Excelsior is a residential home providing care and accommodation for up to 19 older people, with capacity for people in the dementia and physical disability care category. Mrs Lawrence manages the home and jointly owns it with her husband. The single room accommodation is arranged over two floors, with access to the first floor via a stair-lift. All rooms have an en-suite facility. Communal areas consist of two lounges, one being a sunroom, and a separate dining room. The home is located in Mitchell Avenue, Ventnor and is less than a quarter mile from the shops and amenities of the town. The building itself is a period town house, with off-road parking to the front, and ramped access into the home. Outside at the rear is a large decking area with seating for the residents, affording views over the English Channel. Rooms at the front have countryside views.

What the care home does well All information considered as part of this random inspection would indicate that the home continues to provide an excellent service to people who live there. Comment cards and discussions with people who live at the home and a relative indicated that service users are all very happy with the service provided at the home. Several stated that they would recommend the home to anyone requiring residential care. Service users and relatives added comments including `quiet and content atmosphere is good for my relative`, and another stated `my relative is very happy and safe here, excellent care`. Another relative added to a survey `creates a friendly, caring, homely atmosphere - showing genuine interest and empathy consistently`. Staff enjoy working at the home with additional comments from staff noted in the previous section of this report. Staff also commented on surveys that the home has very high standards and that these are monitored and ensured by the registered manager. Staff also commented that the registered manager is approachable and supportive. Care Homes for Older People Page 3 of 9 What we found: A random inspection of the service was undertaken on the 12th May 2010. The inspection was undertaken by one inspector and lasted three hours commencing at 10am and being completed at 1pm. The homes registered manager was present throughout the inspection visit. The home was rated excellent following a key inspection in May 2007 and there was no indication that it was not continuing to provide an excellent service in annual service reviews (ASR) completed by the commission in 2008 and 2009. All information held by the commission indicated that the home is continuing to provide an excellent service and a random inspection was undertaken to confirm this view. The registered manager provided access to all documents requested and communal areas of the home were viewed. Prior to the inspection visit the registered manager completed an annual quality assurance assessment (AQAA) providing details as to how the home is ensuring the quality of care it provides. The inspector was able to speak with staff on duty, people who live at the home and a visitor. Also considered in this report are ten surveys received from people who live at the home and five surveys received from staff. Also considered are notifications received from the home about things that have occurred in the home since the previous ASR. As part of the inspection we looked at one care plan for a person who had been living at the home for a number of years. The care plan viewed provided detailed and individual information as to how the service users needs should be met. The care plan had been reviewed monthly by the registered manager and there was evidence that where needs had changed the care plan had been updated. The care plan also contained risk assessments relevant to the persons needs. The daily records completed by care staff showing how care had been provided were also viewed and showed that care as per the persons care plan was being provided. The service user whose care plan was viewed was met in the homes lounge. The medication records and supplies of medication for the person whose care plan was viewed were also viewed. Care staff confirmed that they had undertaken medications training and were able to discuss how medication was managed in the home. The home has supplies of all the medications required by the person. The medication administration records were well completed although on the day prior to the inspection a staff member had not signed for a medication. Medication stocks would indicate that this had been given. During the inspection the staff member concerned was contacted and confirmed that they had administered this medication and came to the home to sign for this on the record sheets. The inspector spoke with a number of service users and a relative in the homes lounge. All stated that they were happy with the way their health and personal care needs were met at the home. Care Homes for Older People Page 4 of 9 Ten service users completed surveys in February 2010. Eight stated always and two usually that they receive the health and personal care they require. Five staff completed comment cards and they all stated that they always receive up to date information about the people they care for and that they felt there were always enough staff to meet the individual needs of the people who live at the home. The home has an activities staff member who was on duty during the inspection visit. The staff member confirmed that they have the necessary equipment to undertake a range of group and individual activities. The activities staff member is also completing some quality assurance work via individual discussions with service users to gain their views about various aspects of the home. The registered manager described how information gained will be used to ensure that the service provided meets peoples needs and expectations. The registered manager stated in the homes AQAA that no formal complaints had been received. This was confirmed during the inspection visit. Discussions with service users and a visitor confirmed that they would raise any concerns and felt happy that the home would sort out any issues. Nobody had any concerns or complaints at the time of the inspection visit. In the ten surveys received from service users, six stated that they were aware of how to complain and the remaining four were not sure, however all stated that there was someone they could talk to if they were unhappy with the service. Some naming the registered manager as the person they would talk to. The communal living areas of the home, the lounge, dining area and sun lounge were viewed with the registered manager. The communal areas are spacious and the sun lounge provides excellent views of the English channel and access out onto a decked patio area. Furniture in the communal areas is appropriate for the people who live at the home. There were no unpleasant odours noted during the inspection visit. At the time of the inspection visit staff on duty corresponded to those on the homes duty rota. Care staff stated that they felt there were sufficient staff to meet peoples needs and this was also the view of the people who live at the home. Positive comments were made by service users about the homes staff stating that they were all very nice. Care staff added comments on surveys stating that they enjoyed working at the home including I am lucky to work here, I like working here and everyone is nice, another stating love the environment and team I work in. I am supported and helped when and where it is needed. Excellent place to work. The registered manager showed the inspector information that confirmed that staff have completed all mandatory and additional training to give them the skills they need to meet peoples needs. Care staff confirmed that they have received training relevant to their role both during the inspection visit and on surveys received. The registered manager confirmed that sixty per cent of the homes care staff have an NVQ of at least level 2 in care. The recruitment records for one new staff member were viewed and confirmed that all the necessary pre-employment checks had been undertaken to ensure that only suitable people are employed at the home. Discussions with a new staff member also confirmed that the home has appropriate recruitment procedures. Care Homes for Older People Page 5 of 9 The home continues to be managed by the registered manager who is also one of the providers (owners) of the home. The registered manager confirmed via records viewed, that they have completed updates and all relevant training required. Positive comments were made on staff surveys in respect of the registered manager including good management of the home, manager will constantly remind us that the residents deserve the best care and that we are here because we care and that its not just a job. Management are very supportive and friendly towards everyone in the home.
