What the care home does well People living at Cotleigh receive a good service. Sheffcare, the manager, and staff have ensured they have improved practices, in order to meet their previous requirements. People living at Cotleigh are safer as a result. People living at Cotleigh can express their concerns. The complaints procedure is robust and easy to access. In the main staff practices, the homes policies and procedures, training and compliance with complaint and safeguarding procedures, help to safeguard people, and protect their rights. What the care home could do better: We have issued three new requirements and one recommendation, referring to people`s safety and rights. Make sure staff understand the implications of Deprivation of Liberty. This helps to protect people`s rights. Make sure staff follow the correct recording procedures following an incident. Including completing the accident record fully, and notifying the Care Quality Commission of incidents that affect people`s well being. This helps make sure safeguarding practices are robust to help keep people safe. The manager also needs to take action to eliminate the smell of urine starting to occur on the dementia unit. It provides personal care for up to 62 older people, 30 of whom have dementia. The Care Homes for Older People Page 2 of 11 Brief description of the care home home provides its accommodation over two floors. There are passenger lifts and stairs to facilitate access between them. The first floor is used for older people and the ground floor for people with dementia. The doors on the ground floor are kept secure to prevent people with dementia from wandering from the building. There are lounges, dining rooms and small domestic kitchens on both floors. On the ground floor there is a cafe area, hairdressers and office reception area. All bedrooms are en-suite. Each floor has an assisted bathroom and ample communal space. There was a secure and accessible garden. The manager informed us that the range of fees charged at 16 April 2010 was between £377 and £432 per week and this includes a weekly top up fee. There are extra charges for hairdressing, personal toiletries and private chiropody. The home has produced its statement of purpose and service user guide, which gives detail of its services. Further information can be obtained from the manager. Care Homes for Older People Page 3 of 11 What we found: We carried this inspection out as a Random Inspection. This means we visit the service and look at chosen areas concerned with the health, safety, and wellbeing of people who use the service. We will only change a star rating following a key inspection. Therefore, the rating for this service will stay the same following this random inspection. This inspection complies with our regulatory processes. The Care Quality Commission will conduct a further review under our new registration procedures before October 2010. The service had its last key inspection on 04 June 2009, and we gave them a 1 Star, adequate, rating. Before we visited the service we looked at any information we had received since the services last inspection. This included, The services AQAA (Annual Quality Assurance Assessment). This is information the service is required to provide us with about how well they are delivering care. The services previous report, and any information from other stakeholders, such as the local authority, relatives and staff, and any surveys we received. Notifications. These are notices of information a service must give us about any incidents that happen which may affect peoples wellbeing. Any safeguarding, concerns and complaints information. During the visit we spoke to people who live at Cotleigh, two relatives, staff and the manager. We observed some care practices and looked at some records and documents. For the purpose of this random inspection we looked at how the home manages concerns, complaints and protection. (National Minimum Standards 22 and 23) This included, How well people can express their concerns. Access to a robust complaints procedure. How the service protects people from abuse. And how the service protects peoples rights. We also looked at requirements and recommendations we made to the service on our last Care Homes for Older People Page 4 of 11 inspection. This included, Improving care plans, recording the quantity of medications, improving activities, carrying out repairs, and making sure an effective quality monitoring tool was in place. We spoke to four people who live at Cotleigh. They told us their care was good, they feel safe and well protected, and they can raise concerns and complaints if they need to. People made positive comments, which included, Yes, I feel safe here I have nothing to complain about It is alright here, yes, it is very good What is good here? All is good here We spoke to two relatives, they told us they felt their family member was safe. They said, Mum is very safe, we have no worries We visit 2 to 3 times a week, there are always staff around supervising We go straight to the manager if we have any complaints, we have no problem doing that, we are very satisfied What is good about Cotleigh? It is safe and secure, and there is a regular routine, which helps people One relative commented We have noticed there has started to be a smell of urine, we see staff cleaning floors, but it is still there We advise the manager to look into this to avoid the situation getting worse. We spoke to two members of staff. And we spoke to them in detail about their understanding of protecting people from harm, and protecting peoples rights. They understood what to look out for to identify if someone could be at risk of abuse, and they knew the right action to take to help people keep safe. They were aware about mental capacity and what this meant to people they cared for at Cotleigh. Staff also understood how to whistle blow if they felt people were at risk, and action was not taken. We saw evidence that staff had good access to training such as dementia care and Care Homes for Older People Page 5 of 11 infection control. The manager had also booked two people every eight weeks on to Sheffield Councils Safeguarding training, in order to keep staff up to date with local policy. This helps make sure people are well protected from harm and poor practices. The manager told us staff have an introduction to safeguarding vulnerable adults (Adult protection) during their induction, then have more in-depth training on an intermediate level. This shows that Cotleigh is proactive in making sure they help protect people they care for. The manager said they understood Sheffcare was reviewing their Safeguarding training package to make sure it included Mental Capacity and Deprivation of Liberty. We noted that each person has a Mental Capacity Assessment in their care plan. We advised the manager that they could improve this by making sure there was a direct link to the individual care plans where some one may lack capacity. Staff were not confident about how Deprivation of Liberty affected the people they care for. We advised the manager to look into how to improve staff understanding. The manager agreed to do this. The manager said they had dealt with three complaints in the past 12 months. One of these was referred to safeguarding, however, no further action was needed. The manager has also made another referral where they had become concerned about someones welfare. The manager is waiting for Sheffield Local Authorities response on this. Safeguarding and the complaints are now concluded, and the home followed correct procedures, and worked with the local authority in order to keep people safe. As an outcome the manager implemented improved practices to help avoid similar incidents. We concluded by, talking to people, looking at records and checking the environment that the service has now met all its previous requirements. We checked a sample of accident records, these related to events in March 2010. We found that there were a significant amount of falls, and person to person incidents. Whilst the service had notified us of some of these incidents, they had not notified us of all that related to peoples welfare put at risk. We also noted that some accident records were incomplete, failing to identify what action staff took to ensure peoples safety. For example, whether they sought medical help, or put monitoring in place to check the persons welfare. We have made a requirement about these findings because it is important the home follows correct procedures to protect peoples welfare and rights. The manager told us the home had done some work in the past with the Older Peoples Reviewing Team in order to reduce the number of falls happening at the home. We strongly advise the service revisits this, and continues to take action to establish why Care Homes for Older People Page 6 of 11 the number of falls is so high, and how to reduce these, without affecting peoples independence and freedom to mobolise. This should continue while ever falls incidents remain high.
