What the care home does well Good systems are in place to support anyone considering moving into 16 Carnatic Road. These include inviting the person and their family to visit and obtaining as much information about their support needs and choices. This helps everyone to decide if the home is the right place for the person to live. Clear records and care plans are in place for everyone living at 16 Carnatic Road. These provide a good basis for staff to understand how people communicate and plan the support they need and choose. The people living at 16 Carnatic Road receive a good level of support with their health care. Staff ensure their health is regularly monitored and that advice is sought from healthcare professionals if needed.The home supports people to maintain contact with their families and welcomes visitors at all reasonable times. The people living there receive support to make daily decisions about their lives and routines are flexible to reflect this. There is a clear complaints system in place within the service that relatives are aware of. In addition staff have received training in safeguarding adults and are able to explain how they can understand via peoples non-verbal communication if they are unhappy with something. Before anyone starts work in the home good checks are carried out on them. This helps to ensure that they are suitable to work with people who may be vulnerable. A good training programme is in place for new staff to ensure they are equipped with the skills and knowledge to support people safely and well. Staff working at 16 Carnatic Road know the people living there well. They have a good knowledge of peoples support needs, their choices and communication methods. What has improved since the last inspection? Since the last big inspection of 16 Carnatic Road the organisation have introduced financial assessments for the people living there. These help to make sure that any large sums of money spent are spent in the persons best interests. What the care home could do better: Staff need to make sure that the people living at 16 Carnatic Road are offered a healthy diet that meets good practice guidance in this area. This will help to ensure that people are receiving as much support as possible to remain healthy. The people living at 16 Carnatic Road do not get out and about in their local community very much. Staff are aware of this and the service needs to ensure that it follow through on its intentions to plan 1-1 support for people to get out more. The shower room within the bungalow appears shabby and uninviting. The service needs to look at ways to improve this room so that it becomes a more inviting place for people to use. They also need to risk assess the arrangements in place for staff supporting people to have a shower and ensure current arrangements are as safe as possible for everyone concerned. Training for long standing members of staff needs to be better planned and delivered in order to ensure staff are equipped with current knowledge and skills to support people safely. Conditions of registration: Date of last inspection 21st November 2006 Brief Description of the Service: 16 Carnatic Road is a small care home registered to provide support and accommodation for three adults who have learning disabilities. The home is run by Community Integrated Care (CIC), a registered charity who provide support services across the country. The Bungalow itself is owned by a local housing association who have responsibilities for the upkeep of the premises. 16 Carnatic Road is a bungalow situated in a residential area of Mossley Hill, Liverpool 18. All areas of the house are accessible for people with mobility difficulties. Everyone living there has their own bedroom and share a lounge, dining area and bathroom. Outside there is an enclosed back garden with seating. Staff are available 24 hours a day to support people when needed. This is an overview of what the inspector found during the inspection. The quality rating for this service is 2 star. This means the people who use this service experience good quality outcomes. Information for this inspection was gathered in a number of different ways. An unannounced visit took place for three and a half hours on the 18th March 2008. This included reading records and looking at the building. We spent time looking at the support the people living at 16 Carnatic Road receive. This included looking at the support they get with their care plans, medication, money, activities and environment. We met all of the people living at 16 Carnatic Road and with three members of staff including the manager. Any information we have received since the last big inspection of the home is also taken into account in writing this report. An easy to understand summary of this report is available. The Commission consider Standard 2 the key standard to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 2 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Polices are in place to help people considering moving into 16 Carnatic Road to decide if it is the right place for them to live. EVIDENCE: The people who live at 16 Carnatic Road have lived there for some time, therefore it is not possible to practically assess the support that would be offered to anyone new who wished to move in. However there is a policy in place from CIC stating how they will support people considering moving in. This says that a full assessment of their needs would be carried out, they would be offered information about the service and invited to visit. The manager confirmed this and explained the person and their family would be as involved in the process as possible. She also explained the person would be able to stay overnight if they wished to do so. This process will help to make sure that everyone has enough information about the person and the service to decide if 16 Carnatic Road is the right place for them to live.

