What the care home does well Standards of care are good and observations of staff and residents together identified they enjoyed good interactions and respected each other. People living in the home confirmed that they got on well with the staff and were well looked after. Comments included: "These staff are wonderful, nothing is too much trouble for them" "Staff are kind and caring and treat us very well" "My carers know what I want and when I want it they are great". The social atmosphere in the home was warm and relaxed. Staff were observed supporting residents around tasks of daily living such as giving drinks and assisted feeding at meal times. Staff were also observed assisting residents to take part in activities and interests which they all seemed to enjoy. Discussion with staff identified that staff feel supported by the home manager and work well as a team. Staff said the manager is open and transparent and works hard to meet and improve standards in the home. Residents are only admitted to the home following a thorough assessment to make sure the home can meet assessed needs. All residents have care plans, which are reviewed monthly and contain a pen picture and social history followed by a list of care interventions. The home is well maintained so people can live in safe, clean, comfortable environment. Staffing in the home is fairly stable with minimal turnover. Recruitment practices are thorough so that staff, who work at the home are fully checked before they are employed at the home to make sure they are suitable to work with the people who live there. Comments from relatives and other people who visit the home were most positive and all said the care in the home was good. Comments included: "All staff are very caring and provide excellent care to my mother" "My mum cannot remember much now but the staff look after her and make sure she gets what she wants. The quality rating for this service is two stars. This means that people who use the service receive good quality outcomes. An unannounced visit was carried out over an eight -hour period, and the methods used to assess the service, included discussion with the people living in the home, their representatives, the registered manager and staff. To ensure that the home is managed in the residents best interests records on staffing and health and safety were looked at. Five care files were randomly selected and assessed in detail to ensure that they contained full information to enable staff to have sufficient guidance to support each individual. Five staff files were looked at to ensure that the recruitment and selection process ensures the protection of the people living in the home. A tour of the premises was carried to assess the environmental standards of the premises and activities of daily life for the people living in the home were observed. The manager completed the Annual Quality Assurance Assessment (AQAA) and returned it to us prior to the inspection. This is a self-assessment document, which gives information about how the home has improved in the last twelve months, plans for ongoing development of the service and barriers to improvement. Questionnaires were sent to staff and people who live in the home and other information since the last inspection was reviewed. Fourteen questionnaires were returned and comments from the surveys and the AQAA have been incorporated within this report. However whilst all documentation was accessible it was noted that several staff/residents personal files would benefit from having a front index and separate sections to enable relevant information to be quickly made available when required. Discussions with the home manager and administrator revealed that they had commenced a review of all administrative documentation and filing and storage systems for their own ease of reference. The home provides information so that new and prospective residents can make an informed choice about their future care. Staff, carry out Pre admission assessments to ensure care needs can be met. EVIDENCE: The home provides information for new and prospective residents to include a brochure, statement of purpose and service user guide. These documents explain the facilities, staffing and general ethos of the home. People living in the home and their representatives said they had been provided with lots of information about the home to include costs and were able to make informed choices about obtaining a room in the home. The manager had undertaken all three assessments and the documentation was clear and relevant. The assessment format covered background, care needs to include activities of daily living and capabilities, choices, interests with extra notes being added as required to cover diversity issues. The manager had requested copies of appropriate social work/heath care assessments and they were held with the pre admission assessment to ensure person centred care could be provided. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 7.8.9.10. