What the care home does well The management team has made many improvements to ensure medication practices in the home are made safe. All GP prescriptions are now checked for accuracy and completeness each month. The management team carry out regular checks of medicines and medication records each month. All staff are carrying out medication safety update training. Advice has been taken from the NHS PCT pharmacy team on updating medication policies and procedures. People`s own preferences for taking medicines are recorded and respected. What the care home could do better: Guidance should be kept in care plans on how to give medicines prescribed to be given as and when needed. All hand written entries and changes to MARs should be signed, dated and checked for accuracy. New MARs should include a record of the quantities of medicines brought forward from the previous month. Staff should use the designated MAR chart omission codes consistently. One building known as the Lodge can accommodate 40 people. 11 people are on one unit, which is for people within the catagory of old age with dementia and the other 29 bedrooms are Care Homes for Older People Page 2 of 8 1 9 1 1 2 0 0 9 Brief description of the care home for people within the category of old age with general Nursing needs. The other unit known as the Annex can accommodate people within the category of old age with dementia that require nursing care. This unit can accomodate up to 21 people. Both units have safe accessible outdoor space. The home is situated in Stainforth close to local amenities. The fees at Dr Anderson Lodge at the time of the inspection ranged from £398.72 - £600.17. Care Homes for Older People Page 3 of 8 What we found: The reason for this inspection was to find out what actions the home had taken to improve medication practices following the last full inspection of the home in November 2009. A pharmacist inspector spent 4.75 hours in the home looking at the medication ordering, storage, handling and disposal arrangements, examining medication administration record charts (MARs) and speaking to staff. The outcome is as follows. We found many improvements in medication ordering, receipt, checking and record keeping. Most medication management processes in the home now follow best practice and staff are aware of current professional guidance. Some additional improvements in medication record keeping are recommended to ensure a complete record can be maintained of all medicines used in the home. The outstanding medication requirements issued at previous inspections are now met. The management team is clearly commited to ensuring all medication practices throughout the home continue to be safe. Authorised staff are receiving appropriate update training in medication safety and regular checks of staff practice now take place. The staff are also following good practice advice received from the NHS PCT pharmacist and they told us they welcome the further support available from this source. We examined the 45 current MARs for accuracy and completeness and found no significant gaps. Together with the checks made by the management team, this indicates that people living in the home can expect to receive their prescribed medicines correctly. The personal preferences of people living in the home had been recorded and were respected by staff when giving medicines to people. This good practice reduces the risk of doses being refused or omitted. We found good records for the use of anticoagulant medicines which showed how dose changes recommended by the NHS were being followed correctly. Some hand written changes and entries on the MARs were not signed or dated and they lacked a witness signature. Hand written entries and changes to MARs must be clear, detailed, complete and safe arrangements must be made to ensure staff know the reasons for the changes. A second person should always check these entries for accuracy so that all staff follow the changes on the MARs correctly. We also found that some care plans did not include adequate information about how and when to give medicines prescribed when required. This information will ensure staff give these medicines consistently and correctly as intended. There was also some inconsistency by staff in the use of omission codes when medicines are not given or when offered but not needed. All staff should follow best practice guidance in this respect to make sure the MARs are an accurate record of why medicines are not given. Not all MARs indicated the quantities of medicines brought forward from the previous month. Without this information, it is difficult to check that medicines are being given correctly or to be sure that sufficient medicines are available until the next delivery is due. Medication ordering, receipt, storage and disposal arrangements in the home are good. The GP prescriptions are checked each month before the medicines are delivered. This helps to make sure the home is aware of any prescription changes and means staff can check that sufficient quantities of everyones medicines have been prescribed each month. Controlled drugs procedures and records in the home now follow accepted best practice and no new discrepancies were found. We were, however, concerned to hear that Care Homes for Older People Page 4 of 8 the homes procedures for controlled drugs had not always been correctly followed by visiting district nurses. This information has now been shared with the NHS PCT to ensure all visiting nurses uphold local procedures for handling and recording the use of controlled drugs at all times when visiting care homes.