This judgement has been made using available evidence including a visit to this service. Peoples individual choices and needs are identified and action taken to meet these. EVIDENCE: Everyone one living at 16 Carnatic Road has individual care plans and records in place. We looked at records for two of the people living there. These were all up to date and had been reviewed regularly. This helps to make sure that any changes to the persons support needs or their choices are quickly noted and can be acted upon. Plans contain information about the support people need with their personal and health care as well as information about the things they do and don’t enjoy. There is also a section on how people communicate non verbally and how staff should respond to this. Although staff working in the home during
Carnatic Road (16) DS0000025235.V354727.R01.S.doc Version 5.2 Page 12 our visit were able to explain how people communicate this is good practice as it helps unfamiliar staff to communicate more effectively with people. Individual risk assessments are in place for people. These identify any risks to the person’s safety and provide guidance to staff in reducing or removing them, whilst still supporting people with the chosen activity. The people living at 16 Carnatic Road have a number of care files. This includes an essential lifestyle plan that has been written with input from them where possible and their family. They also have separate health plans and care plans. In addition some files contained information that was out of date and no longer used. The amount of files and information contained within them may make it difficult for staff to access information quickly and efficiently. Care plans contain references to people using ‘cot sides’ and being ‘wheelchair bound’. The use of terms such as these are neither as respectful nor as adult as they should be. Staff were able to explain how people make choices in their daily lives and indicate if they do or do not wish to so something. This is backed up with clear records of peoples likes and dislikes and how to understand their communication methods. None of the people living at 16 Carnatic Road are able to manage their own money. However where the organisation do this, clear financial assessments have been completed. These help to ensure that any decisions made about peoples money are taken in their best interests. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 12, 13, 14, 15 & 17 Quality in this outcome area is adequate. This judgement has been made using available evidence including a visit to this service. The people living at 16 Carnatic Road receive some support to live a lifestyle of their choosing. Increased support in this area would enable people to lead more active and potentially enjoyable lives. EVIDENCE: The staff rota showed that there are usually two members of staff working at 16 Carnatic Road and staff we spoke with confirmed this. Staff also advised that if going out all of the people living there need 1-1 support from a member of staff. They further explained that there is sufficient equipment and risk assessments in place for one member of staff to go out whilst another supports the two people staying at home. However records since the 1st March 08 showed that people had not been out and about very much in the past eighteen days. For example one person had

His care plan showed that he enjoys eating out, going for a walk and concerts. In February 08 the same person had been out only four times. Twice to the light room and once to a disco. As both the disco and light room are services for adults with learning disabilities, people are not being provided with many opportunities to take part in their local community. Both the manager and staff explained that they are aware of this and had discussed the issue at a recent staff meeting. The manager explained that she intends to identity 1-1 support via the staff rota in order to ensure people can get out and about a bit more. Records in the home showed that people’s family and friends are welcomed to visit. They also showed that staff support people to speak to their family on the phone if they wish to do so. Discussions with staff and records in the home showed that routines are flexible based on the persons choices. For example people can have a lie-in in the morning if they wish. Staff were able to explain the support they offer to people with meal times and their likes and dislikes. Individual records of menus are kept. Although these showed that people are offered a variety of meals, there was little evidence that they are offered sufficient fruit and vegetables. For example in one week records showed only seven portions offered. It is important that people are offered sufficient fruit and vegetables in order to support them to remain as healthy as possible. The home has a mini bus and the people living there contribute to the running costs of this. However staff advised that as it is old petrol costs can be high and it is needs regular repair. Financial assessments for the people living there show that a mini bus is the best use of their money for transport. However given the age of the bus a replacement may be needed shortly. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 18, 19 & 20 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The people living at 16 Carnatic Road receive a good level of support with their health and personal care. EVIDENCE: Peoples care plans contain clear, up to date information about the support they need with their health and personal care. This helps to ensure that they always receive the support they require in a way that they prefer. In discussions with staff they were able to explain how they support people in a way they prefer. Staff monitor peoples health care needs in order to quickly identify and deal with any issues that arise. For example records are maintained of any seizures people may have and their weight is regularly checked and recorded. This is particularly important, as the people living at 16 Carnatic Road cannot verbally tell people if they feel unwell. The clear record keeping along with staff knowledge of how people communicate helps to ensure any issues are quickly noted and dealt with. Carnatic Road (16) DS0000025235.V354727.R01.S.doc Version 5.2 Page 16 This was evident in records, which showed staff had supported one person to see his GP and had then followed this up with a second visit when he did not fully recover. The people living at 16 Carnatic Road are supported by staff to see a range of health professionals. This includes seeing the Optician, District Nurse and Dentist. These regular checks alongside appointments as and when needed help to monitor people’s health and ensure they receive the support with it that they need. Staff were able to explain how they support people with their personal care and it was evident that this is carried out in a way that maintains their dignity and privacy. During our site visit it was evident that the people living there had received support to maintain their appearance and personal care. A locked cabinet is provided to store medication. During our visit medication was stored correctly. Records had been completed and the amount of tablets held tallied with these. It was clear that people are receiving the medication they are prescribed as and when it is due. This judgement has been made using available evidence including a visit to this service. Clear systems are in place for dealing with any concerns or complaints about the service. This helps to ensure that any issued can be quickly noted and acted upon. EVIDENCE: No concerns or complaints about the service provided at 16 Carnatic Road have been received in the past 12 months. Staff who work there have received training in dealing with safeguarding adults issues and copies of the organisations complaints policy and Local Authority safeguarding adults policies are available on the premises. Care plans for the people living there contain information about how to identity what the person is communicating. A member of staff was able to verbally explain to us how they can identify if the person is unhappy about something. As the people living at Carnatic Road do not use verbal forms of communication, the training and knowledge staff have helps to ensure that any concerns can be identified and dealt with quickly. Relatives of the people living at 16 Carnatic Road have previously confirmed to us that they are aware of the organisations complaints policy and how to use it.