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The health and social care needs of people living in the home are well managed and their privacy and dignity is respected. EVIDENCE: The care plans for three people living in the Dementia unit and three in the residential unit were examined. They were of a good standard and included relevant risk assessment so that individual care needs were identified. Records show that care plans were evaluated monthly to enable changing need to be identified and met. Care plans contained a good pen picture and social history of each individual and a detailed plan of care and support interventions. Two care plans in respect of residents who experienced dementia identified they presented challenging issues around the management of personal care Croxteth Park Care Home DS0000068321.V369444.R01.S.doc Version 5.2 Page 13 and general social interactions. There was evidence recorded on file to show the home had liaised well with health and social care professionals and the care plan in operation made all staff aware of the issues and of the care practices in place to manage certain situations. Care plans held signatures of the individual resident and /or their representatives to show that they had been involved in their plan and review of care. Records show that people who needed support from a range of heath care professionals such as nurses and GP’s received this support at an early stage and health care needs were monitored and reviewed as appropriate. Discussions with visiting health care professionals identified that the manager has established strong links with external health care providers and ensures that the health and wellbeing of the residents is maximised. The management of medication was checked and appeared well managed and medication administration recordings (MAR) sheets were clear and accurate. Medicines are received into the home on the MAR sheets and there is a return to pharmacy record. The manager advised that medication audits are carried out internally each week and also on a random basis by external sources. Records show that no residents are currently self-medicating. However the manager has a self -medicating risk assessment form so that any resident who wishes to self medicate can be assessed. Residents seen were appropriately dressed, clean and well presented. Residents said they choose their own clothes and staff assisted with their personal hygiene as required. Staff spoken with fully identified they understood the concepts of privacy and dignity and were observed utilising this knowledge via their care practices and interventions. Comments from residents and their representatives included: “All the staff of this home are good. They understand her needs and treat her with dignity and respect. They are marvellous”, “Just look at the way they are looking after him, they make him feel special, he loves them all” “I am treated very well, they care for me and ask me what I want and if I need anything they get it”. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 12.13.14.15. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. People have access to a good range of activities to enhance their daily life. EVIDENCE: An activity co-ordinator is employed at Croxteth Care Home and was observed interacting with people living in the home and carrying out an activities programme. A notice board in the foyer of the home showed photographs of some events that had recently taken place and identified that a celebration of the home being open for ten years was to take place in the very near future. Care records held information of the social care needs of the people living in the home and of how these needs were being met. Croxteth Park Care Home DS0000068321.V369444.R01.S.doc Version 5.2 Page 15 Discussions with staff revealed that they worked hard to make sure the social needs of the residents were met and they said that time allowing they assisted the activities co-ordinator with things like bingo, quizzes and social events. Staff, were observed chatting with residents and enjoying a laugh and a joke and he atmosphere in the home was most pleasant. Resident’s family and other visitors to the home said they felt staff provided good stimulation for the people living in the home and said that activities were a regular part of the daily living. Residents said they enjoyed the food and observations of them eating their lunch -time meal confirmed this. Menus show that the meals are varied and choice is provided at all times. Staff in the kitchen knew what peoples preferences were and also had full knowledge of specialist diets. The dining rooms in each unit present as comfortable and welcoming and staff were seen to provide discreet assistance to the people who need help to eat their meals. Some residents were able to advise what was the menu choice of the day whilst others were unable to recall this information. However the manager said that a laminated menu was put on each dining table at every mealtime to show what choices were available. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Residents concerns are listened to and acted upon and staff are fully trained in all aspects of adult protection EVIDENCE: There is a complaints procedure on show in the home a copy of which is provided to all residents and their representatives on admission. Residents and Relatives spoken with or who supplied feedback via the survey were aware of how to complain and said the home manager and staff were very approachable and responsive to any concerns or complaints. The complaint book was viewed and it was noted that the manager made an immediate written response to any concerns and complaints. This was followed by an investigation with outcome being detailed by letter to the relevant people. Croxteth Park Care Home DS0000068321.V369444.R01.S.doc Version 5.2 Page 17 The manager identified that she is fully aware of the local procedures for reporting any allegation of abuse and records show that she has utilised these procedures to alert the appropriate people as and when required. Staff spoken with had good understanding of what constitutes abuse and how to identify if abuse had occurred. They said they had undertaken generaltraining in abuse and this training was an ongoing process in the home. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 19.26. