The home used to belong to Crystalline Care Management Ltd. It has been reregistered on the 5th August under Maceile Care Management Ltd. This organisation specialises in the provision of services for people with mental health needs. It has two

The service users guide for the home states: Our primary aim is to promote independence, self-determination, and to contribute to the rehabilitative process. This enables each individual person to attain their optimum quality of life, and move on to more independent living, in a home of their own. The home is situated on the busy Willesden High Street above shop premises. Access to the service is via a door that is on the street-level. Accommodation for residents is provided in well-furnished and appropriately sized bedrooms, each with en-suite toilet and shower, and a kitchenette. The bedrooms are similar to studios. They are found on the lower ground, first and second floors. The home also benefits from a main kitchen, lounge, managers office and conservatory that are found at ground floor level. The home is close to Willesden Green tube station and with easy reach of public transport links and local amenities. There are no parking facilities in front of the home but paid and residents parking is available on the smaller roads near to the home. At the time of inspection there were five residents in the home. The weekly fees for each resident were 980 pounds. What did we do for this annual service review? We looked at all the information that we have received, or asked for, since the last key inspection (25/11/2008). This included: - The annual quality assurance assessment (AQAA) that was sent to us by the service. The AQAA is a self-assessment that focuses on how well outcomes are being met for people using the service. It also gave us some numerical information about the service. - Surveys returned to us by people using the service and from other people with an interest in the service. - Information we have about how the service has managed any complaints. - What the service has told us about things that have happened in the service, these are called notifications and are a legal requirement. - The previous key inspection. - Relevant information from other organisations. - What other people have told us about the service. What has this told us about the service? The home sent us their annual quality assurance assessment (AQAA) when we asked for it. It was clear and gave us all the information we asked for. We looked at the information in the AQAA and our judgement is that the home is still providing a good service and that they know what further improvements they need to make. The home told us about the following improvements they have made since the last key inspection. Promoting the idea of residents being able to have more choice in where they wish to live, more importantly how they wish to live and who they wish to live with. A service user satisfaction questionnaire has recently been sent to all residents and action taken on results. Residents having an opportunity to express there individual political view by being registered to vote at local and general elections. We have improved the records that are kept of all medicines received, administered and disposed of to ensure that there is no mishandling or mistakes made. We have improved in the area of responding to complaints. We now have a maintenance contract with the owner/landlord and a maintenance policy. Training and development of the staff has also been positive this last year with four staff starting and almost now finished there NVQ level 3 in care. We have received five completed surveys from people using the service. The surveys have been positive. People using the service made the following comments. Staff help me to do my shopping, cleaning, listen to my problems and support me well. Another comment made, The staff in the care home are very supportive. Diamond Home is a very good home, staff are very caring and helpful. Five completed staff surveys have been returned to us. One of the comments made was: Diamond Home is very caring, well organised, well structured home, which provides excellent rehabilitation for our clients. The home continues to let us know about things that have happened since our last key inspection and they have shown that they have managed issues well. They work well with us and have shown us that their service continues to provide good outcomes for Annual Service Review Page 4 of 6 the people who use it. What are we going to do as a result of this annual service review? We are not going to change our inspection plan, and will do a key inspection by 1st November 2011.
