Carders Court consists of five separate units each providing care to meet the differing needs of the residents. Two units provide care for residents with a dementia (Linden and Brookfield House), two provide personal general care (Arkwright and Garfield) and one unit (Rakewood) provides general nursing care. Each unit has 30 beds, and all personal accommodation is provided in single rooms that are on the ground floor. The communal areas on each unit comprise a lounge, dining area and conservatory. Garden and patio areas surround each unit. There is a central administrative block that contains the main office, kitchen and laundry areas and a hairdressing salon. Ample parking areas are provided for visitors and staff. Carders Court provides care for 150 people, male or female, generally over the age of 65 years. Care can be provided for an agreed number of young adults, who are aged between 50 and 65 years of age, who have physical disabilities. The registered provider is Care First Health Care Ltd, owned by BUPA. Fees charged by the home, provided in February 2008, are in the range of: £339.90 to £591.01. Additional charges are made for personal toiletries, hairdressing, chiropody and trips out. What did we do for this annual service review? We looked at all the information that we have received, or asked for, since the last key inspection or annual service review. This included: ï· The annual quality assurance assessment (AQAA) that was sent to us by the service. The AQAA is a self-assessment that focuses on how well outcomes are being met for people using the service. It also gave us some numerical information about the service. Surveys returned to us by people with an interest in the service. Information we have about how the service has managed any complaints. What the service has told us about things that have happened in the service, these are called ‘notifications’ and are a legal requirement. The previous key inspection, random inspection and the results of any other visits that we have made to the service in the last 12 months. Relevant information from other organisations. What other people have told us about the service. ï· ï· ï· ï· ï· ï· What has this told us about the service? We conducted a random inspection at Carders Court on 14th November 2008. This involved a visit to Carders Court to look at some concerns that were brought to our attention. We had received concerns about the number of staff available on one of the units, and we had been told about a unit that always held a bad smell. At the random inspection we found that staffing levels on the unit in question met peoples needs and that additional staff was made available when necessary. At the random inspection we found that the unit in question did not smell on the day that we where there, although the manager did tell us that this unit has had a problem with unpleasant smells for some time, and that she plans in place to address this. Her plans included replacing carpets, purchasing additional cleaners and updating the training for staff in how to use cleaning products effectively. The manager sent us their annual quality assurance assessment (AQAA) when we asked for it. It was clear and gave us most of the information we asked for. We looked at the information in the AQAA and found that ï· ï· The policies and procedures in the home are updated by BUPA in October and February each year. The manager needs to identify improvements in relation to Health and Personal Care in the previous 12 months, and she also needs to identify what the service could do better in this area. We received three surveys returned to us by staff. They said the communication at Carders Court was effective. They also said they felt the quality of care was good and that the home was usually sufficiently staffed. The staff also confirmed that the training provided prepared them for their work. At the time of writing this report, we have not received any comments from people living at the home; we will ensure that any comments received will be reflected in the next key inspection. Since the last inspection Key Inspection we have received six complaints about the service at Carders Court - three from relatives, one anonymous and two from Social Services. We have also been told about adult protection investigations that are taking place in the home. We feel that the outcomes of investigations that we have been informed of so far, and other correspondence indicate that the manager has responded appropriately to complaints, concerns and allegations. The manager has kept us informed about the incidents and accidents in the home through sending us in notifications. We have requested that these are completed in more detail, particularly, in relation to names and contact details for other professionals who may also be dealing with the incident. We have also met with the management of the home for them to provide us with an update on how they are addressing concerns/complaints brought to their attention. They told us they are continuing to monitor staffing levels in line with people’s dependency and they are satisfied that they are enough staff on duty. In relation to the particular smell on one unit they have now provided new carpets, additional training to staff in use of equipment and cleaners, and purchasing additional cleaning equipment. The manager was confident that such measures will stop the unit smelling. What are we going to do as a result of this annual service review? We are not going to change our inspection plan, and will do a key inspection by 2nd February 2010. However we can inspect the service at any time if we have concerns about the quality of the service or the safety of the people using the service.
