Court Lodge Nursing and Residential Home is situated in a quiet close on the outskirts of Lymington, within easy reach of local amenities. The home is registered as a care home to provide both nursing and personal care for 43 service users in the older person category. It can also provide for individuals who are terminally ill and for a limited number of older people with dementia or with a physical disability. Colten Care Ltd owns the service with several other homes in Hampshire and Dorset. The premises were purpose built and service users accommodation is located on the ground and first floors of the building and access is provided by two passenger lifts. The communal areas comprise two large lounges and a dining room on the ground floor and a small lounge/library on the first floor. Bedroom accommodation consists of 39 single rooms and one shared/double room and all have en-suite WCs. The service users have the benefit of large well-maintained gardens that are accessible to wheelchair users.

What the care home does well The home`s activities programme for the residents is excellent with daily events and outings. Residents are able to choose what food the would like to eat and at times and location oftheir choice. Residents are consulted about their daily lives and their concerns addressed. The environment is of a high standard with en suite facilities in each bedroom, attractive communal areas, a good standard of furniture and fittings and well maintained grounds and gardens. Care Homes for Older People Page 3 of 10 What we found: The inspection consisted of a 2 hour site visit where policies, documents and records were looked at. Discussions took place with the Registered Manager and 2 staff were also spoken to. 2 residents were interviewed about life at the home. Residents and staff were observed. Surveys were sent to staf and residents to ask their views on the service provided by the home. These were returned by 2 staff and 4 residents. Communal areas and a sample of bedrooms were seen. Choice of Home Residents are able to visit the home before making a decision about whether or not to move in. Information is also available to those considering a move into the home in the form of a Statement of Purpose and Information For Our Residents pack. Residents confirmed that information was provided to them before they moved in. Health and Personal Care Staff stated that the home is very good at looking after the residents and that care needs are met. Information is provided to staff regarding the care needs of the residents. Residents said that they are looked after well, that they receive the support they need and that their personal care needs are met. One person made reference to the laundry saying, Its marvellous. You give it to staff in the morning and it comes back clean by the end of the day. Surveys showed that residents receive the care and support they need although comment was also made that some improvement is needed regarding awareness of specific disabilities. Daily Life and Social Activities The home has an activities co ordinator who works for 30 hours a week. A 4 week activities programme was displayed in the home as well as a daily timetable of events in the home as follows: 1030am room visits, 1100am quiz, 12 noon sherry and chat, 1400 entertainer, and 1500 tea and skittles. Residents were observed sitting in the garden and taking part in a quiz. The homes lounge was decorated to celebrate St. Georges Day. Residents described the provision of activities as very good. One person said how trips to the beach and to the shops take place. Comments was made that, There is always a lot going on. Another person said how he/she can choose to stay in his or her room and enjoys joining the other residents for company in the lounge. 1 person commented that the activities are not always provided when the activities co ordinator is on leave. Another person said that the availability of outings for those with mobility needs could be Care Homes for Older People Page 4 of 10 improved. Residents stated that they are able to spend time as they wish to and that breakfast meals are provided as chosen and at the times preferred by the individual. A resident said, The breakfasts are exceptional. You can have what you want. The days menu was displayed in the hall. Residents confirmed that there is a choice of food and that they are asked in advance what they would like to eat. A resident said that fresh fruit is provided. Fruit bowls were seen in residents rooms. One resident said that suggestions for improving the menu had been acted on by the homes catering staff. A comment was made by 1 person that the breakfast choice is not always provided and fresh fruit is not always available unless requested. Complaints and Protection The home has a complaints procedure in the Statement of Purpose and in the Information For Our Residents pack, which are provided to residents. Residents said that any concerns that they raise are addressed and that someone is available to speak to informally if they have a concern. Residents state that they know about the complaints procedure. The AQAA sates that there have been 5 complaints in the last 12 months, that all of these were resolved in 28 days and 2 were upheld. The home has a complaints log where each complaint is recorded along with the action the home is taking to investigate plus the final outcome with any action plan needed. The records also give details of whether the complaint was substantiated, partly substantiated or not substantiated. These records are signed and dated by the staff member completing them. Staff receive training in safeguarding vulnerable adults. A record of this training is maintained. The home also has literature on the subject entitled, Elder Abuse Awareness. Environment Residents described how much they like the environment. One person said that the furniture and fittings are of a very good quality and the gardens are well maintained with flowers and shrubs. Comment was made that the residents can bring their own furniture which they had. Residents can also have their telephone line to their room. Bedrooms were seen to be clean with furniture and fittings intact. The home has a hairdressing room and several communal areas for residents to use, including seated areas in the garden. Staffing The staffing levels were looked at by discussions with staff, manager and the residents, as well from the details of the staff duty roster. The home aims to provide the following staff: 8am to 4pm 2 Registered General Nurses Care Homes for Older People Page 5 of 10 (RGN) and 1 RGN 4pm to 8am the next day, 8 am to 2pm 7 care staff and 5 or 6 care staff 2pm to 8pm. Night time care staff consists of 3 waking care staff and a RGN. The staff rota showed that these staffing levels were being maintained. In addition to the above staff hours are the following: activities organiser 30 hours a week, a receptionist, laundry staff for 42 hours a week, a chef each day plus 2 kitchen assistants each day, a waitress from 11am to 6.30pm each day, cleaning staff for 102 hours a week, maintenance person 2 days a week and a gardener 8 hours a week. Staff said that the home has sufficient numbers of staff to meet residents needs. Residents described the staff as exceptional and most agreeable. Comment was made that staff respond promptly when they ask for assistance using the call points in their rooms. Staff are said to work well as a team. The home has a training programme and matrix displayed in the managers office, which has details of each persons mandatory training courses, including first aid, dementia care, safeguarding adults, basic food hygiene, medication management, customer service, moving and handling, health and safety and fire safety. The home has 31 care staff and 15 of these have the National Vocational Qualification level 2 or above in care. Staff confirmed that they have access to a variety of training courses and that they receive regular supervision and appraisals of their performance. Recruitment procedures were looked at. Records show that the home checks the nurses registration status with the Nursing and Midwifery Council (NMC). The manager states that nurses are supported by the organisation to receive training to maintain their registered status. Recruitment procedures were looked at for 1 person who has recently started work at the home. The records show that the required checks were carried out before the person started work, including obtaining 2 written references and a criminal record bureau (CRB) check. The persons records showed that an induction was completed and recorded to prepare the person for the role. Management and Administration Staff described the manager as approachable and that they get support from a member of the management team. Staff confirmed that they meet with their manager for supervision. The manager has completed qualifications in management in care. A resident described the homes management as well organised. A resident confirmed that his/her views are taken account of and described how the service carried out a detailed survey of his/her views about the home.
