wish to vote will be enabled to do so. 2. Assisting service users to access all elements of the National Health service as required and assisting service users to choose / change doctors or dentists, etc. as appropriate. 3. Assisting service users to access public facilities such as shops, libraries etc as appropriate. 4. Encouraging service users to contribute to the wider society, and in their home, through helping each other and taking on some responsibilities within and, where possible, beyond the home. Choice – Service users will be assisted and encouraged to have opportunities to make choices in all aspects of their lives by – 1. Providing meals, which as far as possible enable service users to decide for themselves from a range of options where, with whom and when they will eat. 2. Offering a range of leisure opportunities. 3. Encouraging service users to manage their own time and activities. 4. Avoiding, whenever possible, treating service users as a group. 5. Respecting individual, unusual or eccentric behaviour in service users that does not unduly impose on other service users. 6. Retaining as much flexibility as possible in the home’s daily routines. Fulfilment – We wish to help service users to realise personal aspirations as best we can in all aspects of their lives. We will do this by – 1. Keeping ourselves informed as fully as possible about each service user’s personal characteristics and histories. 2. Providing a range of leisure and recreational activities, to suit the ages, wishes and abilities of all service users. 3. Responding appropriately to the personal, intellectual, artistic and spiritual values and practices of every service user. 4. Respecting each service user’s religious, ethnic and cultural diversities. 5. Helping our service users to maintain existing contacts and to make new liaisons and friendships. 6. Always attempting to listen and by attending promptly to any service users wishes to communicate at whatever level. Quality Care Bramley House Residential Home. As we wish to provide high quality care, we will give priority to a number of areas relating to the operation of the home and the services we provide. Choice of Home – We recognise that each prospective service user or their relatives should have the opportunity to choose a home, which suits their needs and abilities. To assist with that and to ensure that prospective service users and their families know exactly what we offer, we will – 1. Provide a ‘Statement of Purpose’, which gives detailed information about the home in a form that may also be understandable to the service user i.e. large print. 2. Provide a detailed ‘Contract’ or ‘Statement of Terms and Conditions’, specifying the details of the relationship in a form that may also be understandable to the service user. 3. Before decision will be taken about admission, we will ensure that an assessment has taken place and the service user’s needs can be met. 4. Demonstrate to every prospective person and their families that we are confident that we can meet assessed needs. 5. Offer introductory visits to a prospective service user and attempt to avoid unplanned emergency admissions. Health & Personal Care – We will draw on expert guidelines for the services that the home provides. In our pursuit of providing the best possible care, we will – 1. Produce as far as possible with each service user’s input a Plan of Care, which will be updated regularly and reviewed completely once a year. This Plan of Care will be based on the initial (if that is recent) and continuing assessments. 2. Seek to meet or arrange for the most appropriate professionals to meet the health care needs of the service users. 3. Establish and carefully carry out medication administration procedures for those service users who can, or do not want to, self-administer their medication. 4. Take steps to safeguard the privacy and dignity of service users in all aspects of providing care. 5. Treat with special care those service users who are dying or near death. Daily Life / Social Activities All our service users need some care and support in a range of aspects of their lives. We will respond to the wide variety of needs and wishes of service users by – Bramley House Residential Home. 1. Aiming to provide a lifestyle for service users, which will satisfy their social, cultural, religious and recreational interests and needs. 2. Helping service users to exercise choice and control over their lives. 3. Providing meals and drinks, which constitute a wholesome, attractive and balanced diet in pleasant surroundings and at times convenient to service users. Complaints / Protection – Despite all that we do, we are aware that service users may be unhappy from time to time about some aspect of the house or their care – and may even suffer some form of abuse within or outside of the home. To deal with such issues we will – 1. Provide, and when necessary, operate a simple, clear and accessible ‘Service users Issues’ and ‘Complaints’ procedure. 2. Take all necessary action to protect the legal and moral ‘rights’ of service users. 3. Make every effort to protect service users from any form of abuse and from various possible abusers. Environment – The physical environment of the home is designed for the service users’ comfort and use. We will – 1. Maintain the buildings and grounds in a safe condition 2. Make detailed arrangements for the communal areas of the home to be safe and comfortable. 3. Supply toilet, washing and bathing facilities that are suitable for the service users we care for. 4. Arrange for any necessary specialist equipment to be provided in order to maximise service users’ mobility and independence. 5. Provide individual accommodation that meets the National Minimum Standards. 6. Ensure that service users have safe, comfortable accommodation together with their own possessions around them. 7. Ensure that the premises are kept, clean, hygiene and free from unpleasant odours with systems in place to prevent the spread of any infection. Staffing – Care staff plays a very important part in the service users’ welfare and happiness. To maximise this contribution, we will – 1. Employ staff in sufficient numbers and with the relevant mix of skills to meet service users’ needs. 2. Provide at all times an appropriate number of staff with qualifications in health and social care. Bramley House Residential Home. 3. Observe recruitment policies and practices, which both respect equal opportunities and protect service user’s safety and welfare. 4. Ensure that all staff appointments are subject to a minimum three-month probation period. 5. Offer staff a range of training, which is relevant to their induction, foundation and further career development. 6. Install and operate effective quality assurance and quality monitoring systems. 4. Work to accounting and financial procedures which safeguard service users’ interests. 5. Supervise all staff and volunteers regularly and carefully. 6. Keep up to date and accurate records on all aspects of the home and its service users. 7. Ensure that the health and safety of service users, staff and visitors to the home are promoted and respected. Underpinning Elements Focus on Service users – We want everything that we do to be driven by the needs, abilities and wishes of the service users and not by what staff, management or any other group’s wishes. We recognise that this focus might easily slip and we will try to ensure that the facilities, resources, policies, activities and services remain service user led. Fitness for Purpose – We are committed to achieving our stated aims and objectives and we welcome fair scrutiny by our service users and / or their relatives. It should be noted that relatives are not automatically entitled to confidential information about their relatives. Comprehensiveness – We aim to provide a total range of care, working with other professionals, to meet the overall personal and health care needs and preferences of each service user. Meeting Assessed Needs – Bramley House Residential Home. The care we will provide is based on a thorough assessment of needs and the systematic and continuous planning of care for each service user. Quality Services – We aim for a progressive improvement in the standards of training at all levels of our staff and management.