This judgement has been made using available evidence including a visit to this service. 16 Carnatic Road provides a safe, comfortable environment for people to live in. More consideration to how the bungalow is adapted and decorated would further support people with their choices and disabilities. EVIDENCE: 16 Carnatic Road is situated in the Mossley Hill area of Liverpool within a short walking distance of Sefton Park and close to local shops and bus routes into the City and nearby localities. It is a three bedroom bungalow owned by Maritime Housing and leased through contracts to CIC and the people living there. There is a small garden to the front of the home, with some parking and a large, well laid out and enclosed garden to the rear. The rear garden has lawns, patio and paved areas and a raised sensory bed. All areas of the bungalow and gardens are accessible for people with mobility difficulties.

This was domestic in appearance however no adaptations had been fitted to make it easier for people who use a wheelchair to access the facilities in there. Staff were unaware if this had been considered when planning took place for the new kitchen. As 16 Carnatic Road supports people who also have physical disabilities, this is a lost opportunity. Adaptations such as a lower sink and work surface may have supported people to become more involved in their everyday lives and increase their independence both now and in the future. The people living at 16 Carnatic Road all have their own bedrooms. These are equipped with their personal possessions and equipment to support them with their mobility. We noticed that all areas of the bungalow had similar wallpaper and décor. Staff advised that this is because the choice of materials offered is limited. More choice would enable people and their support staff to make rooms more personal and reflective of the choices and personalities of the people living there. There is one shower room in the bungalow that is shared by the people living there. This provides an accessible shower that can be easily used by someone who has a wheelchair. However the room was uninviting in appearance and needed some attention. The seal around the shower appeared to have perished, skirting boards were dusty, grout was stained and the toilet roll holder was broken. Two members of staff we spoke with explained that the shower grid cannot be lifted for cleaning and that it often has an offensive smell. They also explained that as there is no door for the shower they find this a risk, as they can get very wet when supporting people and potentially slip. All other areas of the home were clean and tidy during our visit. Staff have access to disposable gloves and aprons to us if needed, this helps to prevent a possible outbreak of infection. JUDGEMENT – we looked at outcomes for the following standard(s): 32, 34 & 35 Quality in this outcome area is adequate. This judgement has been made using available evidence including a visit to this service. The people living at 16 Carnatic Road are supported by a staff team who understand their support needs and choices. A lack of staff training in some areas could affect the wellbeing of the people living there EVIDENCE: Discussions with staff working at 16 Carnatic Road showed that they have a good understanding of the support needs and choices of the people living there. Staff were able to explain how they understand peoples non-verbal communication and respond to this. During our site visit the three people living at 16 Carnatic Road were watching TV with staff and appeared comfortable in their presence. Over half of the staff team have obtained a qualification (NVQ) in care. This helps to equip staff with the skills and knowledge to support people in line with good care practices. Records showed that before anyone is appointed to work at 16 Carnatic Road a series of checks are carried out. These include obtaining written references

These checks help to ensure the people living there are safe and that staff are suitable to support them. The company have a clear programme in place for new staff to equip them with the skills and knowledge they need to support people. Discussions with a newer member of staff confirmed that this is followed. New staff spend their first few days working as additional staff and getting to know the people living there. They follow a set learning programme that covers a variety of areas. These include fire, safeguarding adults and providing personal care. This process helps to ensure that the people living at 16 Carnatic Road are supported by staff who have the knowledge and skills to support them safely and well. Records for staff who have worked at the home for some time show that they are supported to undertake some training each year. Recent training has included supporting people with medication and dealing with safeguarding issues. However they did not all have up to date training in other areas such as moving and handling people and fire. This could have an impact on the safety of those who work and live at the home. No training plan for the forthcoming year was in place. A training plan would help to identify both gaps in staff knowledge and areas in which they needed updating. Training could then be prioritised and planned. The manager stated that she is aware of the need to update training in some areas and plans to request this. EVIDENCE: Mrs Sharon Bevan is the appointed manager at 16 Carnatic Road. She is experienced in working with adults who have a learning disability and has been managing the home for several months. Staff spoken with told us that they find her approachable, supportive and active in promoting improvements to the service. Records showed that Mrs Bevan is undertaking training in managing a care service and also attends other training relevant to her role. This registration helps to ensure people are suitable to work as managers of a registered service. There are several systems in place for reviewing the quality of the service provided at 16 Carnatic Road. Each month a senior manager from the organisation visits the house and carries out a review of the service. This covers staffing, resources, care and planning. Following on from this visit an action plan is drawn up to deal with any issues that have arisen. Files for the people living there showed that surveys had been sent to their relatives seeking their views of the service. Replies that had been received were positives about the service and support offered. There are a number of checks carried out within the home to check the service regularly. These include checking people’s money daily, reviewing their care records and auditing their medication weekly. These checks all help to ensure that 16 Carnatic Road is providing an effective service, to note and quickly deal with any potential issues that arise and plan future improvements. Records and certificates showed that regular heath and safety checks and tests are carried out to ensure the house is a safe place for people to live in. This will help to ensure that people are provided with clean, welcoming facilities within their home. The shower rooms must be monitored for any offensive odours rising from the shower grid. This will help to ensure staff have up to date knowledge and skills and can support people safely and well.