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The home continues to benefit from ongoing refurbishment to enable the residents to live in a safe well-maintained environment. EVIDENCE: The home is clean and well maintained internally so that residents can enjoy living in the home. There is disability access to the front and rear of the home and the gardens have been well developed to enable residents to access fresh air and hopefully some sunshine. The manager advised that the bedrooms and communal areas of the home have befitted from replacement of furnishings and observational practices and Croxteth Park Care Home DS0000068321.V369444.R01.S.doc Version 5.2 Page 19 records show that the general housekeeping policies and procedures in place are carried out to a high standard. The premises are purpose built to enable all living space to be provided on the ground floor with easy to read signage for toilets lounges and bedrooms. This assists residents with their orientation. Residents and their representatives said they had seen great improvements in the general appearance of the home to include new pictures on the walls and bright well maintained grounds. All bathrooms are used and have aids and adaptations in place as appropriate. The kitchen and laundry area of the home presented as clean tidy and well managed. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. This judgement has been made using available evidence including a visit to this service. Staff are provided with ongoing training and are provided in sufficient numbers to provide a competent service. EVIDENCE: Five staff recruitment records were seen. These included all the necessary documentation and security checks so that people staff are suitable to carry out safe care and support services for the people living in the home. Staff are highly regarded by the residents and their representatives and observations of staff interacting with residents showed they were sensitive and supportive at all times. The staffing levels in the home appeared satisfactory and staff advised that they worked well together as a team. Records show that staff turnover is low and as a consequence residents are provided with a consistency of staff whom they know and who know them. Croxteth Park Care Home DS0000068321.V369444.R01.S.doc Version 5.2 Page 21 Information provided by the manager on the pre inspection document states that the home has an ongoing staff- training programme and staff are encouraged to gain NVQ qualifications. Discussions with staff confirmed this and staff advised that they are well supported by the manager to maximise their potential and build on their skills and knowledge base. Staff interviewed advised that following an induction programme they had been provided with ongoing mandatory training and were also offered specialised training- for example in dementia care. People living in the home said staff were good and looked after them well. Comments included: “Staff are always around to help me, they know how to look after me” “The staff are well trained to care for us here” “They make me feel safe and well”. Resident’s relatives and friends spoken with were positive when talking about the staff and comments received confirmed that staff are well thought of in respect of the way they carry out their roles. Comments included: “Staff here are second to none, they are great” “The staff really care about the people in here, they cannot do enough for them” “They cared for my mother so well, they knew just what she wanted and made sure she got it”. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 31.33.35.37.38. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The manager leads by example and ensures the home is run in the best interest of the residents. EVIDENCE: The home manager Lorraine Disley is knowledgeable and experienced in the care of older people and has completed her registered managers award. During the visit she was able to demonstrate her knowledge and understanding of the various care needs of the people living in the home to include dementia care. Croxteth Park Care Home DS0000068321.V369444.R01.S.doc Version 5.2 Page 23 Staff interviewed and comments received from questionnaires identified that staff respect the manager for her direct approach and open door policy and comments included: “The manager leads by example, she would not ask us to do anything she would not do herself” “She has made me feel good about myself. She has encouraged me to take on more responsibility and gain more skills. She is most supportive” “She manages well, she knows what’s going on and does not have her head in the clouds” “She cares about this home, the residents and their families and the staff. She does all she can to make the home the best it can be”. The manager arranges staff and residents meetings on a regular basis to make sure people can have their say about how the home is run. Minutes of meetings confirmed that agendas are provided and full feedback is recorded on file. The home carries out an annual customer survey, which includes attention to relative’s feedback, and internal audits are undertaken both by the home manager and a regional manager of the organisation. The manger completed and Annual Quality Assurance Assessment (AQAA) prior to the inspection. This document contains information about the current service and any plans for the future. The AQAA contained all the information we requested and identified the manager has a clear vision of where the service needs to develop and of how this will be achieved. Records of the management of resident’s personal allowances were clear and showed that they were correctly managed. All transactions had receipts and signatures recorded. The homes policy statements include good practice guidelines around receiving gifts and the safeguarding of residents finances. All documentation was accessible however it was noted that several staff/residents personal files would benefit from having a front index and separate sections to enable relevant information to be quickly made available when required. Discussions with the home manager and administrator revealed that they had commenced a review of all administrative documentation and filing and storage systems for their own ease of reference.
